1. Assessment:
1. Assess the effectiveness of the needs for assistance with
perineal care
2. Determine the self-care abilities in completing perineal
care.
3. Assess the presence of irritation, excoriation,
inflammation and swelling
4. Assess for allergy or sensitivity to drugs used
5. Assess woman’s knowledge of perineal hygiene in terms
of its importance, preventive measures to take and
common problems.

1. Review order for procedure and nursing care
plan.
2. Explain the procedure to the woman of what you are
going to do, why it is necessary and how she can
cooperate.

3. Implementation:
1. Greet the woman and introduce yourself.
2. Introduce your self and identify patient
using two identifiers; like name and
birthday.
3. Assess environment for safety, make sure that equipment
needed is working properly.
4. Perform hands hygiene using warm water, apply clean
gloves and ensure that the patient is not allergic to the
latex.
5. Provide privacy to the patient, close room,
door and windows.
6. Place woman in dorsal recumbent position
7. Place waterproof pad under patient’s buttocks
8. Drape patient with bath blanket placed in shape of
diamond.
9. Lift lower edge of bath blanket to expose perineum.
10. Fold lower corner of bath blanket up between patient’s
legs onto abdomen.
11. Wash and dry patient’s upper thighs.
12. Wash labia majora. Use nondominant hand to gently
retract labia from thigh, with dominant hand wash
carefully in skin folds.
13. Wipe in direction from perineum to rectum.
14. Repeat on opposite side with separate section of
washcloth.
15. Rinse and dry area thoroughly.
16. Gently separate labia with non dominant hand to expose
urethral meatus and vaginal orifice. With dominant hand,
wash downward from pubic area toward rectum in one
smooth stroke.
17. Wash middle and both sides of perineum.
18. Use separate section of cloth for each stroke.
19. Clean thoroughly around labia minora, clitoris and
vaginal orifice.
20. Avoid placing tension on indwelling catheter if present
and clean area around it thoroughly.
21. Rinse thoroughly. Dry thoroughly from front to back.
22. Apply betadine solution if necessary
23. Remove soiled blanket and change linen if necessary.
24. Make patient in a comfortable position.
25. Do after care of all the equipments.
26. Remove screen and tidy up the unit
27. Wash hands thoroughly

4 Evaluation:
1. Evaluate the effectiveness of the perineal care techniques
2. Document the procedure in patients plan of care as well
as specific observation of any abnormal findings and
patient’s comfort level.
3. Record the procedure with date and time and observation
made.
