is committed to provide guidelines in
safety and health in accordance with the
government mandated Occupational
Safety and Health Standards (OSH), as
provided by the Department of Labor
and Employment (DOLE) and the Bureau
of Working Conditions (BWC).

The Department of Labor and


Employment (DOLE)
shall be responsible for the
administration and enforcement of
Occupational Safety and Health laws,
regulations and standards in all
establishments and workplaces
wherever they may be located.

Medical and Dental


Services
Book IV, Title I, Chapter I
of the Labor Code of the Philippines

This shall apply to all employers whether


operating for profit or not, including the
Government and any of its political
subdivisions and government-owned or
controlled corporations, which employ in
any workplace one or more workers. The
development and enforcement of dental
standards shall continue to be under the
responsibility of the Department of Health.

Every
employer
shall
keep
in
his
establishment such first-aid medicines and
equipment as the nature and conditions of
work may require, in accordance with such
regulations as the Department of Labor
shall prescribe. The employer shall take
steps for the training of a sufficient number
of employees in first-aid treatment.

It shall be the duty of every employer to


furnish his employees in any locality with
free medical and dental attendance and
facilities.

Emergency Hospital

An employer need not put up an emergency


hospital or dental clinic if there is a hospital
or dental clinic which is not more than five
(5) kilometers away or which can be
reached by motor vehicle within twenty-five
(25) minutes of travel and that the
employer provides transportation facilities.

Health Program

The physician engaged by the employer, in


addition to providing medical services in
cases of emergency, must also perform the
following:

Conduct
pre-employment
medical
examination, free of charge, for the proper
selection of workers.

Conduct free of charge annual physical


examination of the workers.

Collaborate with safety and technical


personnel in the selection and placement of
workers, including investigation of work
related accidents due to exposure to health
hazards.

Develop and implement a comprehensive


health program for the employees.

This shall apply to all establishments,


workplaces, other undertakings, including
agricultural enterprises, whether operating
for profit or not except the following:
(a) those engaged in land, sea and air
transportation;
(b) those classified as Mining industry by
the National Economic Development
Authority;
(c) and residential places exclusively
devoted to dwelling purposes.

The secretary of Labor shall, by appropriate


orders,
set
and
enforce
mandatory
Occupational Safety and Health Standards
to eliminate or reduce occupational safety
and health hazards in all workplaces and
institute new, and update existing programs
to ensure safe and healthful working
conditions in all places of employment.

Art. 163: Research

It shall be the responsibility of the Department of


Labor and Employment to conduct continuing studies
and research to develop innovative methods,
techniques and approaches for dealing with
occupational safety and health problems; to discover
latent diseases by establishing causal connection
between diseases and work in environmental
conditions; and to develop medical criteria which will
assure insofar as practicable that no employee will
suffer impairment or diminution in health, functional
capacity, or life expectancy as a result of his work and
working conditions.

Authority to Local
Government

The Secretary may authorize local government


units to assume responsibility for technical safety
inspection within their respective jurisdiction
where they have adequate facilities and
personnel and subject to national standards
prescribed by this department. Technical safety
inspection covers (a) safety determination of
mechanical equipment such as boilers, pressure
vessels, internal combustion engines, etc.; (b)
safety determination of electrical
installation.

is an established minimum requirements or


criterion on working conditions reasonably
necessary or appropriate to protect every
working man against the danger of injury,
sickness or death.

The goal of all occupational safety and


health programs is to foster a safe work
environment.

As a secondary effect, it may also protect


co-workers, family members, employers,
customers, suppliers, nearby communities,
and other members of the public who are
impacted by the workplace environment.

Each employer covered by the provisions of


this Standards shall:

Furnish his workers a place of employment free


from hazardous conditions that are causing or
are likely to cause death, illness or physical
harm to his workers;
Give complete job safety instructions to all his
workers, especially to those entering the job for
the first time, including those relating to the
familiarization with their work environment,
hazards to which the workers are exposed to
and steps taken in case of emergency;

Comply with the requirements of


this Standards; and
Use only approved devices and
equipment in his workplace.

Every worker shall cooperate with the


employer in carrying out the provisions of
this Standards. He shall report to his
supervisor any work hazard that may be
discovered in his workplace.

safeguards and safety devices furnished in
accordance with the provisions of this
Standards for his protection and that of
others, and shall follow all instructions given
by the employer in compliance with the
provisions of this Standards.

It shall be the duty of any person, including any


builder or contractor or enforcement agent, who
visits, builds, renovates, or installs devices, or
conducts business in any establishment or
workplace, to comply with the provisions of this
Standards and all regulations of the employer
issued there under as well as with other
subsequent issuances of the Secretary.

Rule 1013:
Hazardous
Workplaces
For the purpose of this Standards, the following
are considered Hazardous Workplaces:

Where the nature of work exposes the


workers to dangerous environmental
elements,
contaminants
or
work
conditions including ionizing radiation,
chemicals, fire, flammable substances,
noxious components and the like;

Where the workers are engaged in


construction work, logging, firefighting,
mining, quarrying, blasting, stevedoring,
dock work, deep-sea fishing and
mechanized farming;

Where the workers are engaged in the


manufacture or handling of explosives
and other pyrotechnic products;

Where the workers use or are exposed to


power driven or explosive powder
actuated tools;

Where the workers are exposed to


biological agents such as bacteria, fungi,
viruses, protozoas, nematodes, and other
parasites.

The training course prescribed by the


Bureau under this rule shall be a requisite
for the appointment of the safetyman in
place of employment.

Rule 1041: General


Requirements

In every place of employment, a health and


safety committee shall be organized within
(60) days after this Standards takes effect
and for new establishments within one (1)
month from the date the business starts
operating. In both cases the Committee
shall reorganize every January of the
following year.

Committee

Rule 1043.01:
Health and Safety
Committee
The Health and Safety Committee is the
planning and policymaking group in all matters
pertaining to safety and health. The principal
duties of the Health and Safety Committee are:

Plans and develops accident prevention


programs for the establishment.

Directs the accident prevention efforts of


the establishment in accordance with the
safety programs safety performance and
government regulations in order to prevent
accidents from occurring in the workplace.

Conduct safety meetings at least once a


month.

Review
reports
of
inspection,
accident
investigations and implementation of program.

Submit reports to the manager on its meetings


and activities.

Provide
necessary
assistance
to
government inspecting authorities in the
proper conduct of their activities such as
the enforcement of the provisions of this
Standards.

Initiates and supervises safety training for


employees.

Develops
and
maintains
a
disaster
contingency
plan
and
organizes
such
emergency service units as may be necessary
to handle disaster situations pursuant to the
emergency
preparedness
manual
for
establishments of the Office of the Civil
Defense.

Rule 1044: Terms of Office of


Members
Rule 1044.01: Health and
Safety Committee

In order to provide an opportunity for other


workers to become member and participate
in safety program planning, a periodic
change in membership is encouraged.

For this purpose, the term of office of the


department head in the committee may be one
(1) year. The term of office of the worker
members in Type A and Type B may be for two
(2) years each; in Type C, D, and E Committees,
may all be one (1) year. The chairman,
physician or nurse and the safety man shall be
permanent members of the committee.

Rule 1045:
Duties of the
Employers

Health and Safety committees play very important


roles in eliminating work hazards. Developing workers
interest and participation in the planning and
development of safety program is the responsibility of
the employer. The employer must exercise the
necessary leadership and provide support to make the
program work. The principal duties of the employer
are:

adopts
in
writing
administrative
policies
on
safety
in
conformity with the provisions of this
Standards outlining therein his responsibility
and authority delegated.

Reports to the enforcing authority in two (2)


copies of the policies adopted and the health
and safety organization established to carry
out the program on safety and health within
one month after the organization or
reorganization of the health and safety
committee.

Reports to the enforcing authority having jurisdiction


at least once in every three (3) months, counting
from January, the health and safety program of the
organization outlining the activities undertaken
including its safety performance, health and safety
committee meetings and its recommendations and
measures
taken
to
implement
such
recommendations.

Acts on recommended measure by the health


and safety committee by adopting the
elements of the health and safety program in
the production process or workplace and in
case of non-adoption of the Health and Safety
Committees recommendation, to inform the
committee of his reasons.

Rule 1046:
Duties of the

Works in accordance with accepted safety


practices and standards established by the
employer in compliance with the provisions
of this Standards.

Reports unsafe conditions and practices to


the supervisor by making suggestions for
correction or removal of accident hazards.

Serves as members of the Health and


Safety Committee.

Cooperates actively with the Health and


Safety Committee.

Assists government agencies in the conduct


of health and safety inspection or other
programs.

First Aiders
One who gives immediate care to a victim
who has been injured or suddenly ill.
Serves as the bridge that fills the gap
between the victim and the physician.
Will respond immediately to the scene once
alarmed.

Fire Extinguisher
Squad

Responds to small fires in their designated


area.

When the fire cant be handle anymore, FES


will notify the fire hose team (FHT) and the
Security & Communication Team (Guards on
duty) to warn the outside fire departments.

Fire Hose Team

They will try to cease the fire until the fire


fighters from outside arrives.

Attendance Checker

To know if someone is missing, ACs will


check the attendance of their assigned
areas and will report immediately to the
Pre-designated Assembly Point Captain
(PAP-C).

Circuit Broker

CBs are responsible to close the circuit


breaker as ordered by the Building Captains
(BC).

Floor Warden

Floor Wardens are responsible for the


immediate
response
to
emergency
situations.

Also, they are the leaders of evacuation for


their
respective
areas.
They
should
communicate with the BCs.

Security &
Communications
Team
The security and communications team will

communicate within the company or warn


outside agencies about the emergency in the
facilities.

They are also responsible to handle bomb


threat cases and will coordinate all the time
with the BCs and the PAP-C.

PAP Captain

On our Pre-designated Assembly Point (PAP),


the PAP-C will be the one who will supervise
and give orders to everyone in that area.

He will check the attendance in each


departments, and will monitor injured cases
with the first aiders as well as communication
with the building captains.

Building Captain

We will have two building captains (BC), one for MDI


Bldg. Office, and another for Fabrication Area.

These BCs are responsible to solve situations which


the FW cant handle, or would declare evacuation if
needed, and will ensure that everyone will evacuate
the building/area.

They are also the head of the Search & Rescue Team
and the security & communications team.

Search & Rescue


Team

Together with outside agencies, the search


& rescue team will work hand in hand to
search & rescue victims that are still inside
the company facilities

GENERAL
EMERGENCY
PLAN

1. DONT PANIC!

2. Evaluate if you
can respond to that
situation:

YES. Then tell someone closest to you about the


situation and go for it. The person notified by
the witness should immediately report it to the
floor warden (FW). If there is no one near you,
just go for it. If you solved it already, well done,
but if it just got worse, report it to the floor
warden (FW).
NO. Dont be a superhero, just report
immediately to the floor warden (FW).

3. The floor warden


(FW) will:

HANDLE THE SITUATION. They are trained.


NOTIFY OTHER SPECIAL DUTIES (FA, FES).
NOTIFY THE BUILDING CAPTAIN (BC). If
evacuation procedure is needed, the FW will
command someone nearest to him/her to
notify the building captain. The building
captain is responsible to notify all personnel
and visitors when it is necessary to
evacuate the facility/building.

EVACUATION
PROCEDURE

1. DONT PANIC and


listen for information
or instructions.

2. Employees seated near


windows shall secure that
windows are close. Dont
jump out of the window,
unless eagerly needed.

3. Any employee next to a


restroom or a vacant room shall
check that room and, if anyone is
there, instruct that person(s) to
proceed to the pre-designated
assembly point.

4. Employees are not to


take anything with them
unless ordered by their boss
for important documents or
in case of fire.

5. Floor wardens will be the


last person to leave in their
assigned areas, ensuring
everyone to be evacuated
first.

6. The Pre-designated
Assembly Point (PAP) are
sidewalks of the road
outside the MDI area.

7. Employees should move


quickly and safely as
possible without creating
more hazards than already
encountered.

8. Routes:
For MDI Building 1 Office. Take the lobby entrance
going to Gate 2.
For MDI Building 1 Fab. You can take the exit located
at the left side (where delivery materials are
released) going to Gate 1, or the right side entrance
going to Gate 2.
For MDI Building 2 Machine and Hot Works Area. You
can go directly to Gate 2, or take the improvised
ladder going to Gate 3.

9. If primary exits are blocked, for any


reason, then go to alternative exits.
For MDI Building 1 Office Accounting &
Purchasing Dept. If the lobby entrance is
blocked, take the second floor route to
executive office going to the fire exit door and
going out to Gate 1. For HR Dept., you can take
the warehouse entrance door.

10. If a fire is in the corridor just


outside the room or close
enough to the exit route, then
close the door block all gaps
around the door until a move
can be decided.

11. Attendance in each area


should
be
checked
by
Attendance Checker (AC), and
should report directly to the Predesignated
Assembly
Point
Captain.
