UTILIZZO
Il kit fornisce il materiale necessario per la genotipizzazione di polimorfismi a singolo nucleotide (SNPs) presenti nel gene dellinterleuchina 28B (rs12979860 e rs8099917) associati alla risposta alla terapia interferonica in pazienti affetti da HCV (genotipo 1) .
Il prodotto compatibile con gli strumenti Rotorgene 3000, 6000 e Q, i sistemi di estrazione su colonnina (cod NLM AA1001, QIAamp DNA blood Mini kit Qiagen) ed Estrazione Rapida (cod. NLM AA898), sistemi da ordinare a parte. SPECIFICITA
La specificit del test del 100%
INTRODUZIONE
Circa 170 milioni di persone nel mondo sono affette da HCV e tale infezione la causa principale dellepatite cronica che spesso evolve in cirrosi o carcinoma epatocellulare. Attualmente la terapia standard nel trattamento dellepatite rappresentata dallassociazione tra peg-interferone e ribavirina (PEG-RBV), ma solo nel 50% dei pazienti affetti da HCV di genotipo 1 (il pi frequente nella popolazione caucasica) si osserva una remissione virale sostenuta (SVR); oltretutto questi farmaci comportano gravi effetti collaterali che richiedono talvolta la sospensione o la modifica della dose somministrata. Molteplici studi hanno dimostrato che lesito di tale terapia influenzato sia dal genotipo virale che da cause legate allospite. Lidentificazione di fattori predittivi della risposta terapeutica quindi si presenta come un valido approccio per la determinazione dei candidati pi adatti a questo tipo di cura; a tal proposito, studi di associazione genome-wide hanno evidenziato come la variabilit genetica del paziente possa influenzare la risposta al trattamento farmacologico. Studi indipendenti hanno individuato una regione del gene IL28B sul cromosoma 19 che codifica per la proteina INF-3 implicata nella risposta immunitaria contro HCV. In particolare in questa regione sono emersi due polimorfismi (rs12979860 e rs8099917) fortemente associati alla variabilit interindividuale.

Molteplici studi hanno dimostrato che le due varianti correlano con unaumentata SVR in pazienti con infezione da HCV di genotipo 1. La riduzione di HCV-RNA dopo 7 giorni di terapia si dimostrata pi pronunciata in pazienti con C/C (rs12979860) o T/T (rs8099917) rispetto a soggetti portatori delle altre due varianti. I due SNPs sono in linkage disequilibrium (D_ = 1, r2 = 0,44), ma il C/C (rs12979860) meno comune (43% vs 71%) rispetto al genotipo T/T (rs8099917). I portatori degli alleli favorevoli mostrano dopo un mese di terapia, elevate percentuali di SVR rispetto a quelli con CT o TT (rs12979860) e GG (rs8099917); la frequenza del genotipo GG di rs8099917 inoltre significativamente pi elevata nel gruppo dei soggetti non-responder (NR).

Ci si verifica anche e nonostante la presenza di alta carica virale al basale, et elevata o fibrosi epatica grave. La clearance virale sembra perci significativamente influenzata dai polimorfismi indipendentemente dai fattori predittivi tradizionali. In particolare, gli individui portatori degli alleli protettivi in questo locus genetico hanno una probabilit di eliminare l'infezione del virus doppia rispetto ai non portatori, e, anche se non arrivano ad eradicarlo, rispondono alla terapia in modo molto pi efficace. Una valutazione di questi SNPs in combinazione con gli altri fattori predittivi tradizionali di risposta terapeutica possono permettere di suddividere i pazienti in cosiddetti non responder, per i quali dovr essere valutata la possibilit di somministrare terapie alternative anche in base ai nuovi farmaci antivirali presenti, e pazienti invece responder con genotipo favorevole, ai quali pu essere somministrata la terapia standard magari personalizzandola in termini di durata.

In assenza del bersaglio non avviene il trasferimento di energia; la quantit di coppie di sonde ibridate ed il conseguente segnale luminoso aumentano proporzionalmente al prodotto di PCR. Le sonde di ibridazione permettono di identificare varianti della sequenza bersaglio mediante la curva di dissociazione (melting curve), sottoponendo lamplificato ad una rampa crescente di temperatura con conseguente distacco delle sonde dal DNA. Se la sonda in 5 complementare al filamento mutato di DNA, libridazione tra i due sar pi stabile rispetto al duplex tra la sonda medesima ed il filamento che non porta la mutazione (single point mismatch), con conseguente picco di dissociazione ad una temperatura pi elevata. Mediante lanalisi della curva di melting si possono pertanto distinguere i tre genotipi in base alla posizione dei picchi: singoli a due diverse temperature, per i campioni omozigoti (normale/mutato); doppio per leterozigote.

PRECAUZIONI
I campioni vanno manipolati seguendo le buone pratiche di laboratorio e devono essere considerati come pericolosi in quanto potenziale fonte di infezione. La procedura va eseguita utilizzando le buone pratiche di laboratorio ed i comuni dispositivi di protezione individuale. Utilizzare guanti monouso per ogni passaggio Tutti i consumabili (puntali e provette) devono essere sterili. I puntali devono avere il filtro per evitare la contaminazione delle pipette. Utilizzare un nuovo puntale ogni volta che viene dispensato un volume. Eliminare il materiale monouso utilizzato, i guanti indossati e tutti i reattivi come rifiuti speciali. Non mangiare, bere, fumare o applicare cosmetici nelle aree preposte allesecuzione del test.

Se vi esposizione di occhi, cute o mucose alle sostanze utilizzate, lavare abbondantemente con acqua e contattare al pi presto un medico. E opportuno assicurare una temperatura il pi possibile costante ed uniforme in laboratorio ed evitare di posizionare gli strumenti in prossimit di fonti di calore/raffreddamento che possano comprometterne il corretto funzionamento.
AMPLIFICAZIONE
IMPORTANTE! Prima di iniziare la seduta, verificare la modularit di utilizzo del kit nel paragrafo Componenti del kit. ATTENZIONE: Lutilizzo di campioni di sangue o di estratti che abbiano subito ripetuti scongelamenti o unimpropria conservazione pu pregiudicare lesito positivo del test. Mantenere in ghiaccio tutti i reagenti per lamplificazione durante lesecuzione dellintera procedura. Evitare ripetuti scongelamenti delle mix. Le sonde contenute nella Mix 2 sono fotosensibili: evitare lesposizione prolungata alla luce. Utilizzare solo provette da PCR dedicate: Provette da 0,2 ml con tappo piatto per RotorGene 3000, 6000 o Q. Impostare il profilo termico (vedi paragrafi successivi) prima di preparare le mix. Preparare la mix di amplificazione come segue:

Si consiglia di preparare la mix di amplificazione per un quantitativo di test pari al numero di campioni +1 (per n 10) o + 2 (per n >10). Miscelare la soluzione ottenuta evitando la formazione di schiuma. Dispensare 21 l di mix di amplificazione in ciascuna provetta precedentemente marcata . Aggiungere 4 l di DNA (La concentrazione del DNA estratto da analizzare deve essere di circa 30-40 ng/l). Se si effettua una seduta mista si consiglia di posizionare le provette nel rotore nel seguente ordine: rs12979860 poi rs8099917.

ATTENZIONE:
Se la tipizzazione riportata nel Report finale non corrisponde ai picchi rappresentati graficamente bisogna alzare la soglia in modo da escludere dallanalisi il segnale di fondo, eliminando cos le eventuali interferenze che falsano il risultato. Se lampiezza dei picchi non elevata si pu ripetere lo step di melting sugli stessi amplificati aumentando il gain: a) selezionare licona New per effettuare un nuovo esperimento b) nella funzione Profile impostare il profilo termico come segue (durata 15 minuti circa):

ATTENZIONE
Se si utilizzano campioni di sangue o DNA estratto scongelati pi volte o conservati scorrettamente si potrebbero ottenere dei picchi piuttosto bassi o addirittura lamplificazione potrebbe non avvenire. Si consiglia pertanto lutilizzo di campioni freschi o congelati non pi di una volta.
SICUREZZA
Non mangiare, bere, fumare o applicare cosmetici nelle zone di lavoro. Utilizzare guanti nel maneggiare reagenti e campioni. Considerare tutti i campioni biologici come potenzialmente capaci di trasmettere infezioni. Pulire e disinfettare tutte le superfici che siano venute a contatto con i campioni.

INTENDED USE
This kit is an in vitro diagnostic test for genotyping single nucleotide polymorphisms (SNPs) in the Interleukin 28-B gene (rs12979860 e rs8099917) involved in the different response to interferon-based therapy of HCV (genotype 1) infected patients. This product is compatible with Rotorgene 3000, 6000 and Q (Corbett Research), column based extraction kit (cod NLM AA1001, QIAamp DNA blood Mini kit Qiagen) and with the NLM Rapid Extraction (cod. NLM AA898).
SPECIFICITY
Test specificity 100%.
INTRODUCTION
Over 170 million people worldwide are infected with the hepatitis C virus (HCV) and it has become the most common cause of liver-related death in the United States. Although the standard-of-care treatment for chronic HCV infection is interferon (IFN)-based therapy, only about half of patients with genotype 1 infection achieve sustained virologic response (SVR) to peg-interferon/ribavirin. Recent genome-wide association studies have identified single-nucleotide polymorphisms (SNPs) in or near the interleukin-28 gene that help define variants in treatment response. The IL28-B gene on chromosome 19 codes for INF-3, a protein involved in the immunological response to HCV. Investigators have begun to reveal the genetic basis for not only differential responses to HCV treatment but also for discrepant rates of spontaneous viral clearance when patients are exposed to the HCV. Several studies focused in particular on two polymorphisms (rs12979860 and rs8099917) involved in the response to standard therapy.

Different researcher suggested that the two variants correlate with SVR during treatment and natural viral clearance in patients infected with HCV genotype 1. The reduction of HCV RNA after 7 days of treatment was more pronounced in patients with C / C (rs12979860) or T / T (rs8099917) than subjects with the other two variants. The two SNPs are in linkage disequilibrium (D_ = 1, r2 = 0.44), but the C/C of rs12979860 is less common than the T/T of rs8099917. Patients carrying both favourable genotypes achieved lower levels of HCV RNA at week 4 than those with CT or TT at rs12979860 and GG rs8099917 and this independently of baseline viral load, age or high or severe liver fibrosis. GG genotype frequency of rs8099917 is significantly more frequent in the group of nonresponder subjects. Viral clearance seems therefore influenced by polymorphisms independently of traditional predictors. In particular, individuals carrying the protective alleles in these loci have the double chance to eliminate the viral infection than non-carriers, and their response to the standard treatment is much more effective. A combined assessment of these SNPs in conjunction with standard response predictors may better foretell the outcome in difficult-to-treat patients, thereby sparing them the expense and side effects of a treatment that would be unlikely to be beneficial. These patients could be given the possibility of receiving alternative therapies, like the next discovered antiviral drugs.

TECHNOLOGICAL PRINCIPLE
The test is based on the Real Time PCR, a technology that allows monitoring in real time of the amplification process. Hybridization of the specific fluorescent probes to the target DNA results in fluorescence resonance energy transfer (FRET) between two fluorophores. Extracted DNA is amplified in the presence of two contiguous probes that are both complementary to the sequence which contains the mutation: probes at the 5 is marked by a donor-fluorophore, probe in 3 carries the acceptor-fluorophore. During the PCR, probes hybridizes to the target DNA and fluorophores are very closer; in presence of a source of light there is the energy transfer from the donor to the acceptor which emits a signal (with a specific wavelength) that allows monitoring of the PCR process. Emitted signals are proportional to the accumulation of PCR product. Without a target DNA no energy transfer is possible. Hybridization probes allow identification of mutations in the target sequence through the melting curve analysis; PCR product is submitted to a crescent temperature with consequent detachment of the probes from the DNA. Because the probe is an exact complement of the mutant sequence, mutant templates will form stronger hybrids with the probe and will consequently melt at a higher temperature than wild type templates. The distinctly different melting temperatures of the two possible hybrids allow identification of each unknown sample as mutant, wild type, or heterozygous (one allele of mutant and one allele of wild-type sequence).

Use only dedicated PCR tubes : no-domed tube 0.2 ml (RotorGene 3000, 6000 or Q) Set the term profile (see paragraphs below) before starting to prepare the mix. Prepare the amplification mix as follow:
Prepare the amplification mix for n+1 samples ( if n 10) or + 2 (if n >10). INTERPRETATION OF RESULTS
Click Analysis and select the Melt option: a window with the graphic analysis of the assay will be displayed . IMPORTANT: Analyze each polymorphism separately. The curves of rs8099917 could be higher than those of rs12979860; although this doesnt invalidate the final result, when analyzing the two polymorphisms separately, youll have better curves. The graph shows the melting-curve of each sample(s) that could be: rs12979860

Example:
WARNING Do not use blood samples or DNA which has been kept frozen for more than one year, or gone trough many freeze-thaw cycles in order to avoid low peaks or problem related to the amplification step. We recommend the use of fresh blood o frozen samples thawn only one time.
SAFETY
Do not eat, drink smoke or make up in working areas. Use disposable latex gloves when handling reagents and samples. Do not pipette with mouth. Consider any biological samples as potentially dangerous and infected. Clean and disinfect each surface where biological samples were handled.
