protein malnutrition and high interdialytic weight
gain as well as to regulate blood glucose level.

Nurse and physician should instructed the patient and


family members about the hypoglycaemia attacks
and the proper use of insulin drug.

potassium is essential to reduce further complications

Nurse dietitian collaboration

A nurse makes a request for assistance of a


dietitian for a patient having an existing issue
in nutrition , whether such patient may be under
nourished as consequential to the frequent
dialysis.

The dietitian will then discuss the nature of the


patient 's issues followed by nutritional
assessments and diagnosis on the case .

Once the data has all been gathered , the dietitian
will then discuss the course of action to implement
in which they (the nurse and the dietitian )
will both plot out pertinent nutritional plan specific
for the patient 's needs .
The nurse can then reinforce the plan of action
prescribed by the dietitian by explaining to the
patient the relevance and essence to acquire
cooperation as well as discussing the importance
aspect that the patient must remember .

As a nurse, it is essential to we give up some


power and trust on other members of the health
care team are just as important in providing
comprehensive, quality care.
