(TSSD)
 All incoming and rejoined members shall contribute to the
Tinabangay Special Savings Deposit (TASSD). The maximum
age is 70 years old (at the time of his/her 70th birthday).
 Members shall be required to open a non-transferrable and
non refundable amount of P1,000, with an interest of 0.05%
per annum.
 To keep the membership in active status, he or she have
atleast a remaining balance of 200 pesos.

 in case of membership withdrawal, a beneficiary can avail the benefits with in
sixty (60) days from the date of withdrawal. The remaining fund of the
withdrawn member shall be added to the Death Assistance Fund of the
Cooperative.
 Re-enrolled Members. A re-enrolled member shall pay a minimum of P500.
Provided, a promissory note to complete the total required amount within the
contestability period, is executed. In case a member fails to comply.
 No fund, No claim Policy.
 A remaining balance of 9.99 below is considered as Zero Fund. A member
with Zero Fund shall be given 30 days grace period to complete the said
fund.
 Membership Documentation Requirements
 Regular members are required to appear in
person upon application
 Photo copy of any valid Identification Card (I.D.)
 Submit 1x1 colored picture
 Benefits
 Beneficiary of deceased member can benefit a
maximum of Ten (P10.00) pesos from every
member as financial assistance with the following
percentage of claim based on age bracket at the
time of membership:

 All incoming an rejoining members shall contribute to the Kalusugan
Special Savings Deposit (KASSD). The maximum age is 70 years old
(at the time of his/her 70th birthday).
 Members shall be required to open a non-transferrable and non
refundable amount of P500, with an interest of 0.05% per annum.
 Contestability period of six(6) months is required for a new member
to avail of the hospitalization program except in case of accident.
 To keep the membership in active status, he/she have at least a
remaining balance of Php 200.

centavos from every member per day of
confinement as financial assistance. A member can
avail of a hospitalization benefit for a maximum of
15 aggregate days per year with the following
percentage of claim based on age bracket at the
time of membership:

 Must be confined for at least 24 hours
 Must submit Hospital Bill indicating the time of
Admission and Time of Discharge. If no time is
indicated in the hospital Bill, additional documents
such as Medical Certificate and Discharge Summary
shall be required.
 Authorized representative to claim is the nearest kin of
the member. In the absence of the assignee, the
member shall provide an authorization letter on his/her
authorized representative. The authorized
representative may be the recipient of the check that is
named after him/her.

LOAN DEDUCTION
 SERVICE FEE – 2% of approved loan

 NOTARIAL FEE – P 50.00 for loan in excess of P 5,000.00 except for


loans equivalent to share capital

CAPITAL BUILD UP (CBU)

5% capital retention shall be deducted from the loan


granted to member whose share capital is P 10,000.00 below
in accordance to Article 4 Section 2 letter C of the MMPC By
laws.

2% capital retention shall be deducted from the loan


granted to member whose share capital is P 10,000.00 up in
accordance to Article 4 Section 2 letter C of the MMPC By
laws.

Loan requirements
A. COMMON REQUIREMENTS
1. Duly accomplished application form

3. Marriage contract (if applicable)
4. 2 x 2 ID picture with name tag both applicant and spouse
5. Barangay Clearance for first loan
6. At least of any of the following:
a. Police clearance
b. Voters ID / Certificate or Postal ID
c. Driver’s license
d. PRC ID / SSS ID / UMID
e. Proof of Billing (water or Electric Bill)
7. Certification from the barangay if spouse’s working abroad on
vacation or separated with a minimum of two years . For
separated spouses affidavit of martial dispensation shall be
attached.
8. Authenticated Death of spouse if the applicant is Widow.

Re-loan
A member is allowed to avail re-loan under the following
conditions
A. 100% of the original loan after paying atleast 25% of
the existing loan
B. 200% of the highest availed loan after paying atleast
50% of the existing loan provided it is fully secured by
a collateral loan guarantee
Criteria to reject a credit
application
1. Bad credit history
2. Purged from the cooperative or other credit unions
3. Questionable reputation
4. With personal and or family problems
5. Unprofitable business or occupation
6. Distribution of products which is harmful to the health
7. Offered collaterals and guarantees having difficulty to
control
8. Refuse to disclose information about their financial
status or adequate references
