DEFINICION: ADA: Es un grupo de enfermedades metablicas cuyo nexo en comn es la hiperglucemia secundaria a un dficit de la secrecin de la insulina, a un defecto de su actividad metablica, o ambos; teniendo como sntomas: poliuria, polidipsia, prdida de peso y polifagia. OMS: Es una enfermedad crnica que aparece cuando el pncreas no produce insulina suficiente o cuando el organismo no utiliza eficazmente la insulina que produce.

Comnmente ocurre en la niez y adolescencia, pero puede ocurrir a cualquier edad (8 - 9 dcadas de la vida).
Results from a progressive insulin secretory defect on the background of insulin resistance. Corresponde entre el 90% y 95% de pacientes con diabetes. Inicialmente no se necesita del tratamiento con insulina para sobrevivir y no ocurre destruccin de las clulas . La mayora de pacientes son obesos y la obesidad causa resistencia a la insulina. El riesgo de desarrollar esta forma de diabetes se incrementa con la edad, la obesidad y el sedentarismo.
3. ENFERMEDADES DEL PNCREAS EXOCRINO: Tenemos los siguientes procesos: pancreatitis, trauma, infeccin, carcinoma pancretico; las cuales pueden llevar a una reduccin en la masa de las clulas con el posterior reemplazo de tejido fibrtico.
4. ENDOCRINOPATAS: Tenemos Acromegalia, Sindrome de Cushing, el Glucagonoma y el Feocromocitoma; cuyas hormonas son antagonistas de la accin de la insulina. 5. DIABETES INDUCIDA POR DROGAS: Las cuales pueden no causar Diabetes por s mismas, pero podran precipitarlas en caso de resitencia a la insulina, por ejemplo: la nicotina y los glucocorticoides. 6. INFECCIONES: Virus: Coxsackie B, citomegalovirus y adenovirus. 7. FORMAS NO COMUNES DE DIABETES MEDIADA POR MECANISMOS INMUNES: Sindrome del hombre rgido: desorden autoinmune que se caracteriza por la formacin de anticuerpos antireceptor de insulina.
EPIDEMIOLOGIA
En el mundo hay ms de 346 millones de personas con diabetes
Se calcula que en el 2004 fallecieron 3,4 millones de personas como consecuencia de exceso de azucar en sangre.
Mas del 80% de las muertes por diabetes se registran en pases de ingresos bajos y medios Casi la mitad de esas muertes corresponden a personas de menos 70 aos y un 55 % a mujeres. La OMS prev que las muertes por diabetes se multipliquen por dos entre 2005 y 2030 La dieta saludable, la actividad fisica regular, el mantenimiento de un peso corporal normal y la evitacin del consumo de tabaco pueden prevenir la diabetes tipo 2 o retrasar su aparicin
DIAGNOSTICO
Current criteria for the diagnosis of diabetes in nonpregnant adults are shown in Table 2. Three ways to diagnose diabetes are recommended at the time of this statement, and each must be confirmed on a subsequent day unless unequivocal symptoms of hyperglycemia are present. Although the 75-g oral glucose tolerance test (OGTT) is more sensitive and modestly more specific than the fasting plasma glucose (FPG) to diagnose diabetes, it is poorly reproducible and difficult to perform in practice. Because of ease of use, acceptability to patients, and lower cost, the FPG has been the preferred diagnostic test. Though FPG is less sensitive than the OGTT, the vast majority of people who do not meet diagnostic criteria for diabetes by FPG but would by OGTT will have an A1C value well under 7.0% (6).
Though the OGTT is not recommended for routine clinical use, it may be useful for further evaluation of patients in whom diabetes is still strongly suspected but who have normal FPG or IFG (see Section I.C). The use of the A1C for the diagnosis of diabetes has previously not been recommended due to lack of global standardization and uncertainty about diagnostic thresholds. However, with a world-wide move toward a standardized assay and with increasing observational evidence about the prognostic significance of A1C, an Expert Committee on the Diagnosis of Diabetes was convened in 2008. This joint committee of ADA, the European Association for the Study of Diabetes, and the International Diabetes Federation will likely recommend that the A1C become the preferred diagnostic test for diabetes. Diagnostic cut-points are being discussed at the time of publication of this statement. Updated recommendations will be published in Diabetes Care and will be available at diabetes.org.

DIAGNOSTICO
1. SINTOMAS DE D.M. + VALORES DE GLUCOSA PLASMATICA >=200mg/dl .cualquier momento del da 2. GLUC.PLASMTICA EN AYUNAS(GPA) >=126 mg/dl
. Ingesta calrica por lo menos 8 horas
Estos criterios deberan ser confirmados mediante la repeticin de los Test en un da diferente.

Test de Tolerancia Oral a la Glucosa (TTOG): - Glucosa 2 horas post TTOG < 140 mg/dl = Tolerancia normal a la Glucosa. - Glucosa 2 horas post TTOG de 140 199 mg/dl = Deterioro de la Tolerancia a la Glucosa. - Glucosa 2 horas post TTOG 200 mg/dl = Dx provisional de Diabetes (se debe confirmar en un da diferente).
DIABETES MELLITUS GESTACIONAL: Se realiza en base a un Test de Tolerancia Oral de la Glucosa con una carga de 100 g 75 g de Glucosa. FACTORES DE RIESGO: 1) Obesidad (el de mayor riesgo) 2) Antecedentes familiares de DM 3) Antecedentes obsttricos: Abortos a repeticin, prematuridad, macrosoma FACTORES fetal,etc. 4) HTA crnica, preeclampsia, candidiasis, ITU a repeticin. 5) Antecedentes personales de DM gestacional. 6) Glicemia basal 126 mg/dl en 2 das diferentes 2 valores al azar 200 mg/dl = DM gestacional, sin necesidad de hacer screening.
SCREENING: Test de Osullivan: 1) 24 28 semanas de gestacin 2) Se administra 50 gramos de glucosa y se determina la glucemia basal y 1 hora post ingesta. 3) Si el valor es 140 mg/dl se practica un Test de Tolerancia Oral de la Glucosa.
.Deteccin de diabetes en personas de 45 aos a mas.Si es normal,se debe repetir el examen c/3 aos. . En <45 aos , se debe realizar en personas: -OBESAS(>120% peso deseado o IMC >27) -con ANT.de partos con nios recien nacidos >4kg o Dx. De diabetes gestacional .HTA .HDL<35 mg/dl trigliceridos>250mg/dl .TGA o GAA

La periodicidad de los controles mdicos y de laboratorio depende de la necesidad de cada paciente. Lo ideal es realizar mnimo un Cada 60 a 90 control cada 3 meses.

Examen fsico y pruebas de laboratorio similares a los de evaluacin inicial para reevaluar el tratamiento.
Comprende tres estudios bsicos: agudeza visual (siempre con paciente metablicamente compensado), fondo de ojo y tonometra. Considerar, de ser necesario, la realizacin de examen Oftalmolgico cardiolgico vascular perifrico y neurolgico.
Consulta nutricional
Revisiones peridicas de los hbitos de alimentacin del paciente. Mantener una dieta saludable; Llevar una dieta que refleje sus gustos y estilo de vida. Tener una ingesta calrica apropiada al peso corporal deseado; Mantener un horario apropiado de comidas. Adecuar su dieta a situaciones especiales (ej. reduccin de la ingesta de sal en hipertensos).
