ANATOMI MAXILLOFACIAL
Facial danger zone
Facial danger zone 1
• Facial danger zone 1
is centered around a
point in the middle of
the
sternocleidomastoid
muscle belly 6.5 cm
below the caudal
edge of the external
auditory canal .
• It contains the
emergance of THE
GREAT AURICULAR
NERVE from beneath
the
sternocleidomastoid
muscle , becomes
more superficial , and
is thus more
susceptible to injury .
After the postauricular
incision is made ,it is
helpful to begin the
dissection superficially ,
just deep to the
subcutaneous fat ,
which is thin and
superficial to the deep
cervical fascia and the
sternocleidomastoid
muscle
Consequence of injury
Permanent injury to the
nerve results in
numbness of or, painful
dysesthesia of the lower
two thirds of the ear &
adjacent neck and
cheek skin .
• The nerve is posterior
to and not protected
by the platysma
through most of its
course .
• Note :

• It is outlined by
drawing a line 0.5 cm
below the tragus to a
piont 2 cn above the
lateral eyebrow ,
drawing a second line
on the zygoma to the
lateral orbital rim ,and
connecting these two
lines with athird line .
• In this zone the
TEMPORAL
BRANCH OF THE
FACIAL NERVE lies
on the undersurface
of the temproparital
facia
Consequence of injury
• Injury to the temporal
branch results in
paralysis of the
frontalis muscle . •
Clinically ,the involved
side of the forehead
becomes parylized ,
with resultant ptosis
of the brow ,
asymmetry of the
eyebrows .

• It is defined by a piont
drawn on the
midmandible at a
level 2 cm posterior to
the oral commissure
and a circle drawn
with a radius of 2 cm
around this point

Consequence of injury
• This nerve innervate
the depressor anguli
oris muscle .injury
creates a noticeable
and extremely
distressing deformity .
• Be careful during
subcutaneous
dissection and using
electrocautary

• It is deep to the
parotid fascia . It is
outlined by placing a
mark on the highest
point of malar
eminence , another
one on the
mandibular angle ,
and a third mark at
the oral commissure .
• This triangle contains
thr ZYGOMATIC and
BUCCAL branches of
facial n
Consequence of injury
• Injury to these nerves
can result in paralysis
of the zygomaticus
major & minor m. and
levator labii superioris
alaeque nasi m.
,causing the upper lip
and oral commissure
on the affected side to
sag .

• It is defined by a
circle with a RADIUS
OF 1.5 CM DRAWN
AROUND THE
SUPRAORBITAL
FORAMEN.

• Injury to
SUPRAORBITAL &
SUPRATROCHLEAR
branches results in
numbness or painful
dysesthesia of the
medial forehead
,scalp , upper eyelid ,
and nasal dorsum

• It is described by a
circle with a 1.5 cm
radius around the
infraorbital rim , along
the line down through
the midpupil and
second mandibular
premolar.
Consequence of injury •
• Injury to
INFRAORBITAL N.
lead to numbness of
the lateral nose
,cheek, upper lip , and
inferior eyelid

TRAUMA MAXILLOFACIAL
Definisi
Trauma maksilofasial adalah suatu ruda paksa/trauma fisik
yang dapat mengenai jaringan keras dan lunak pada wajah.
KLASIFIKASI
Area maksilofasial dibagi menjadi 3 bagian:
• fraktur yang melibatkan os frontalis dan sinus
Upper face
frontalis
• Atas:
• Os zigomatic
• Os nasal
• Os ethmoid
• Os maksila (le fort II dan III)
• Nasoethmoidal kompleks
Mid face • Zigomaticomakxilla complex
• Dasar orbita
• Bawah:
• Os maksila (le fort I)
• Alveolar maxilla
• Gigi
• Palatum.
Lower face • Os mandibula
TRAUMA MAXILLOFACIAL
EPIDEMIOLOGI ETIOLOGI
• Puncak insidensi trauma
maxillofacial adalah rata-rata
pada usia 20-30 tahun.
• Insidens lebih tinggi pada laki-
laki.
• Di negara berkembang,
kecelakaan lalu lintas sebagai
penyebab utama trauma
maxillofacial. Insidensi nya
menurun seiring dengan
peningkatan keamanan dalam
berkendara.

INITIAL MANAGEMENT
• Cedera wajah jarang mengancam nyawa
indikator energy injury
• Penanganan inisial fokus pada ATLS
• Cedera fasial tanda kemungkinan
permasalahan airway, cedera cervical,
dan cedera CNS
• AIRWAY:
– Permasalahan biasanya akibat perdarahan
dari upper airway, benda asing (aspirasi
gigi/fragmen os), cedera langsung laring,
dasar mulut dan lidah kehilangan sokongan
akibat # mandibula
– Upring positiong+cervical spine improve
airway
– Benda asing finger sweep technique
– Endoskopik nasal/oral intubasi lebih aman
krn menghindari manipulasi servikal
– Trakeostomi biasanya dilakukan di kamar
operasi akan tetapi pada situasi tertentu akan
dibutuhkan emergensi
trakeostomi/krikotiroidotomi
• HEMORRHAGE:
– Kepala dan leher kaya akan BV resiko blood loss
besar pada cendera soft tissue
– Kebanyakan cedera memiliki akses untuk
penekanan langsung dan kontrol perdarahan
– Perdarahan yg tdk dpt di kontrol dg tekanan
langsung packing
– Packing pd kavum nasi biasanya efektif
– Perdarahan hebat management airway dahulu
(intubasi emergensi) packing dan direct pressure
• Central Nervous System:
– Cedera neuro berhubungan dg trauma fasial
parah
– Px dg trauma fasial biasanya + karena cedera

– Lakukan CT scan
– Neuro injury GCS

Anamnesis (Auto/alloanamnesis):
AMPLE (Allergies, Medications, Past history, Last meal, Event
surrounding accident)

menjadi 3 bagian yaitu:
1.Fraktur Sepertiga Atas
Wajah (os frontalis dan sinus
frontalis)
2. Fraktur Sepertiga Tengah
Wajah

– Os zigomatic, Os nasal, Os ethmoid,
Os maksila (le fort II dan III),
Nasoethmoidal kompleks,
Zigomaticomakxilla complex, Dasar

• Bawah:
– Os maksila (le fort I), palatum,
alveolar dan gigi di bagian maksila
3. Fraktur Sepertiga Bawah
Wajah
Pemeriksaan Secara Umum
• Inspeksi : deformitas/asimetri
• Palpasi : krepitasi dan iregularitas
• Pemeriksaan CN V dan VII
• Inspeksi : intranasal hematom septum
• Oftalmologi exam : defisit nervus optikus
• Intraoral : maloklusi, #, gigi avulsi
Penanganan Umum
(Prehospital)
Jangan lakukan manipulasi pada tulang wajah terlalu banyak kecuali sudah di pastikan
tidak ada cedera cervical.
Survei primer:
1. Airway & c-spine support
– Beri O2
– Jaga jalan napas paten
– Imobilisasi cervical
– Bersihkan airway dari hambatan Suction bila ada darah
2. Breathing
– Nilai frekuensi napas
– Nilai suara nafas tambahan
– Atasi kesulitan bernapas
3. Circulation
– Nadi, suhu akral syok curiga perdarahan intra abd/thorax/pelvis
4. Disability
– GCS
– Status mental dan catat setiap perubahannya
5. Exposure
– Expos pasien lepaskan baju bila perlunamun jaga tetap hangat
6. Rujuk kpd Sp.BP
1/3 atas wajah
UPPER FACE
Upper face
• Fraktur sepertiga atas wajah mengenai
tulang frontalis, regio supra orbita, rima
orbita dan sinus frontalis.
• Fraktur tulang frontalis umumnya bersifat
depressed ke dalam atau hanya
mempunyai garis fraktur linier yang dapat
meluas ke daerah wajah yang lain.
• Ditandai dengan destruksi atau krepitasi
pada supraorbital rims, emfisema subkutan,
dan parestesi pada supraorbital nerve.

FRAKTUR NASAL
• Fraktur pada hidung didiagnosis berdasarkan:
• Riwayat trauma
• Epistaksis
• Nasal obstruction
• Deformitas external: edema, deviasi septum
• Inspeksi intranasal: septal hematoma
• Palpasi: Adanya krepitasi atau mobilitas pada segmen
yang patah.
• Fraktur yang lebih parah dapat disertai periorbital
ekimosis, perdarahan subkonjungtiva, dll
PEMERIKSAAN FISIK
• Palpasi dan lakukan gerakan atau goyangkan akar hidung
• Jika terdapat krepitasi atau imobile kemungkinan ada
frakture
• Evaluasi yang paling penting adalah inspeksi intranasal.
Dengan penambahan topical decongestion pada mucosa
hidung khususnya untuk pemeriksaan adanya dislokasi
septum atau hematoma submukosa.
• Septal hematoma yang tidak ditangani dapat menyebabkan
resorpsi septum kartilago sehingga terjadi saddle nasal
deformity.
TATALAKSANA
• Pasien dengan epistaksis dilakukan pemasangan tampon.
• Early: Closed reduction, dilakukan pada 2-3 minggu
pertama setelah terjadi trauma.
• Fiksasi dengan external/nasal splinting
• Namun biasanya menghasilkan residual deformity
sehingga perlu dilakukan rhinoplasty sebagai pilihan,
setelah bengkak dan memar membaik.
• Jika terdapat septal hematoma lakukan drainase secara
perkutan.
• Late: Septoplasty atau Rhinoplasty
• Fraktur kominutif nasal berat: trans-nasal wire serta
koreksi dan elevasi fragmen tulang.

FRAKTUR LE FORT
Definisi
Merupakan trauma yang melewati
maksila secara transversal, diantara akar
gigi dan infraorbital rims, dengan
infraorbital rims yang utuh.
Fraktur maxilla horizontal yang
menyilangi aspek inferior maxilla dan
memisahkan procesus alveolar yang
mengandung gigi maxilla dan palatum
durum dari bagian lain maxilla
Garis fraktur berjalan ke belakang melalui
lamina pterigoid.
Fraktur ini memungkinkan maksila dan
palatum durum bergerak secara terpisah.
Manifestasi klinis
Edema facial
Pergerakan hard palate, alveolus maksila dan gigi

Pemeriksaan Umum Pada Fraktur
Maxilla
• Inspeksi untuk tiap deformitas dan asimetri.
• Palpasi seluruh tulang kraniofasial untuk melihat ada tidaknya
iregularitas atau krepitasi.
• Pemeriksaan nervus trigeminal dan fasial.
• Inspeksi intranasal untuk melihat hematomaseptum.
• Pemeriksaan oftalmologi untuk menilai adanya jebakan ekstraokular
atau defisit nervusoptikus.
• Pemeriksaan intraoral untuk menilai adanya maloklusi dan fraktur
atau gigi yang hilang.

Tatalaksana
Reduksi dengan pendekatan
sulcus ginggivobucalis dan infracilliar palpebra
inferior.
Fiksasi dengan wire atau mini plate screw
Nasoorbitoethmoid Fractures
• Fraktur yang melibatkan medial orbit,
nasal bones, septum, nasofrontal junction
dan etmoid bone.
Markhowitz classification:
• TYPE I: Involves single segment
central fragment fractures.
• TYPE II: Comminuted central fragment
with fracture lines remaining peripheral
to the MCT insertion.
• TYPE III: Comminuted central fragment
with fracture lines extending beneath
the MCT insertion.
In unilateral Markowitz type I fractures, there is a single large NOE
fragment bearing the medial canthal tendon.
In unilateral type II fractures, there is often comminution of the NOE area, but the
canthal tendon remains attached to a fragment of bone, allowing the canthus to
be stabilized with wires or a small plate on the fractured segment.
In type III fractures, there is often comminution of the NOE area (as in type II
fractures) and a detachment of the medial canthal tendon from the bone.

keparahan fraktur
Tatalaksana
• open reduction and a combination of
interfragmentary fixation and plate/screw fixation.
• Type 1 -> fixation of the bone fragment to adjacent
surrounding bone
• Type 2-> When the fragment is too small for fixation,
transnasal wiring can be used to position the
fragmentand medial canthal tendon appropriately.
Additional bone grafting may be required.
• Type 3 ->transnasal fixation of the medial canthal

Manifestasi klinis
• Nyeri saat menggerakan rahang bawah
• Maloklusi gigi
• Kesulitan membuka mulut atau menggigit ke
arah bawah
• Palpasi-> mobilitas dan krepitasi sepanjang
simfisis, angle, body.
• Edema intraoral, ekimosis, perdarahan gusi
• Parestesia, anesthesia setengah bibir bawah,
dagu,gigi, gusi.
Pemeriksaan fisik
• Pemeriksaan lokal fraktur mandibula
• Inspeksi : bengkak, eekimosis,lacerasi,
astmetris, intra oral dlihat gigi patah atau
hilang, deviasi
• Palpasi border madibula dan
preaulikular area, tendernes atau
krepitasi
Pemeriksaan penunjang
• X-ray
• Panoramic
• Caldwell
• Waters
• CT SCAN Kepala
Tatalaksana
• objectives in treating mandible fractures
include achieving reduction and
stabilization, pretraumatic occlusion, facial
contour and symmetry.
• Subcondylar fractur -> intermaxillary
fixation (arch bar/screw)
• Open reduction subcondylar fractur ->
condylar dislocation, severe malalignment,
or loss of mandible height.
• Displaced fractures of the angle, body, and
symphysis are treated via open reduction.
• Coronoid fractures often require no
intervention.
• Postoperatively, antibiotics and aggressive
oral hygiene care.
• Patients are routinely examined in clinic with
a follow-up Panorex to assess occlusion and
bony union
