Known diabetic and hypertensive for 12 years following last pregnancy Controlled on Insulin and Enalpril Attends Chaguanas health facility every 6 months for management of both
Visual assesment Foot care

Has never been hospitalized for either condition since diagnosis Compliant with all medication

Non-functioning glucometer, no self blood pressure monitors at home


Recieves medication and insulin needles via CDAP Knows how to self administer insulin

Does not have a record of glucose measurements Recently her insulin does was increased due to poor readings at the health center. May have been precipitated by a major stressor

Patient feels her medical conditions are under control Not viewed as adversities Does not affect her daily living

Patient described a loving relationship Since his death she feels depressed
Depressed mood, difficulties sleeping and concentrating

Her greatest concern following the death is her financial needs

Son is now forced to fill this role.


Patient dislikes the fact that her son now has to fill this role.

Her other children also contribute what they can. Patient would like to go back to work, however her disability poses a problem Patient is unaware of the social welfare benefits.
