A hazard is any existing or potential condition that can lead to injury,


illness, or death to people; damage to or loss of a system, equipment,
or property; or damage to the environment.

To identify and assess hazards, employers and workers: Collect and


review information about the hazards present or likely to be present in
the workplace. Conduct initial and periodic workplace inspections of
the workplace to identify new or recurring hazards.

Real Life Incident: Crew Member Falls From 14 Metre
Inside Tank, Loses Life

A tanker was in port for repairs and dry-docking. In


preparation for extensive hot work during the repairs,
some deck crew were assigned to clean cargo tank
number two port. In the morning, a toolbox meeting
was held for the ship’s senior management – but not
the men intended to do the actual cleaning work.
The crewmember assigned the cleaning duty
entered the tank in the afternoon. After a few hours,
he came out for a break. Before re-entering the tank,
he asked another crewmember to give him a hand
in mopping out the tank. The second agreed and, as
it was now raining, the two crew hastened their pace
to get to the entrance to the tank and inside. The
two entered the tank, but as the second descended,
he saw the first fall over the guardrail of the tank
access ladder. The victim had fallen about 14 m. The
alarm was raised, a rescue team mustered and shore
side medical help called. Unfortunately, a little while
later the victim was pronounced dead.

What went wrong in the Given Scenario?

1. Toolbox meeting for senior management only


2. Crewmember calling upon another crewmember for help shows
that there was a lack of control for the job. Bad planning, lack of
safety measures in place
3. Rushing the job that caused the fall
4. Consideration of when the work involves work at height Aloft
procedures should be considered.

Conclusion
What is the Aim of Conducting a Risk Assessment?

The aim of the risk assessment process is to remove a hazard or reduce


the level of its risk by adding precautions or control measures, as
necessary. By doing so, we can create a safer and healthier
workplace.
