Wound

• Any break in the external or internal surfaces of

the body involving a separation of tissue, and
caused by external injury or force
Figure 28-1 • The
inflammatory response.
The words in red are the
five classic signs and
symptoms of inflammation.
Stages of Wound Healing

• It consists of four highly integrated and

overlapping phases :
• 1- Hemostasis
• 2- Inflammation
• 3- Proliferation( Cell Differential )
• 4- Tissue Remodeling(repithialization).
The process of wound healing is retarded by the
presence of Biofilm
Components of Biofilm

• GRAM + Ve BACTERIA ( s. aureus – MRSA – MRE –streptococcus

strains ) and Etc.

• B. GRAM – Ve BACTERIA ( pseudomonas aeruginosa-acinetobacter

– enterobacter strains ) and Etc.

• C. Candida Species ( Candida Albicans) and Etc

Efficacy of various wound irrigation solutions

against biofilms. Biofilm equivalents (EU/ml)

Objective 120

The objective of this study was to test the efficacy of three

100
wound cleansing solution against biofilms.

Results 80

The results showed no decrease in the original biofilm load 60

after exposure to normal saline solution as well as Ringer’s -87%

solution, while the surfactant polyhexanide solution achieved a 40
significant reduction (p<0.001) of the biofilm by 87%.

Conclusion
0

Using the Biofilmyl® method, polyhexanide Solution shows a Baseline load 0.9% Saline Ringer’s Polyhexanide
solution solution solution
better reduction of biofilm when compared to normal saline
and Ringer’s solution.

Actolind ®
Actolind ®

1- physical Cleaning solution / irrigation of wounds

2- chemical removing of Biofilm

Content :
Two effective ingredients combined to maximize cleansing
and Antimicrobial powers :
- Polyhexanide/polyhexamethylene biguanide (PHMB)
-Poloxamer
Actolind ®
1- Polyhexanide ( polyhexamethylene bigunide )(PHMB):
( Promotes Healing Minimises Bioburden )

 Polyhexanide has a broad spectrum antimicrobial activity against Gram +ve,

Gram –ve and Fungi (components of biofilm), which achieves immediate and
effective bactericidal and fungicidal results.

 Polyhexanide is a positive polymer (cationic) interact with the negative

charges of the microbial membrane Acidic phospholipids, altering their
organizational, structural and load distribution. Thus the micro-oraganism
become unable to maintain there normal functions, then occurring their lysis.
Actolind ®
1- Polyhexanide (PHMB): ( SPECIFIC MODE OF ACTION )

 The efficacy spectrum of Polyhexanide covers a wide range of microorganisms,

including Methicillin Resistant Staphylococcus Aureus (MRSA), Methicillin Resistant
Enterococcus (MRE),Vancomycin Resistant Enterococcus (VRE), viruses (Corona virus
and influenza virus) and fungi (Candida species)

 Polyhexanide (PHMB) has good tissue compatibility based on its activity against the
acid lipids contained within the micro-organisms cell membranes and no effect on the
neutral lipids of human cell membranes. This helps to prevent damage to the
surrounding healthy tissue.
• 2- Poloxamer
( gentle effective surfactant )

.
Poloxamer

has moisturizing, regulating and anti-inflammatory

characteristics it improves the cell remodeling and decreases
irritation.
Clinical trials
Polyhexanide and betaine solution and gel reduce the growth of
microorganisms by more than LOG 5 in-vitro

 Objective
• To investigate the antimicrobial effects as a possible
supportive mechanism of action of Actolind®
Wound Irrigation Solution and Actolind® Gel.
 Methods
• In-vitro testing was performed according to USP 32-
NF 27 2009, method 51 evaluating 13
microorganisms at 7, 14, and 28 days following
exposure to 3 lots of the compounds/products.
 Conclusion
• The log 5 reductions in antimicrobial activity in
12/13 microorganisms tested is suggested as a
possible supportive mechanism of action of
enhanced wound healing when using a
combination of 0.1% polyhexanide and 0.1% of
betaine either as a gel or an irrigation solution.
Experiences in using Polyhexanide in the management of chronic wounds results of a
methodical and retrospective analysis of 953 cases
Standardized comparison of antiseptic efficacy of triclosan, PVP–iodine,
Octenidine dihydrochloride,
Polyhexanide and chlorhexidine digluconate

.
Actolind® Safety and
Tolerability
Evaluation of the efficacy and tolerability of Actolind® solution

 Conclusion
• Treatment with Actolind®
Wound Irrigation Solution
can lead to
 A decrease in pH, which is a
surrogate marker for
bacterial burden
 And is well tolerated for the
treatment of chronic ulcers.
Exploratory toxicological data of common antiseptics compared to Polyhexanide
Clinical cases
Clinical case demonstrate the use of Actolind® indifferent type of burns

• 29-year-old female  Surgical debridement of the 2 weeks (post-burn) 6 weeks (post-burn):

patient sustained a burn wound.  No signs of infection  Complete wound
superficial to deep  Regular wound cleansing  Successful epithelialization closure with little
partial-thickness
burn to face and with Actolind® Wound  No need for grafting scarring
neck. Solution and Wound
coverage with Actolind®
