Before hospitalization, the patientwas very busywith his work as a tricycle driverand according tohimhe dont eventhink
toget some rest because of his busyschedule. He is fun of drinking alcohol with his friends but hestop smoking26 years
ago.

Whenhe had CVD attack last 2009, he was run to the nearest health care facility and from then on he was diagnosed and
hospitalized with hypertension.He was given medicationamlodipine. After 3 yearshe was advised to have hemodialysis
because her renal condition gets worse. Despite of all the patient still continue his activities of daily living, he dont even
think heis not well.

Before admission,the patient is living with his wife andsix children.But some changesfrom the patient were noted.
According to his wife, the patient becomes irritableand sometimes his angry that he needs to take so many medicationsfor
his illness. Above all the family give their deeper understandingfor the patient. Thewife and the older son strived hardjust
toprovidemedications for him.

Duringhospitalization, He undergone laboratory and diagnosticprocedures and was initially diagnosed with Uremia Problem
secondary to CKD stage five. Buthis final diagnosis wasESRD secondary toHypertensiveNephrosclerosis. Whileadmitted to
MCPICU his wife and children is always there for himto give theircomfort.The patientiscoherent, he canmanaged to talk
in fact he is very open that hetold us abouthis journey before and after hospitalization. His only complaintwhen we first
handled him he feels nauseatedevery time after he ate food or even after taking his medications.He was discharged last
July 7, 2017.
 Nutritionaland Metabolic pattern

Before hospitalization, the patientusualdietare vegetables, meat andchicken, he eats without restrictions. According to
his wifehe doesnthave problem with the foodthat is served to him. Before he can eat 3 fullmeals per day and have
snacks in the afternoon, but sometimes he forget to eat during lunch break because his very busy with his work. Mostly,
he eats fatty foods, he is also fun of eating fast foods; at work.Hedrinks water buthe dont have ideahow much he take
everyday.

During hospitalization he loses his appetite.He can only eat5-6 tablespoonbecause after eating he experienced being
nauseated. He only drinks toglass of waterevery meal. Helooks thinand have low hemoglobin as evidenced by
laboratory result of 4 gm/dland he needs to havehemodialysis as forinterventionthe physician prescribed him
supplement like ferrous sulfate +folicacid. He was also givena hypoallergenicdiet.

EliminationPattern

Before hospitalization, the patient usually voids 2-3 times a dayand since he has kidneyproblemhe sometimes
heexperiencedpain when urinating while his usualbowel pattern is once a day.

During hospitalization,the patient only defecates 1-2 times a week usually early in the morning. Patient is with indwelling
catheter anda yellow colored urine was observed.
 Activity and Exercise Pattern

PTA, the patientis focused on his work, driving his tricycle which he considered as a form of his exercise.According to his
wifehis husband is veryenergetic beforeand hedidnt even think to take some rest.

Duringhospitalization,as what observed the patient seemsfatigue but still can manage to cooperatewith hisattending
nurses, since the patient was in the hospitaland stays on bed every timeas what thephysician advised and turn every 2
hours to prevent stasis.

Cognitive PerceptualPattern

As verbalized by his wifebecause of patient condition there are some changes on hisbehaviorhe becomes irritable and the
children also added their father become less patience.

During hospitalization, as observed there are times the patient shows irritable. Despite his condition he can still manage to
smile and entertain visitors and attending nurses.

Sleep Rest Pattern

Prior toadmission the patient sleeps around9:00 10:00 pm and woke up4:30 5:30 amdue to his schedule in hiswork. He
had enough sleepand rest.Sometimes if he is not busy he takes a napin the afternoon from 2:00 pm until 4:00 pm.

During hospitalization, the patient verbalizedhe is adjustingto his environment in the hospital, the reason why his sleeping
pattern was altered. He added whenever the nurses rounds heimmediately woke upandit is difficult for him to go to

Beforehospitalization,according to his wife the patient is always conscious with his appearance.The patientgives us his
joke saying " basta driver sweet lover" and he laughs. He is a positive personhis wife added.

Duringhospitalization,the patientis very cooperative with his nurses
