Steps in the Delivery of Modules to
the Barangay Hall/Covered Court
1. Inform the Barangay Captain of the said Barangay and
provide the schedule of Module Delivery and Retrieval to
avoid miscommunication and ask the Brgy. Captain a
designated area to distribute the module – Brgy Hall or
Covered Court. The assigned teacher will make this
communication before the date of module delivery.
2. The advisers will prepare the plastic envelop with
names of their advisees. This is where the modules be
inserted -packing of modules.
3.The subject teacher must distribute the
modules to the advisers before the
delivery date.
4.The advisers will consolidate the
modules and insert them in the plastic
envelop -for delivery.
5.Each adviser will coordinate to the
assigned teacher per barangay to
distribute the modules for delivery. The
assigned teacher will receive the modules.
6. Each adviser should provide the lists of the
names of their advisory using the distribution
and retrieval form/template. Advisers are tasked
to provide list of the students per barangay. The
lists will be given to the assigned teacher who
will deliver the module.
7. During the delivery, the following documents
must be available to teacher -deliverer: the
modules, distribution retrieval form, kasunduan
ng magulang at guro, tseklist ng pagsubaybay,
tseklist ng mag aaral, ID to be given to the
parents.
8. Ensure that the distribution-retrieval form will be
signed by the parents upon receiving and during
returning of modules.
9. The distribution -retrieval form should be returned
to the advisers upon completion of the delivery-
retrieval cycle. The process will be repeated every
week or every second week depending on the
agreed time frame between teachers and students.
10. In the next module/answer sheet -subject
teachers must include the total score or feedback
from the teachers for students to be informed of
their own progress.
