A 68 years old patient was admitted from

Urogynecology Polyclinic of Dr. M. Djamil Central
General Hospital on July, 17th 2019 with diagnose
grade IV uterine prolapse
 Present Illness History
 The patient was admitted from Urogynecology Polyclinic at Dr. M. Djamil
General Hospital at July, 17th 2019 with chief complain a mass came out
from vaginal as big as adult fist since 3 years ago
 Abdominal pain (-)
 Bleeding came out from vagina was (-)
 Dyspareunia (-) Post Coital Bleeding (-)
 No complain of urinary
 History of fever (-), trauma (-), leucorrhea (-)
 Have no menstruation since 16 years ago
 No complain of bowel tract disorder.
 Menstrual History : patient already menopause since 16 years ago
Previous Illness History:
• There was no previous history of heart, lung, liver, kidney
disease, DM , hypertension or allergy.
Family Illness History:
• There was no history of any hereditary or contagious disease,
or psychological illness run in the family.

 Transvaginal Hysterectomy

Transvaginal Hysterectomy and colpocleisis were performed
Intraoperative bleeding 300 cc
Uterine mass was checked to PA Lab

Diagnosis:
Post Transvaginal Hysterectomy + colpocleisis on indication of grade IV
uterine Prolapse

Management:
Post operative monitoring
Cross match PRC
Transfusion 1 Pack of PRC post operatif
 Post Operative Treatment

• Control GA, VS, Vaginal Bleeding,

Intraabdominal Bleeding
• IVFD RL 20 gtt
• Inj. Ceftriaxone 2x1 gr (IV)
• Urine catheter 24 hours
• Laboratorium post operative
