Meraux Refinery is submitting this written report as a follow-up to the verbal notifications to the NRC on December 12, 2009 and to SPOC and SERC on December 13, 2009. DESCRIPTION, CAUSE, AND WHAT OFFSITE IMPACT RESULTED: ‘On December 12, 2009 at 16:30 hours the Murphy Oil Meraux Refinery (MOUSA) discovered a sheen being discharged to the 20 Arpent Canal via Outfall 003. MOUSA was forced to open Outfall 003, in accordance with our LPDES permit on December 11, 2009 due to prolonged rains inundating the facility in excess of the capacity of our storm water management systems. The release of a sheen was an excursion from the LPDES permit conditions for this outfall, and triggered notification to the National Response Center. On December 13, 2009 at approximately 07:30 hours, MOUSA discovered a release of dieseb-like oily water on the 20 Arpent Canal, presumably from Outfall 003. ‘At 09:30 hours, MOUSA determined that the release exceeded the LDEQ Reportable Quantity of 1 barrel of oil, and promptly notified SPOC. Between the two aforementioned notifications, MOUSA received an additional 7 inches of rainfall. Fortunately, MOUSA had previously deployed sorbent booms in the canal, which helped contain the release despite the very high levels of water in the canal, MOUSA acted quickly in contacting and deploying an onsite vacuum truck contractor to start recovering the material accumulated at the booms. MOUSA quickly hired an oil spill response ‘contractor to také the lead ir the" emergency response; including the-use of pressurized water to move the material to oil skimmers, which were pumped to vacuum trucks. Recovered liquids were retumed to a refinery slop tank for reprocessing in the Crude Unit. Sorbent booms were replaced as they became saturated, and hard booms were deployed as more storms hit the area. The spill response contractor also conducted air monitoring (LEL, VOC and benzene), and detected no levels of concern. The refinery did receive several complaints for odor in the area, and some allegations of health impacts. Personnel from LDEQ, St. Bernard Fire Department and US Coast Guard responded to the incident to review our response actions. The response was concluded on December 16, 2009. It should be noted that the entire greater New Orleans area, and much of the Southeastem United States, was inundated by the effects of a massive stationary low pressure system which resulted in the most rainfall ever recorded in New Orleans in December. Numerous streets around the refinery were flooded several times during this storm. 2 EMERGENCY OCCURRENCE AND/OR AIR UPSET NOTIFICATION FORM (SERC Incident Number: 09-06735) DATE/TIME INCIDENT BEGAN AND TIME IT LASTED: ‘On December 12, 2009 at 16:30 hours the Murphy Oil Meraux Refinery (MOUSA) discovered a sheen being discharged to the 20 Arpent Canal via Outfall 003. On December 13, 2009 at approximately 07:30 hours, MOUSA discovered a release of diesel-like oily water on the 20 Arpent Canal. At 09:30 hours, MOUSA determined that the release exceeded the LDEQ. Reportable Quantity of 1 barrel of oil, and promptly notified SPOC. The release was secured at 11:00 on December 13, 2009. Clean-up of the spilled material is completed. WHICH SPECIFIC’ POLLUTANTS WERE EMITTED AND HOW-MUCH-OF EACH-COMPOUND - WAS RELEASED? The episode resulted in the release of approximately 20 barrels of diesel-ike mixture of material. LRTK Reportable Quantity: 1 bb Quantity Spilled: 20 bbl Release to Air: 0.93 bbl Quantity Recovered: 18 bbl WHAT OTHER AGENCIES WERE NOTIFIED? NRC, SERC and LEPC. IMMEDIATE CORRECTIVE ACTION TAKEN? MOUSA acted quickly in contacting and deploying an onsite vacuum truck contractor to start recovering the material accumulated at the booms. MOUSA then contacted a third-party oil spill response contractor to supplement those efforts and take the lead in the emergency response, including the use of pressurized water to move the material to oil skimmers, which were pumped to vacuum trucks. Recovered liquids were returned to a refinery slop tank for reprocessing in the Crude Unit. Additional sorbent and hard booms were deployed as more storms hit the area. SPECIFIC ACTIONS TAKEN/PLANNED TO PREVENT RECURRENCE? This incident is still under investigation. MOUSA will review storm water management procedures to determine what improvements can be made to avoid a recurrence of the problem. PREVENTABLE? No. The refinery’s storm water management systems are not designed to store and treat water ‘generated by 12 inches of rain in a 3-day period, which peaked at 1.5 inches per hour on the evening of December 12.
