VARIANCE REPORTING
• Variance report (also called an event report or occurrence report)
is a formal report written by practitioners, nurses, or other staff
members.
• It is not intended to point blame, just document the facts. There
purpose is to identify situations or system issues that contributed
to the occurrence and to engage strategies to prevent
reoccurrence or to correct the situation.
• Generally, the report is confidential communication. The report
should not be placed in the chart.
VARIANCE REPORTING
Variance may be related to an action or outcome. A variance in an action can
include:
Something performed that was not intended to be performed (e.g., patient
was required to have a repeat diagnostic test because the results of the first
were not provided to the intended recipient)
Something performed at the wrong time (e.g., a patient transportation
company came a day earlier than requested)
Something that was not performed that should have been performed (e.g.,
Meals-on-Wheels did not supply food for an entire day)

2. Unanticipated client death
3. Malfunction of equipment
4. Unanticipated adverse reactions
5. Inability to meet client needs--system
problem, order problem, lack of qualified staff,
or client/family refusal of care.
6. Unethical, illegal, or incompetent practice.
7. Client/ family complaint about care.
8. Toxic spills, fires, and/or other enviromental
emergencies.
9.Violent behavior by the client/ family.
10. Loss of property.

VARIANCE ANALYSIS
• Used by healthcare organizations to explain variation
between planned and actual costs and charges. This type of
analysis is becoming even more common as healthcare
executives work to improve efficiency, to set priorities for
organizational improvement as part of strategic planning, and
to explain costs and charges to interested groups such as
purchasers and payers.
