Program Planning
Initiates the planning

Implementation of Plan
Specified plans to be implemented from the National Health Office

III. Supervision and Monitoring

Supervision to nurses and midwives

IV.

Evaluation
Done monthly, quarterly or annually Evaluation of how many children were immunized in that particular barangay and if the number of FIC goal was met.

V. Validation of Reports

Consolidation of reports monthly, quarterly, annually Inventory checking of how many vials were given by the National Health Office and how many were used.

VI.

Coordination or Reporting Services

Agency linkages If the City Health Office could not handle the case or it is out of their jurisdiction and capabilities
