Risk Description Action
20 – 25 Extremely Risk A risk that will result in fatality, serious injury or very high potential for loss of
worker life. It might be happen very frequently if no any action was take. The action
must be taken is manage risk utilizing precaution or mitigation with the highest
priority.
A risk that is being particularly subject to a high level of damage or cause a bad
effect to the worker due to the occupation that running at high rise building and
12 – 16 High Risk could possibly involve him in a risk of death, injury and failure. A detailed of
handling and mitigation action must be considered to make the risk less severe or
painful.
A risk that may result in death, damage, injury, and failure to the worker but it was
not frequently happen. The worker will cause a short-term injury such as a
5 – 10 Medium Risk backpain, strain and sprain if he falls and it require days off work for recovery. The
action are needed to ensure that the risk is reduced by practicing the risk control
measures such as administrative controls or personal protective equipment (PPE).
A risk that have little or no cause of bad effect on any aspect to the worker. No
3–4 Low Risk specific action is needed to decrease the risk but can be managed by practice the
standard procedures and should be monitor the risk frequently.
1–2 No Risk No mitigation action required and can be considered as safe level of risk.
