evaluate the effective use of the
documentation form, Accommodation
Effectiveness Form, for the special
education students in inclusion
classrooms.
Description of Participants and
Setting:
The participants in the study are special education teachers.
Demographics are as follows:

● 3 females and 4 males

● Age Ranges: 20-29 (28.56%), 30-39 (28.56%), 40-49 (14.28%),
50-59 (28.56%)
● Job Title: 100% special education teachers; and one teacher is both
a special education teacher and a general education teacher.
● Years of Experience: 0-5 (28.56%), 6-10 (14.28%), 11-15
(14.28%), 16-20 (14.28%), 21-25 (14.28%), 26-30 (14.28%)
● Campus Level: 6 participants selected High School Level, and 1
participant seleceted all (pk-12).

Setting: The research took place during the special education

department meeting; participants took the survey on-site.
Ethical Considerations:
● All participants were provided this general background information prior to
taking the survey:

You are being asked to participate in a short survey about the effective use of documentation for special
education students in inclusion classrooms. I am asking that you take part and support me in the fulfillment
of my Action Research Seminar II (Educational Research Design and Data Analysis) class.

The survey will include, but is not limited to, questions about your job, class size, students, and challenges
faced within the school setting.

There are no anticipated risks to you for providing answers to the questions regarding children who participate
in the special education program. Your specific answers for this survey will be kept confidential and no names
will be used in my research. I will be the only one with access to the survey responses.

● Upon reading the background, survey participants were asked to agree to

participate by answering the following yes or no question:

I have read the above information and voluntarily give my consent to provide answers to the survey questions.

● If the participant selected yes, we used his or her responses and if he or she
selected no, we did not use the responses.

Qualitative Data Coding
Procedures:

• Survey responses were gathered and input

into Data Sheets for each category
question.
• Each question responses was coded
according to the categories.
• The informaiton was then finalized and
entered into the data tables.

Conclusion:

 The survey findings displayed that the new

Accommodation Effectiveness Form used for record
keeping of student’s accommodations in the general
education setting is a practical and effective tool to use
for uniform documentation, daily documentation, and
for ARD meetings.
 The Accommodation Effectiveness Form is easy for
teachers to utilized and can be used in other capacities
such as lesson planning, etc.
 The Accommodation Effectiveness Form has been made
mandatory for all special education in-class support
teachers to use along with general education teacher to
help provide proper documentation.
