OF A CONTRACTOR

I, ATTY. ERIC R. CORTES, Filipino, of legal age and with office address at 6 th
Floor, Vernida IV Building, 128 L. P. Leviste St., Salcedo Village, Brgy. Bel-Air, Makati
City, after having duly sworn do hereby certify that:

1. I am the duly elected and qualified Corporate Secretary of ELEV8

HOLDINGS, INC. (hereinafter, ‘EHI’ or ‘the Corporation’), a domestic corporation
duly organized and existing under Philippine law with principal office address at the 6th
Floor, Vernida IV Building, 128 L.P. Leviste St., Salcedo Village, Brgy. Bel-Air, Makati
City;

2. As custodian of corporate records and recorder of corporate affairs, actions and

transactions of the Corporation, I hereby attest, confirm and affirm that the Corporation
(through its Governing Board) did not hire or engage the services of a ‘contractor’ for
the purpose of performing renovation works at the office space being leased by EHI
located at the ‘Penthouse’, Vernida IV Building, 128 L.P. Leviste St., Salcedo Village,
Brgy. Bel-Air, Makati City;

3. Instead, the aforementioned renovation will be carried out by construction

workers that are duly employed by EHI which will exercise supervision and control over
such personnel; and

4. I am executing this Affidavit to attest to the truth of the foregoing facts, for the
information and guidance of competent authorities concerned and for whatever legal
purpose/s it may serve.
