-There is evidence that some people have a better
understanding of the work

- The timescales for the Transforming Care work mean not
enough time is taken to make sure people and families are
understanding the information given.

- There are a number of people and families involved in the
Transforming Care work in their area
- Some good work has been highlighted in a number of areas
- The work in Kent showed the importance of meeting people
with complex needs in a way that works for them
- Some areas felt that inaccessible information was a barrier to
people being involved.

- Areas that were able to tell us about discharge
plans questioned how good they were
- There is some very good practice in Kent

More people and families were aware of the risk of
admission registers
- Some regions gave examples about things that were
being put in place to stop people going into hospital
- Two regions said family carers were more likely to be
involved in a meaningful way with planning what local
services and support look like than people with
learning disabilities and autism

Keeping people safe
- Most regions werent able to give evidence of people
with learning disabilities and families being involved
in local Safeguarding Boards as equal partners
- Regions have questioned if this is because of
concerns about keeping information confidential
- There is some work going on in the South East
exploring new ways of keeping people safe in their
community.

Changing how care and support
happens
- Only two regions London and the South East were able to
give any feedback in this area
- There is some evidence that some families are using
personal budgets to manage their relatives support very
successfully
- These families are well-informed with many skills to manage
a team of support staff
- There were concerns that this wasnt open to all
- Long-term planning for people who have very high support
needs is not well-thought out
