Etiologi of Hipertension

Essential Hypertension (Primary)

Hypertension of unknown cause. Occurs in
about 90% of patients with hypertension.
Sensitive salt
Genetic (derivative)
Homeostasis Renin
Age
Insulin resistance
Obesity
Sleep apnea

3
Etiologi of Hipertension
Secondary hypertension
1. 5-10% of patients with hypertension, kidney
disease and the cause is the renovascular
disease.
2. 2. 1 _ 2% of patients with hypertension,
kelaianan cause is hormonal and use of certain
medications.
. Kidney disease:
Kidney disease polikista
Trauma to the kidneys (wound)
glomerulonephritis
Irradiation of the kidney
Kidney Tumors

Drug of Hipertension
Pharmacology
1. Diuretics (thiazides)
2. Adrenergic inhibitors (Bolker alpha, beta-
blockers, alpha-beta blocker labetalol).
3. ACE inhibitors.
4. Calcium antagonists
5. Vasodilators (nitroglycerin, diazoxide, etc.)

*drugs this can be given its own or

combination

5
Drugs of Hipertension

Non-Pharmacology
- Lifestyle Modifications :
1. Weight Loss
2. Improving diet
3. A diet low in sodium
4. Physical activity (aerobic)
5. Do not drink alcohol and quit smoking

Clinical Trial Of Hibiscus Sabdariffa L.
Tea (Tisane)

In this studies result between H. sabdariffa L. and

placebo was not significant. Clinical trial was conducted
65 person with prehypertensive and mildly hypertensive
adults following a 6 week intervention with placebo
beverage and H. sabdariffa tea the result its compare.

15
Since that suggest daily consumption of hibiscus tea, in an
amount readily incorporated into the diet, lowers Blood
Preasure in pre- and mildly hypertensive adults and may
prove an effective component of the dietary changes
recommended for people with these conditions

16
Pharmacology Activity

There are some activities from Rosella, such as :

Antibacterial
Antihyperlipidemia
Antihypertension
Anti-inflammation
Anti diabetic mellitus

19
Pharmacokinetic Profile
The urinary excretion of
cyanidin-3-sambubioside : 0.016%
delphinidin-3-sambubioside : 0.021%,
and total anthocyanins: 0.018%
of the administered doses, respectively.
Maximum excretion rates were determined at
1.5 to 2.0 hours after intake.

20
Pharmacokinetic Profile

The dose-normalized plasma area under the curve

estimates were 0.076, 0.032, and 0.050 ng/mL/mg for
cyanidin-3-sambubioside, delphinidin-3-sambubioside,
and total anthocyanins, respectively.

The dose-normalized Cmax estimates were 0.036, 0.015,

and 0.023 ng/mL/mg in the same sequence. They were
reached each at 1.5 hours (median) after intake.

21
Pharmacokinetic Profile

The geometric means of t1/2 were 2.18, 3.34, and 2.63

hours for cyanidin-3-sambubioside, delphinidin-3-
sambubioside, and total anthocyanins, respectively.

The urinary excretion of intact anthocyanins was fast

and appeared to be monoexponential

22
Drug Interaction
Chloroquine, Acetaminophen And Diclofenac
Hibiscus sabdariffa caused a reduction in
the elimination of acetaminophen and
diclofenac and also a reduction in the
bioavailability of chloroquine.

The mechanism responsible for this

pharmacokinetic herb-drug interaction
has not been fully elucidated.

23
Drug Interaction

Hydrochlorothiazide
Plasma clearance and the elimination rate
constant of HCT (hydrochlorothiazide )
decreased with increasing dose of HSE
(hibiscus Sabdaariffa Extract) co-administered
with the HCT.

24
DOSAGE AND DOSAGE FORM

Herb therapy:
For hypertension therapy, Hibiscus
sabdariffa tea (consist of 65% of
Chinese hibiscus and 35% of Thai
hibiscus) is consumed 3 cup/day (720
mL/day).
