CLASSROOM ASSESSMENT

for the K to 12 Basic


Education Program
(DepEd Order No. 8 s. 2015)
MAIN TYPES OF ASSESSMENT
Two Types of Classroom Assessments:
1. Formative – identify the progress, strengths and
weaknesses of students.
2. Summative - measures whether the students have
met the content and performance standards.

The numerical system of grading is
used and grades are expressed in 2
decimal place for Non-honor and 3
decimal points for honor pupils.

Highest possible grade equivalent is 100.

Lowest possible grade equivalent is 68.

Passing grade equivalent is 75 which is


equivalent to 50% correct of the test items
given.
The Personal, Social, and Emotional
growth of the pupils are also rated by
the teachers based on the CORE
VALUES OF THE SCHOOL.

*** Rating is based on DEpEd Equivalent


CLFP (Catholic Life Formation PROGRAM)
Catholic Life Formation Program is graded
based on the components given by the
Department of Education however, the CLFP
teachers will also include the consolidated
behavior grade of each student (graded by all
teachers) and the grade is incorporated under
PT (Performance Task).
The behavior grade is based on the CORE
VALUES OF THE SCHOOL
Absence During Exam with Valid Reason
 The periodical test grade equivalent is
85% of the average of the equivalent of WW
and PT that is:

No special test is given.

PT equivalent grade = 0.85 x WW + PT

Absence During Mastery Test
 The equivalent grade of the Mastery Test
is equal to 85% of the average of the QA
from 1st quarter to 3rd quarter that is:

NO special test is given

MT = QA1 +QA2 +Qa3 x 0.85

Policy on the Determination of Honor:
1.Candidates for honors at any grade level shall be drawn from the
Top 5 students with no grade below 85% in all grading periods.
2. The following scholastic distinctions are warded:
•First Honors – 94% and up
•Second Honors – 92-93%
•Third Honors - 90-91%
Excellence in Scholastic Performance -----Students who do not
make it to the top 5 with general average of 90% with no grade
below 85% in all grading periods
