TRIGGER 1
Mr.Shriram Subba,a 35 year old microbus
conductor felt pain on lower left back tooth
region for past 1 month
Pain was mild earlier & increased lately
Pain on having cold water & difficulty in
chewing
took painkiller
No problem in any tooth when he looked at
the mirror
FINDINGS:
Causes of tooth pain:-dental caries
-wisdom teeth
-mumps
-swelling of submandibular
gland
-dental trauma
Stimuli for pain:cold water i.e.sensitivity & chewing
Painkiller:ibrufen,nimesulide,paracetamol
Types of dental pain:dull & sharp
TRIGGER 2
Visited dentist & taken history
No family history of dental problem
Keeps hardboiled candies in his mouth at
work
Forgets to brush his teeth
FINDINGS:
Severity,intensity,progression
Aggravating factor:gum
recession,heat,cold,cavities,tooth
sensitivity,tmj disorder
Relieving factor:proper brushing,oral
hygiene maintenance,painkiller
History of night pain:decreased saliva
secretion
Brushing technique:Bass or sulcus
cleaning method
The rolling stroke method
The stillman method
The charters method
TRIGGER 3
Discolored area with chalky white
surrounding on 36 with a catch
Soft debris on distal aspect on occlusal
surface
Chipping of enamel on buccal surface
Small, brown colored crown was visible on
46
Foul smell from mouth
FINDINGS:
Causes of discoloration: staining, trauma,
internal bleeding, ageing, demineralization
Causes of chalky white color: poor
calcification, demineralization, flourosis
Causes of soft debris: accumulation of food
particles,
Causes of chipping of enamel: trauma,
extension of caries along DEJ
Causes of visibility of small portion of
crown: excessive wear, crack or broken
crown, complication of tooth decay
Causes of foul smell: oral infection, dry
mouth, smoking, food(onion, radish),drugs

IOPAR on 36
Black patch on crown portion
Assessment of pulpal status
Result showed healthy pulp
FINDINGS:
3 types of intraoral radiography: periapical
bitewing
occlusal
Indications of periapical radiography:
detection of periapical infections
assessment of periodontal status etc
Causes of black patch: caries, pus, abcess
Pulp tests: cold test, electric pulp test, heat test
Positive result of pulp test shows vitality of pulp
TRIGGER 5
He had deep dentinal caries in 36
Tests were done to confirm the diagnosis
Consequences of not treating it timely
Restoration of tooth as a treatment
Extraction of 46 followed by prosthesis
Prevention of caries
FINDINGS:
Consequences: pulpitis, abcess, granuloma or
cyst formation
Treatment : restoration, root canal treatment
Replacement can be done by removable
partial denture or fixed partial denture(tooth
supported or implant supported)
Caries prevention: reduce frequency of
sweets
maintain oral hygiene
using flourinated toothpaste
MINI PROBLEM
18 month old child with complaint of
multiple white spots and cavitated lesions
in oral cavity
Smooth surface caries on maxillary
incisors, first molars and canines
Mandibular incisors are unaffected
Child is fond of candies and has irregular
brushing habit
Breast fed and prolonged bottle feeding
Type of dental caries: baby bottle
caries(early childhood caries)
Risk factors: candies and bottle feeding,
irregular brushing habit
Clinical features of caries:- pain and
discomfort
-disturbed development of permanent
teeth
-loss of appetite
-disturbed occlusal development
-reduced esthetics
Mandibular teeth are unaffected from
caries:
mandibular teeth are blocked by tongue
during bottle feeding and hence
fermentable carbohydrates cant be
deposited there.
Preventive in children for dental caries:
-bottle feeding shouldnt be done
-habituated to regular brushing by
flourinated toothpaste
-decrease frequency of eating chocolates
