Exfoliation of Deciduous Teeth

Tooth germ of succedaneous permanent teeth develop lingual to their


predecessors.
Predecessors hold space for the
permanent teeth.
SHEDDING OF TEETH
1. Resorption of deciduous teeth occurs on the lingual surface.
2. Odontoclast migrate from blood vessels
to the resorption side fuse form
the characteristics multinucleated
odontoclastruffled border

3. Root resorptionCoronol pulp


appears normal and odontoblast still
lines the surface of predentin.

4. Root resorption is almost complete


odontoblast degenerate and
mononuclear cells emerge from the
pulpal vessel.
5. The mononuclear cells migrate to predentin surface where they
fuse with other mononuclear cells.
6. Just before exfoliation, resorption ceases as the odontoclast migrate
away from dentin.
7. The remaining pulp cells deposit cementlike tissue.
8. Tooth shed with some pulpal tissue intact.

Premature exfoliation.
What happens? Why it happens?

Deciduous teeth can be lost early because of premature resorption of their


roots, trauma, or decay (caries) followed by extraction. Loss by premature
resorption and trauma happens only in the anterior region. Untimely loss due
to caries and extraction mainly affects the deciduous molars.
What happens when it happens

Spatial conditions in the dental arches are important :- In the absence of


excess space, complications might appear; in cases of crowding, complications
are a certainty.
Tongue and buccal musculature, the timing of the loss :- Loss long before the
time of emergence of the successor leads to its delayed emergence; loss
shortly before emergence has an accelerating effect.
Delayed Exfoliation

Delayed exfoliation is when the deciduous teeth doesnt shed in a normal time
span.
Causes:
Pathology (ie: odontoma, metabolic disorders)
Obstructions
Trauma
Malnutrition (EC-PEM)
Absence of permanent tooth
Hereditary
Endocrine disorders (ie: hypothyroidism, hypopituitarism)
