ORAL HYGIENE INSTRUCTION

PRESENTATION
DH 220 A

INTRODUCTION
The patient that I selected for my OHI presentation is also my
Caries Management patient, and is also a part of the Fresh
Mouth Campaign at the LCSC clinic site.
The Fresh Mouth Campaign allows patients that smoke or use
tobacco products to receive help and support in their goals for
cessation through the Idaho Public Health Department; and
makes them eligible for a dental cleaning, electric Oral-B
toothbrush, and restorative work up to $200 at no charge to the
patient.
PATIENT INFORMATION
My patient is a female in her early 30s. She initially
came into the clinic seeking treatment due to pain from
several teeth in her mouth. However, after learning both
about the Fresh Mouth Campaign and her restorative
treatment options, she became very interested in
improving her overall oral health.
INITIAL PATIENT MOTIVATION FACTORS

Initially, this patient was only motivated to


improve her oral health due to the pain she
was experiencing.
1ST APPOINTMENT
Patient was disclosed, and intraoral photos were taken.
Her plaque index score was 58% Plaque Free.

RECOMMENDED OHI
Instructed patient on the Bass method of brushing,
Instructed about C-shaped method of flossing, and
discussed the importance of regular homecare. I also
recommended biotene, xylitol ice chips, and xylitol gum
for xerostomia.
Informed patient that she would be receiving an Oral-B
electric toothbrush at her next appointment, and
explained how she would be able to use it, and the
benefits.
MODIFIED PATIENT MOTIVATION
After a dentist exam at the first appointment, the patient was
made aware that she had multiple carious lesions that needed
to be restored. This motivated her to start a good homecare
routine, as she did not want any more teeth to become decayed.
Another motivating factor that changed was the patients interest
in smoking cessation. Aesthetically, she wanted to stop smoking
due to the stain it was causing on her teeth, and she also
wanted fresher breath. These factors also motivated her to
improve her homecare.
2ND APPOINTMENT
At the second appointment, I had my patient brush with the clinics
demonstration Oral-B toothbrush, disclosed and took intraoral
photos.
The patients plaque index score was very improved at this
appointment! She had gone from 58% plaque free to 95% plaque
free!

PATIENT RESPONSE TO OHI
My patient was very excited about her success with her homecare! She felt
that she had greatly improved, and that she had worked hard to achieve
homecare success. She also loved using her electric toothbrush, when she
received it, and felt it was easier to achieve excellent results with it instead of
a manual toothbrush.
At her initial appointment I had only begun to scale her teeth, and hadnt
completed a full dental cleaning, so at her 2 nd appointment, it was amazing to
see so much tissue change, mostly from her improved homecare!
Her color had improved from generalized slight hyperemia to a more pale
pink indicative of gingival health, and she only exhibited slight marginal and
papillary inflammation.
MODIFICATIONS TO HOMECARE
My patient was doing very well with homecare! I
encouraged her to keep up the good work, and
also showed her how to slide her mandible to
either the right or left in order to access the most
posterior maxillary molars more easily. Overall,
she was doing an excellent job!
REFLECTION
From my patients experience, it seemed that just making
some simple changes to her homecare routine not only
benefited her oral health, but also improved her overall
confidence. It was very rewarding to be able to provide
tools that would help my patients success, and to be able
to encourage her along the way. Behavior change does
not come easy, and it was very exciting to see that my
patient was committed to her own change, and was able
to experience such great success .
