JOINT AFFIDAVIT OF ARREST

That , We, PO3 Pilar E. Trago and PO2 Raine P. Santos, both of legal age, both
member of Philippine National Police and presently assigned at Pasig Police Station, Pasig
after having duly sworn to in accordance with the law, do hereby deposed and state the
following, to wit;

THAT, We are presently assigned at Pasig Police Station, with 12 hour tour of du
everyday;

THAT, On about 13:00h of june 18 2015 we received a phone calling from conce
citizen reporting that there are number of people using illegal drugs every night at 755
lansones st. Along Mansanas st. Corner of Duhat st. Manggahan Pasig City between ten t
eleven in the evening.
 THAT, On the response to a certain call, we immediately conducted a positive one
day surveillance to verify the veracity of the place, after that we proven positively verified
the information provided by concerned citizen, on the day after the surveillance in a given
area, We conducted with PDEA planning on how the person arrest. Around 10;30 in June
19 the evening we immediately proceed to the said place with the using mobile car plate
no. FER567 and upon our sudden arrival we caught them doing pot session inside the
house upon the arrest 3 had fled out and 2 were left and we were recover the all evidence
such as shabu paraphernalia 3 unit Cellphone, one nokia1129, NokiaD115 and Cheery
mobileL31 and money with control no.3476555.

THAT, We arrest the two suspects Mr. Ronald M y Tayo 30 y/o and Mr. Ricky E.
Belo 36 y/o both legal age were brought to the nearest hospital for medical observation.

THAT, it was ironic that based on the result of drug test the two suspects were found
negative.
 THAT, above named never the less two arrested together the pieces of
evidence gather from the pot session were brought before the office of Pasig Police
Station for proper disposition.

THAT, arrested persons was APPRIZED OF HIS CONSTITUTIONAL RIGHT

1. Reference Police Blotter with entry no. 0001 page 0001 dtd June 18 2015 re subject
same as above.
 2. the above cited subject and reference, please be informed that on or on about 1300h of
June 18 2015 we receive a phone calling from concern citizen reported that there are those people
who was use illicit drugs every night at 755 Lansones st. Along Mansanas st. Corner of Duhat st.
Manggahan Pasig City and said they do it between ten to eleven in the evening.

3. initial investigation conducted by EPD. That the two person arrested from the buy bust
operation with three days surveillance who identify as one Mr. Ronald M y Tayo 38 y/o marriage and
Mr. Ricky E. Y Belo 36 y/o marriage both residing at Manggahan Pasig City who are the person in the
vicinity and the rest of them escaped, except for the two left which saw to them the evidence which
is money, foil,10 g of methamphetamine, lighter, candle, the two arrested person brought into
nearest hospital before the office of PNP and the evidence gather were brought to the same office
for technical examination.

003 6/21/15 2300H After the 3 days surveillance conducted and the information confirm PO3
Trago PO2 Santos cooperate the number of anti illegal drugs to conduct the path session
and immediately proceed to the said place the rest of them escape except for the two left
which saw the the paraphernalia and other instruments and apparatus using drugs. The
two person brought into the office of PNP for prepare the investigation.
004 6/21/15 2400H On this given date and time there were two person brought into this office as
one Ronald Manahan y Tayo, 26 y/o ,single , jobless, native in Camarines Sur and
presently residing at no.3 mansanas St. Manggahan Pasig City and Ricky Nievas y Belo, 27
y/o, married, jobless, native in Bicol, presently residing at no.110 Labanos St. Manggahan
Pasig City. Arrested from path session and anti illegal drugs campaign.
