Methodology

The survey was conducted over the using a form which was
communicated to all via email. User can either fill the form and
send the hard copy or scan and send the soft copy to OHS
Department. Respondents were able to complete the survey at
their own pace. Several respondents either did not have Workstation
Internet access or were not comfortable answering the survey layout
on-line. These people were sent a printed version of the survey.
In two cases, the respondents preferred to answer the survey Training and
over the phone. In these cases, the survey was carefully read to Awareness
the respondent over the phone and care was taken to read the
survey word for word without providing extra details.

Survey form consisted of 6 sections including 3 questions in

each section.
