La fiebre reumtica aguda (FRA) es una enfermedad caracterizada
por un proceso inflamatorio que afecta a diferentes rganos del
cuerpo.
Es una de las pocas enfermedades reumticas cuyo agente causal ha
sido identificado, el Streptoccocus pyogenes -hemoltico del grupo A.
Los individuos susceptibles presentan una respuesta autoinmune
exagerada frente a este agente infeccioso y desarrollan una
enfermedad inflamatoria que puede afectar al corazn, articulaciones,
cerebro, vasos sanguneos y tejido subcutneo
La mayor implicacin clnica deriva de la lesin a largo plazo por la
enfermedad cardiaca reumtica (ECR) y, en concreto, por la
afectacin de las vlvulas cardiacas.
Con la extensin del acceso a los tratamientos antibiticos en los
pases desarrollados, la incidencia de esta enfermedad descendi
significativamente; no obstante, hay que tener en cuenta que la fiebre
reumtica contina siendo un problema de salud pblica de primer
orden en pases pobres y en vas de desarrollo.

La fiebre reumtica (FR) es una
enfermedad inflamatoria, sistmica
caracterizada por la existencia de
lesiones que afectan al corazn,
articulaciones, sistema nervioso central,
piel y tejido celular subcutneo, como
secuela de una infeccin farngea por
esteptococo betahemoltico del grupo A.
Edad de mxima No existe
incidencia: predileccin

Organismo especfico: estreptococo hemoltico del grupo

Localizacin corporal especfica: el tracto respiratorio

superior.
Nunca se ha demostrado que la infeccin estreptoccica
cutnea, precursora de la glomerulonefritis aguda, cause
fiebre reumtica.
Protena M (serotipos 1, 3, 5, 6, 14, 18, 19, 24, 27 y 29)

ETIOPATOGENIA
Aproximadamente 3 a 6% de toda poblacin es susceptible
Nios de 5-15 aos mayor riesgo de faringitis estreptoccica.
Susceptibilidad a la ARF es una caracterstica hereditaria (HLA-II).

La expresin en alto grado de un determinado aloantgeno presente en los
linfocitos B, D8-17.
PATOGNESIS
Dao inicial se debe a

- +/- Exantema escarlatiniforme
Dentro de los criterios de Jones se establece ciertas manifestaciones clnicas :
CRITERIOS MAYORES
POLIARTRITIS Signo ms frecuente (75% ) .Grandes articulaciones: rodillas, los tobillos, las muecas y los
MIGRATORIA codos. es migratoria, errtica, cambiando en pocos das de una a otra articulacin.
Presenta signos inflamatorios con calor, rubor y tumefaccin con dolor intenso. La artritis
cura sin dejar secuelas. Rpida respuesta a los salicilatos.
Puede volver a la normalidad al cabo de 1-3 das sin tratamiento. La artritis grave puede
persistir durante varias semanas en los pacientes no tratados
CARDITIS Afectacin ms grave, se trata de una pancarditis que afecta principalmente a las vlvulas.
Suele aparecer antes de la tercera semana.
Aparicin de un soplo cardiaco significativo que antes no exista; la presencia de
cardiomegalia; el fallo cardiaco congestivo y la presentacin de una pericarditis.
Soplo cardiaco apical de insuficiencia mitral. El ms frecuente; es un soplo holosistlico,
intenso II-III/VI, que suele irradiar a la axila. Soplo mesodiastlico ,Soplo protodiastlico en
foco artico.
COREA DE 10-15%
SYDENHAM Son caractersticos la labilidad emocional, la incoordinacin, el bajo rendimiento
escolar, los movimientos involuntarios y la gesticulacin facial, que se exacerba por
el estrs y desaparece durante el sueo.
Las pruebas clnicas para poner de manifiesto una corea incluyen:
1) prensin de ordeador (contracciones irregulares de los msculos de las manos al
apretar los dedos del explorador).
2) supinacin y pronacin de las manos al mantener los brazos extendidos.
3) movimientos reptantes y rpidos de la lengua al intentar mantener la protrusin de
la misma y
4) el examen de la escritura para evaluar la movilidad motora fina.
ERITEMA Exantema raro (<3% ). Consiste en lesiones maculares, eritematosas, serpiginosas,

MARGINATUM: con centros plidos, no prurginosas.
Aparecen principalmente en el tronco y en las extremidades, pero no en la cara, y
pueden acentuarse al calentarse la piel.

El diagnostico clnico de carditis se basa en los datos de exploracin cardiaca
enumerados antes y especialmente en la aparicin de un soplo de naturaleza
orgnica, no apreciado con anterioridad.
El diagnostico de artritis reumtica surgir ante la evidencia de fenmenos

inflamatorios y, especialmente, tumefaccin, calor e impotencia funcional a nivel de
las articulaciones afectas.
Tambin se debe conceder valor a la afectacin poliarticular y al carcter migratorio

de las artritis, pero no puede olvidarse la existencia de formas monoarticulares.
El cuadro describe las enfermedades ms
importantes, cuya forma de presentacin clnica
puede confundirse con la FR.
El tratamiento de eleccin contina siendo la penicilina. Debe orientarse a eliminar
el germen, a controlar la inflamacin y a prevenir las posibles secuelas.
Reposo en cama. Es indispensable en la fase aguda y sobre todo, si

hay carditis.
Tratamiento antiinflamatorio. Salicilatos a 75-100 mg/kg/da
Tratamiento antibitico. Penicilina-benzatina.

La prevencin de la FR est en funcin del tratamiento adecuado de las infecciones
respiratorias estreptoccicas producidas por el estreptococo del grupo A.
Es la prevencin de los ataques iniciales de FR aguda y depende

faringoamigdalares
Prevencin Consiste en la prevencin de las recadas tras un primer ataque de

secundaria. FR aguda. Precisa de un programa continuado de profilaxis.

CATEGORA DURACIN
Fiebre reumtica con carditis y enfermedad 10 aos o hasta los 40 aos de edad (casi
residual del corazn (enfermedad valvular siempre es ms tiempo), algunas veces se da
persistente*) profilaxis de por vida (pacientes de alto riesgo)
Fiebre reumtica con carditis pero sin 10 aos o hasta los 21 aos de edad (casi
enfermedad residual del corazn (sin siempre es ms tiempo)
enfermedad valvular*)
Fiebre reumtica sin carditis 5 aos o hasta los 21 aos de edad (casi
siempre es ms tiempo)
*Evidencia clnica o ecocardiogrfica

1. La FR es una enfermedad inflamatoria sistmica, caracterizada
por la existencia de lesiones que pueden afectar articulaciones,
piel, tejido celular subcutneo, corazn y sistema nervioso,
siendo ms comn entre los 5 a 15 aos y muy rara por debajo
de los 2-3 aos.
2. El agente etiolgico de la FR es el estreptococo -hemoltico del

grupo A. Entre los factores que predisponen a padecer FR se
encuentran la susceptibilidad gentica y del agente; la teora que
mejor explica es la inmunolgica.
3. El cuadro clnico clsico se caracteriza por el precedente de una
faringoamigdalitis estreptoccica 2 a 3 semanas antes del brote agudo de
fiebre y los sntomas propios de la enfermedad. Estas manifestaciones han
sido agrupadas en criterios mayores y menores, siendo los primeros:
poliartritis migratoria, carditis, eritema marginado. Ndulos de Meynet y
corea de Sydenham. Dentro de los criterios menores son: sndrome febril.
artralgias.
4. Los criterios mayores para el diagnstico de la fiebre reumtica son carditis,

poliartritis, corea, eritema marginado, ndulos subcutneos; y los criterios
menores son fiebre, artralgia, fiebre reumtica previa, reactantes de fase
aguda incrementados.
5. El tratamiento de eleccin de la Fiebre Reumatoide contina siendo la

penicilina. Debe orientarse a eliminar el germen, a controlar la inflamacin y
a prevenir las posibles secuelas. Reposo en cama (es indispensable en la
fase aguda y sobre todo, si hay carditis), Tratamiento antiinflamatorio
(Salicilatos a 75-100 mg/kg/da) y tratamiento antibitico (Penicilina-
benzatina).
