effettuato in data 26/04/2022 il test rapido (nome test "One Step Test for SARS-CoV-2 Antigen (Colloidal Gold)", prodotto
da Getein Biotech, Inc.) per la rilevazione dell’antigene virale SARS-CoV-2, Presso Farmacia per motivo Motivo non
sanitario.
Il suddetto test, in data 26/04/2022 ha dato esito

NEGATIVO.

Il destinatario del presente attestato è, comunque, tenuto ad osservare le misure generali di igiene e prevenzione del
contagio e può rientrare o vivere in comunità senza ulteriore formalità solo se non sono presenti sintomi compatibili
COVID-19 e se non sono stati emessi provvedimenti diversi a suo carico da parte dell’autorità sanitaria competente.

L’esito negativo al test antigenico rapido non esclude la possibile presenza di infezione da SARS-CoV-2 e, pertanto, il Suo
medico curante o il Dipartimento di Prevenzione dell’ASL territorialmente competente potranno eventualmente adottare
eventuali provvedimenti ivi compresa l’eventuale esecuzione di test molecolare di verifica.

Si pone in evidenza che il destinatario del presente attestato dovrà contattare il proprio medico curante in caso di
insorgenza di sintomi Covid-19 compatibili.

Il presente attestato NON è equivalente e NON sostituisce la Certificazione Verde COVID-19, anche detta Digital Green
Certificate rilasciata dalla Piattaforma Nazionale – DGC, e recuperabile:

a. accedendo al portale della Piattaforma Nazionale‐DGC (www.dgc.gov.it);

b. tramite il Fascicolo Sanitario Elettronico (FSE);
c. tramite APP IO e APP IMMUNI;
d. con l’aiuto dei Medici di Medicina Generale, Pediatri di Libera Scelta e Farmacie.

carried out on 26/04/2022, the rapid test (test "One Step Test for SARS-CoV-2 Antigen (Colloidal Gold)", produced by
Getein Biotech, Inc.) for the SARS-CoV-2 viral antigen research, in Presso Farmacia for Motivo non sanitario.
The above test, on 26/04/2022 gave results

NEGATIVE.

The addressee of this attestation is, however, required to observe general hygiene measures and prevention of contagion
and may return to or live in the community without further formalities only if there are no Covid-19 compatible symptoms
Covid-19 and if different measures have been issued to its burden by the competent health authority.

The negative result in the rapid antigen test does not exclude the possible presence of SARS-CoV-2 infection and therefore
your doctor or the Department of Prevention of the Territorial ASL may take any measures including any molecular
verification tests.

It is emphasised that the addressee of this attestation should contact his/her doctor in the event of compatible symptoms
Covid-19.

This attestation is NOT equivalent and does NOT replace the COVID-19 Green Certification, also called Digital Green
Certificate issued by the National Platform - DGC, and recoverable:

a. by accessing the portal of the National Platform-DGC (www.dgc.gov.it);

b. through the Electronic Health Record (FSE);
c. through APP IO and APP IMMUNI;
d. with the help of General Practitioners, Pediatricians and Pharmacies.
