You are very welcome to NEMBE CREEK from your off-duty, wishing you a very wonderful stay during your work period as you resume. 1) HSE ISSUE Key Issues: Manage contractors HSE, work site PTWs, JHAs, ets. Manage HSE plans and implementation Ensure that all HSE issues are properly edited before implementation/ close out Always ENSURE that you carry along other team member as family with one vision. HSE reporting to Gabriel and Gladys I. should be giving prompt priority. The UA/UC for the period is 10. 2) Key Challenges and Issues that Require Urgent Attention. The following issues needed prompt and adequate attention to close them out the fact that we are the action party and that the target date are close because of the critical nature of it. Do follow up on the installation of the outstanding kitchen equipment; issue was discussed during the last leadership meeting held on 28-04-2012. Do also follow up on the repair of the two faulty hoods in the kitchen as this poise a serious unsafe condition for workers in the kitchen. The CLO building has some snag list that needed to be completed, I have engaged the contractor on this and he promised to complete it within this week. Do follow up with him to ensure completion. The provision of notice board at strategic location in the FLB is now to be providing by HSE focal point. Do follow up with him for close out. Follow up on the provision of foot and bath mat from Madam Nike and Ngozi. Follow up on the engraftment of Nembe district materials such as TV, fridges, radio etc as a way of inventoried infrastructure materials in the district. Follow up on provision of 63amps switch gear for the new laundry. This was one of the recommendations by Tin industrial to fix the faulty laundry equipment in the new laundry. Follow up on the installation of analogue temperature gauge on the cold room. There is a plumbing issue in the kitchen; I have asked the Nestoil plumber to block it because our maintenance contractor was not at site. Please, call the maintenance contractor to close out this action. Do ensure that the kitchen vicinity is kept clean always. The fly catchers must be cleaned regularly.

The exposed cable at the WTP still poses a serious unsafe condition for workers around that vicinity. This we must close out to avoid potential incident. People are complaining of poor room services; do ensure adequate supervision of this function. The maintenance contractor had handover the keys of the CLO building and they are in the bed side locker. There are some bulbs issues in the accommodation and clinic, I have discussed with the maintenance contractor and he promised to handle it this week. Do call him to close out this action. Most importantly do send all the list of outstanding maintenance Ayenor copy me and madam Ada for follow up and close out. NOTE; ALL ACTIVITIES TO BE INVOLVE IN SHOULD BE WITHIN THE FLB. 3) CONTRACT MANAGEMENT There are two contracts to be managed in this facility these include the maintenance contract and the catering and Housekeeping contract. Do ensure that management of the said contract is in line with the contract terms. Escalate any challenge to the office for advice. I held a meeting with the maintenance contractor representative and the maintenance team lead on the way forward to deliver acceptable services to our customer. Lots of factors were identified to be the possible cause of poor services rendered by the contractor. Going forward we had equally we have identified solution areas if implemented might address must of the challenges we are currently facing in this maintenance contract. Do liaise with the maintenance team lead to debrief you. Ensure that all faults request from customer are properly written down for accountability. 4) MEETINGS AND INSPECTIONS Please, as usual take note of the following meeting and inspection date. As usual do attend the normal morning toolbox meeting with the asset team. Time remain 0700Hrs. Leader meeting on Saturday by 1000am is also mandatory. Note that most of the infrastructural action items are still open. Therefore, you need to touch base with the office on how to close them out soon. 5) CIVIL/ELECTRICAL FLB MAINTAINACE: Key issues: Contractor was not on site during the period that I was on duty. However, a meeting was held with him on how to improve services. The management of the contractor is entirely your responsibility, therefore, ensure that you manage him well to deliver. 6) STAFF/PERSONNEL ON DUTY THERE ARE 7 STAFFS ON DUTY 7) PROJECT: Key issues: NO PROJECT IS IN PROGRESS

8) PLANON: Key issues: As I earlier mentioned, the maintenance contractor did not carry out any work during the period that I was on duty; hence, there is no overdue planon jobs to be loaded on planon. However, the contractor claim that there are jobs he did and has not being loaded on planon; you need to liaise with Adebayo to clear this issue. Very important you do this. 9) STOCK LEVEL: The stock level as at today is as follows; Dry food; 5Days Frozen food; 5Days Vegetables/fruits; 5days Water; 6days
