Administering Z-track

Injection

Z-track is used to prevent
backflow of medication
into subcutaneous
tissue.
Insert needle at 90
angle, aspirate, and
administer if no blood
is present. Withdraw
needle before
releasing skin.
Definition
Assessment
1. Reviews physicians order for clients name, drug name,
dose, time and route of administration.
2. Gathers drug reference information pertinent to drug(s)
ordered.
3. Considers factors that may contraindicate Z-track
injection.
4. Assesses clients medical history, history of allergies and
medication history.
5. Assesses integrity of selected site and size of the muscle.
6. Determines clients age and weight.
7. Determines clients ability or willingness to cooperate.
Planning
8. Organizes equipment and supplies.
8.1. syringe 2-3 ml for adult; 0.5 to 1 ml for infants and
small children
8.2. needles (2): 21to 25 gauge, 1 to 1 inches for
adults; 1 inch for children
8.3. 2 x 2 gauze pad (optional)
8.4. medication ampule or vial
8.5. disposable gloves
8.6. medication card
9. Follows the three checks for administering medications.
Read label on the medication:
9.1. when it is taken from the medication cart
9.2.before withdrawing the medication; and
9.3. after withdrawing the medication
Implementation
10. Closes room curtain and /or doors.
11. Performs hand hygiene and apply gloves.
12. Keeps sheet or gown draped over body parts not
requiring exposure.
13. Calculates and prepares correct dose from ampule or
vial. Prepares the syringe for injection.
14. Changes needle after drawing up.
15. Assists patient into prone position; with toes pointing
downward.
16. Cleans site with alcohol swab. Apply swab to center of
site and rotate outward in circular direction for about 5cm(2
inches).
17. Holding the syringe between the thumb and forefinger,
pierces the skin quickly and smoothly at a 90degree
angle, and insert the needle into the muscle.
18. Holds the barrel of syringe steady with your non-
dominant hand, and aspirate by pulling back on the plunger
with your dominant hand. Aspirates for 5-10 seconds. If blood
appears in the syringe, withdraw the needle, discard the
syringe and prepare a new injection.
19. Withdraws the needle while releasing the skin.
20. Applies gentle pressure at the site with a dry sponge or
cotton swab.
21. If bleeding occurs, applies pressure with a dry, sterile
gauze until it stops.
22. Repositions patient and place call light within reach.
23. Disposes off equipment properly.
24. Wash hands.
Evaluation
25. Evaluate the clients response to medication within an
appropriate time frame.
26. Document any significant findings.
27. Administration of drug is guided by the 10Rs of drug
administration.
