Research and development

There is a long way from the first idea till the realization. The designing of each device is preceded by surveying of the beauticians' demands: in addition to defining the treatment objection, we pay attention to the cosmeticians' other requirements as well. The selecting, afterwards the testing of the most appropriate treatment methods, and finally the combination of the treatment programs of the devices are all based on this information. The last phase is the ergonomic and electronic designing with meticulous care. The first prototype will be further developed depending on cosmetic testing paying attention to the medical considerations as well. The series production begins after further evaluations and technical improvements, thus the first buyer of the device will be just as satisfied as the one who buys another piece later on. Our dermatologist specialists support the successful functioning of our devices by testing the effectiveness, and also take part in the further improvement of the instruments. So the dermatologist work-team of Dr Derm equipment took part in the fourth world congress of International Academy of Cosmetic Dermatology (IACD) in Paris, in which the results of skin rejuvenation made with the Dermabrasion Professional device were described. In the presentation entitled Surface evaluation of living skin before and after microdermabrasion of photodamaged skin Gza Garay MD and Gbor Varju MD demonstrated the effects of the microdermabrasion with photos taken by microcamera and digital skin analysis. Their presentation describing their results was also selected into the scientific program of the annual meeting of the European Academy of Dermatology and Venereology (EADV) held in London in 2005 (OpticalPhotoanalytical Evaluation of the Skin Surface during Microdermabrasion Treatment Course). We use their scientific results for the product development and for the improvement of the recommended treatment method.

The blood vessels in the skin surface treated with galvanic current dilate and blood flow is stimulated. Oxygen and nutritional supply to the cells improves, as well as the removal of waste materials and skin functions. Owing to the vasodilating effect, the speed of absorption of active ingredients may be multiplied. As a result of these, the iontophoresis device has been pertaining to the basic devices for decades. Though the use of iontophoresis has been limited by the appearance of ultrasound, but even today, the iontophoresis devices can be applied the more effectively to take in water-soluble, charged ingredients. However, the use of galvanic current has various side effects as well: it causes the migration of ions in the cells, which tilts over the normal equilibrium of ions in the cells and alters their charge, beyond that causes a shift of the skin pH value. With the exception of the fact that we use this effect during the deep-cleaning (desincustration), this is to be seen as a non-desirable side effect of the iontophoresis treatment, exceptionally because these changes influence the intake of active ingredients and cause itching. Another side effect of the use of galvanic current is the stimulation of the nerves responsible for vascular reactions. The blood vessels in treated area dilate, thus skin types prone to reddening (rosacea) must not be treated with galvanic current. Iontosoft
During galvanic treatments with traditional instrument, when the contact between the handpiece electrode and skin is interrupted, the client experiences a mild electric shock. This inconvenience is well-known by the beauty salon-goer clients, thus many people are reluctant to treatments done with current. Our own development, the iontosoft patient protection electronics integrated in the microderm 2000 monitors the contact between the skin and the handpiece, and immediately intervenes if the contact is broken down. When the beautician touches the skin with the electrode again, the device slowly and gradually restores the output to the preset value. Due to these new technical solutions, those clients would come back as well that before have shrunken back from the iontophoresis treatments.

Microphoresis
After selecting the microphoresis treatment program, the microderm 2000 device carries out a treatment section consisting of three steps.

The first phase of the treatment is LYMPHATIC MASSAGE: the handpieces oscillate at a constant speed, and the movements of the electrodes follow the usual directions of lymphatic massage - simultaneously on both cheeks, from the centerline outwards, to the lymph nodes. The fine vibration triggers the lymph flow in the lymphatic vessels of the face and the neck, thus reducing the oedema of the face and the eye region, stimulating the removal of waste materials, therefore the healing of problem skin.

The second phase of the treatment is STIMULATING-NUTRITION: we have to set only the polarity according to the type of active ingredient applied during the treatment. The handpieces oscillate at a constant speed and conduct autopolar current into the skin as well. First we apply gel ampoule and moisturising gel on the skin, and then we work these active ingredients entirely into the skin by conducting the handpieces with circular, slow and smoothing movements on the surface of the skin. The third and last phase of the treatment is REFLEX STIMULATION: the handpieces oscillate at a continuously changing speed but don't conduct current to the skin. The electrodes are kept on the reflex points symmetrically on both cheeks for a few seconds. The reflex stimulation stimulates the metabolic processes of the skin and refreshes our client.

The viofrim is a combination of two treatment instruments, which is designed for disinfecting with high frequency current and face cleaning with a circular brush. The high frequency glass tube of the instrument contains rarefied inert gas, which starts ionizing by the effect of high frequency. The ions start glowing upon contacting themselves and the wall of the glass tube. Approaching the glass electrode to the skin, sparks jump to the skin surface. High frequency causes capillaries to dilate, blood supply to improve and the created ozone disinfects. The device is to treat skin with seborrhoea and acne, to adjust the excess function of sebaceous glands, to disinfect after cleaning, to improve blood supply, and to refresh the skin.

Vital processes depending on temperature
The metabolism of the skin and muscles depends on temperature: an increase of 1oC results in 13% increase in the speed of the metabolism. Oxygen consumption of cells is multiplied by 2-3 if the temperature is increased by 10oC. The skin responses to the fluctuations of temperature through its regulatory mechanisms: blood flow through increases or decreases, the blood vessels dilate, and more sweat is secreted. Changes of temperature are perceived by the quite special antennas of the skin, by the thermoreceptors. The points of cold and hot perception are present in the skin in different numbers: there are 1-5 cold points in an area of 1 cm2, while the number of hot points is only 1 in an area of 2.5 cm2. The face skin is the most sensitive area of the whole body in respect of heat-effects: here are 16-19 cold points in a unit of skin surface. That's why the cold treatment carried out with the thermoderm 2000 device has such a spectacular effect. Heat receptors are active between 30-45oC their maximum sensitivity is somewhere between 37-43oC. That's why the temperature of the skin-ironing program of our instrument can be set between 36-44oC.

The heating up of the skin

Due to perspiration, superficial thermal therapy results in removal of waste materials from the skin. The thermal effect makes the vessels dilate, thus the blood flow through of the skin is improved, as well as the nutrition of the skin and the removal of waste materials. The dilated pores facilitate the penetration of active ingredients: the emulsions, ointments and serums penetrate more effectively than without the thermal effect. The catalysts of metabolic processes - the enzymes - work with higher intensity at higher temperatures, thus metabolism accelerates and building processes become more emphasized. The collagen and the effect exercised on the flexible fibres make the skin to become more elastic.

Thermophoresis
The device has been designed to carry out one of the three electro-cosmetic methods for intake of active ingredients, the thermophoresis with heat, thus this is its main function. This method is suitable for the intake of every water or oil-soluble preparation, especially for the dissolution and more uniform spreading on the skin surface of oilier and denser cream masks. This method without direct current effect means a safety alternative in all cases when the patient couldn't be treated with current (pregnancy, pacemaker, afraid of treatments carried out with current), and when the use of ultrasound is contraindicated (for example the treatment of tissues of the breast or over sensitive areas), or when the ultrasound can't penetrate into the treatment products (for example preparations consisting powder). The handpiece is evidently made from such type of material that doesn't get into chemical reactions with the active ingredients, therefore doesn't influence their biological activity.

Skin-ironing and trap of active ingredients

The essence of the most complex treatment program of the device is that in the hot phase (36-44oC) and in the cold phase (18-25oC) - using the optimal ranges of temperature for the function of the heat receptors of the skin - can influence the metabolic processes. The heating up is objected to stimulate the metabolism in the skin, the cooling down prevents from this. During the first phase the pores dilate, the active ingredients can penetrate more intensively and more effectively. Due to the full-bloodedness effect, the skin receives more oxygen, that stimulates its metabolism and triggers the removal of waste products.

During the second phase of the treatment the pores contract as a result of the cooling down, thus the skin traps the introduced active ingredients. The decrease of the temperature reduces the activity of the enzymes as well, making metabolism to return to its original level, and by the cooling down the reddening of the skin ceases as well.

Special heating-cooling handpiece

The thermoderm 2000 device treats the skin gently. If the handpiece touches the skin surface, the heat is conducted by the tissues, thus the temperature of the treating area alters. Careful dosing of the heating and cooling effect is ensured by the automatic adjustment of the temperature values programmed in the software of the instrument. The device controls continuously these fluctuations, and sets the intensity of heating or cooling from moment to moment according to it. The cooling down of the skin surface - similar to the function of a fridge - causes formation of heat. Effective cooling and fast changing of temperature can be provoked only if the created heat is conducted away effectively. In the inside of the unique handpiece a tiny fan ensures perfect heat conduction: this costly but safety solution was chosen in order to make the skin-ironing and trap of active ingredients programs to function perfectly and that your hand woldn't be subjected to direct heat effect during the treatment.

Advanced Comedolytic & Normalizing Effect by Heat, Electricity & Light for Prevention & Treatment of Acne.

The acne
More than 80% of the world population suffers at least once in a lifetime from the signs of acne. The different types of acne can be treated the most effectively with medicament for internal use or with preparations for external application, and with the combination of these two methods. More and more data have been published in the last years about the dangers of antibiotics used locally: a resistance for antibiotics of the bacteria develops making the further treatments with medicaments unsuccessful. Nowadays a general opinion about various processes of laser and light therapy has been gaining around that mild and middle severe acne can be treated successfully with some treatments without any side effect.

In the centre: the P. acnes

The chronic inflammation of the sebaceous gland and the hair follicle belonging to it (the so-called pilosebaceus unit) causes the acne. Under normal circumstances the oily material produced by the sebaceous glands gets to the skin surface along the fine hair present in the hair follicle, and there, by spreading, contributes to the building up of the protective film in the skin surface - the so-called hidrolipid film.

The acne process begins when a comedo builds up in the common way leading out from the pilosebaceus unit, thus blocking the clearing of sebum. Thus the sebum gets stuck and the bacterium responsible for the inflammation - the Propioni-bacterium - multiplies in it.

Finally: Acne develops with comedos, pus blisters and hurting nodules, which lasts for a long time and normally causes scars.

The bacterium makes the sebum to become irritating fatty acids, which have a role in the developing of inflammation. Due to different chemotactic factors, leukocytes are gathering together as well, which cause the inflamed infiltration evolving around the sebaceous gland.

Photoinactivation
The extirpation of the Propioni-bacterium, which plays the main role in the pathophysiologic process, causes the stopping of the acne process, thus the mitigation of subjective symptoms. Our objective was to introduce an electro-cosmetic method that without the well-known side effects of medicines, with direct or indirect electric effect could reduce the mild form of acne. In order to be able to understand the joint effect of the three different methods used in our new device, we have to learn about the characteristics of the pathogen bacterium before. During its biological activity, as many other creatures, the P. acnes produces porphyrins, a natural group of compounds, which occur in the human organism as well. If the porfirins are exposed to light of a certain wavelength, free-radicals develop, which extirpate the bacterium. The porphyrins can absorb the energy the most effectively in the spectrum of visible blue light (between 400-430 nm), thus the developing of free-radicals and the extirpation of bacteria can be stimulated the most effectively with this type of light. This means the photoinactivation, which can ruin the multiplying bacteria in the interior of the sebaceous gland - in an area where the cosmetics can't penetrate - with the help of light.

BRH (Blue-Red-Heat) Handpiece

Our development was based on several scientific examinations, during one of these the blue light with low intensity has been found effective in the treatment of mild or middle-severe acne. In this research, the patients involved in it were treated for 15 minutes, twice a week, totally four weeks long. A 60% improvement was observed by the 80% of the subjects, which proved to be long-lasting for 3-8 months. During another research, the blue light was combined with 660nm-wavelength red light, which proved to be significantly more effective in respect to acne on the whole, than the blue light in itself or one of the ingredients of the preparations for external use applied against acne, the benzoyl-peroxid. The more important is the anti-inflammatory effect of the red light, which derives from the fact that this spectrum of light can influence the release of cytokins coming from the macrofags or other cells. The cytokins control the function of the cells of connective tissues and the production of growth factors, thus the red light stimulates the running of the inflammation and healing process. We have developed the BRH (Blue-Red-Heat) handpiece basing on these two scientific results, which can radiate simultaneously blue light at the ideal wavelength between 400-430 nm and the 660 nm-wavelength red light directly to the skin surface, thus can reach a required intensity in the deeper layers of the skin as well. During the light therapy, the treating area that touches the skin is heated up to the preset temperature by a heating cell. Considering the fact that the enzymes work more intensively at higher temperature, we can stimulate the metabolism of the skin cells and the bacteria that cause the inflammation to a higher activity, accelerating the process in this way. During the treatment carried out with the BRH handpiece the inflammation is mitigated, the number of bacteria is reduced, the healing of acne and the regeneration of the skin is accelerated by the effect of the blue and red light and because of the heating up of the cells. The BRH handpiece can be used for stimulating of aging, tired skin, for the regeneration of the fibres of connective tissues, for tightening and rejuvenation as well.

Heated HF handpiece
The high-frequency current makes ozone to develop, which extirpates entirely the causative agents in the surface of the skin and the area near to the surface of the way out of the sebaceous gland. The heated HF handpiece helps loosening the comedos, and contracting the pores after cleaning, in addition prevents from infection of the micro-injuries created in the surface of the skin.

HSR -probe (Heat Shock Response treating probe)

The sudden heating up makes special proteins of the genetic stock of the P. acnes bacteria multiplying in the sebaceous glands to be transferred. These are the so-called heat shock proteins, which finally cause the self-destruction of the bacteria. The HSR-probe should be hold on the pustule to be treated for a short while, and during this time conducts such a high temperature into the skin that triggers the developing of proteins, thus the destruction of causative agents. The heat-conduction occurs by impulses to prevent from the damaging of the surrounding tissues. The HSR treatment programs of the device feed automatically the required quantity of heat, and each time there is a sound effect that warns the user to change to treatment of another pustule.

The ultrasound penetrates several millimetres into the skin, and delivers its effect there. Enhances the absorption of active ingredients by loosening the horn layer on the skin surface. The blood vessels dilate, the permeability of cell membrane increases, as well as metabolism and lymph flow, and the inflammation subsides. Ultrasound stimulates the collagen metabolism and fibroblasts, the removal of waste materials, the diffusion processes and the blood circulation. CDM
During the ultrasound treatment a contact agent (e.g. a gel) provides the free way of the waves between the handpiece and the skin. Dr Derm ultrasound devices are equipped with a contact checking mechanism, which gives a warning beef if contact between the handpiece and the skin is interrupted. This is the so-called CDM automatics, which controls the penetration of ultrasound into the skin and prevents the handpiece from heating up. The device gives a warning sound signal as well when the treatment material isn't the suitable quality and hinders the penetration of ultrasound, for example contains a lot of powder material, air-bubbles or not enough small vegetable parts, because these don't conduct the vibration of ultrasound. With the CDM-automatics the ultrasound treatment always can be carried out with outstanding effectiveness and results.

Real treatment time counter

Owing to its contact checking mechanism, the Dr Derm ultrasound devices only measure the time really spent on treatment. This advanced technological solution guarantees the efficiency of the treatment and the introduction of necessary doses of active ingredients. Due to the real treatment time counter, the effects carried out on the skin, for example the stimulation of the introduction of active ingredients or the filling of the energy reservoirs of the cells will be more effective, than during those treatments that are carried out with devices that don't have any contact checking mechanism.

Pulse mode of operation

Due to its thermal effect, continuous ultrasound heats up the tissues, that has various positive effects. During long treatment periods or when deeper layers should be treated, may be uncomfortable. By properly selecting the modulation value, deeper layers may also be treated without heating up of the handpiece or the skin.

Fountain effect
If the handpiece is turned upwards and water is poured on its surface that touches the skin, the ultrasound makes the water-molecules to move and a special effect may be observed. This is the so-called fountain effect, with which the mechanical effect of ultrasound can be demonstrated. If the handpiece emits ultrasound continuously, the sparkling and evaporation (cold vapour) of water should be observed in the whole surface of the handpiece. In the case of a device with CDM-automatics, the effect of ultrasound may be observed only when there is enough water poured on the surface. The reason for this is that the device perceives the conduction of ultrasound in the treating area, and works with the preset power only when this conduction is perfect. Through the fountain effect the proper function of the crystal can be controlled as well. By devices the handpiece of which doesn't work adequately, the water is heated up quickly or there isn't any sparkling produced. Our laser-ultrasound handpiece emits the light only if there is a perfect contact between the treating area and the skin, ensuring a safety treatment in this way. If water is poured onto the treating surface, by triggering the fountain effect the light sources are lit as well.* * We should take care and wear protective eyewear.

Small handpiece
The small handpiece facilitates to perform an optimal output necessary for treating the face skin. Because of the ergonomically perfected shape and weight-distribution of the handpiece, every part of the face can be reached easily without skating over the minor details. The ultrasound emitted by the small handpiece has a minor penetration depth, considering the sensitive structure of the skin in the face and in the neckline.

The programs offer treatment possibilities both for face and body treatments. After selecting the required treatment program, the device sets automatically the proper parameters, thus you can spend all your time on the client. The R&D group of Dr Derm devices had decided to design a revolutionary new handpiece. As a result of a long designing work, an ultrasound handpiece has been designed, which is combined with red-coloured phototherapy semiconductors and an optical source of light with laser. The instrument, which can be used for face and body treatments as well, has inherited all the advantages of our ultrasound devices. The handpiece controls continuously the contact with the skin, and allows to emit the ultrasound only if the whole treating area touches the skin. It controls the quality of the products applied on the skin as well, and when there is a treating material that the ultrasound can't go through, makes a warning sound signal. The Continuous Dosis Monitoring (CDM) developed by us makes even the operation of the source of laser light to be safety, as it lightens only if there is a perfect contact between the handpiece and the skin. Sono-CoolAir the new ultrasonic handpiece for body treatments

It's an old observation that lypolsis during body treatments is the most effective if the handpiece heats up the tissues through the continuous emission of ultrasound, thus improving the release of lipids accumulated in the fatty cells. The only difficulty is that as an effect of continuous ultrasound not only the deeper layers of the skin but its surface is heated up as well, thus could cause uncomfortable sensation of heat, maybe undesirable damaging of the epidermis. In order to prevent from this, the ultrasound had been made fractional till now, certainly reducing the effectiveness of the treatment by this. The celluloderm, the body shaping and face lifting instrument of Dr Derm device family with unique characteristics, was designed more than a decade ago. The larger and larger need for body treatments and the favourable experiences achieved with the device had forced us to continue to develop this instrument already used in more hundreds beauty salons all over the world.

The renewed exterior hides a new interior

During the further development of the celluloderm device we set three objectives: the improvement of effectiveness, the more extensive applicability and unaltered dimensions. We have reached the improvement of effectiveness through the doubling of the number of channels (4 channels, 16 electrodes) and the increase of the performance by almost a 50%. We offer a more extensive applicability of the device through the new treatment programs, and the new type of electrodes for the face and body. We have considered the unaltered dimensions to be important because of the conception of device family: thus the celluloderm-4 RC muscle stimulator and body shaper instrument with computer could be fitted into the mounting for our devices as well.

Stimulating current and interference current

Stimulating currents are those current forms, which cause the muscles to contract - holds the old definition. The temporally interrupted direct current with a frequency between 10-100 Hz really evokes a muscle contraction. However, the skin resistance to low frequency currents is very high, and the current also stimulates the receptors of pain, which restricts efficiency. The more intensive current you conduct into the skin through the electrodes, the more effective muscle contractions you can evoke - and the more uncomfortable sensation, however. Currents in the middle frequency range (e.g. over 1000 Hz) penetrate into the skin more easily, thus they got to the focus of attention. If two currents of different frequency (e.g 3950 Hz and 4000 Hz) are directed to a given area so that they are crossing each other, then a resulting vibration of 50 Hz appears, accompanied by its characteristic physiological effects. The result is phenomenal: the efficiency of interference current surpasses that of the traditional stimulating current.

Versatility and effectiveness

Additional ones complete the musclestrengthening effect: the current stimulates metabolism and has a positive influence on the function of other tissues as well. A typical example is the effect made through the LDL-receptors on fat metabolism: the interference current inhibits the cholesterol transport in the cells, reducing the accumulation of fat inside the cell, which finally leads to loss of excess fat. Sympathetic stimulation causes the release of free fatty acids in the subcutaneous region, which also has a lypolitic effect. Electrical stimulation affects blood flow and lymph flow as well, and reduces oedema. Owing to its effect on keratinocytes, it enhances the maturation of cells, thus stimulates skin regeneration. The end result is a thinner fatty tissue, better muscle tone and more flexible skin.

Pleasant flow
By using the new device we can forget the misbelieve that efficient body shaping would require powerful electric and a jungle of wires. Unlike traditional instruments using stimulating current, this new idevice doesn't cause reddening of the skin under the electrodes; there is no sensation of electrical effect even during longer treatment sessions. Remote control of intensity

Body shaping treatment with electric current has an interesting feature, which can't even be eliminated by automation - i.e. computer control. The tissues of the treated area get accustomed to the treating current, so amperage must be adjusted - depending on individual characteristics of the client - from time to time. At this point the beautician - interrupting her/his work at another treating bed - hurries to the client being treated with the device. Maybe experienced client is asked to press the appropriate buttons of the device (though this isn't easy if the device is out of reach of the client). The remote control of intensity developed for the celluloderm 2000-4 RC device solves this problem. With the help of the unit placed in the client's hand, the intensity of the current of the four channels may be adjusted separately, depending of the sensation of comfort. Treatment may be temporarily suspended by pressing the SAFETY STOP button: the availability of an emergency stop will thus surely lay the client's fears, if any.

The lymph is a fluid located in the intercellular space, which consists of the fluid being secreted in the arterious side of capillaries and the waste material created during the metabolism of cells. Thus the lymph is in direct contact with the cells and the blood vessels. Any chemical or biological change of the organism will affect lymphatic production and the composition of the intercellular fluid. The lymph is collected in the lymphatic vessels, and filtered by the lymphatic glands located along the lymph routes as a filter system, thus cleaning it of harmful substances. Improper nutrition, excess uptake of fat and carbohydrates, smoking, or drinking coffee, and use of oral contraceptives influence the work of the lymphatic system - due to the growth of fatty cells, damage to lymphatic vessels, and depositing of waste materials. Hyperhidrosis device
Hyperhydrosis is a state with excessive perspiration originating from the increased excitement of the sympathetic nervous system, the disease may involve the whole body surface, but symptoms occur on hands and feet more frequently. Hyperhydrosis, beyond that causes difficulties in the daily life and work, could trigger various dermatologic diseases (e.g. eczema on hand, infections caused by virus or mycosis). There are various effective therapies, with which symptoms of the disease can be treated. Of these, tap water iontophoresis with galvanic current is one of the cheapest and most effective methods, and which, after local anti-perspirant agents, it is the first among therapies to be applied*.

The treatment of hand and foot hyperhydrosis

During treatment, the electrophysiological effects of galvanic current on skin are applied. Two active electrodes become connected to the body, which are located in separate tanks filled with water. During treatment, the therapeutic current migrates from one electrode to the direction of the other. The most obvious explanation for the effect is that chlorine ions in tap water decrease the production of perspiration in the perspiratory glands. The results of the treatment vary with each person treated.

Hyperhidrosis device from Dr Derm

By analysing the physiological effects of tap water iontophoresis, the Dr Derm company developed a device, with the use of which treatments can be implemented in the simplest way for each patient, either at the outpatient clinic or at home. The plastic casing of the Hyperhidrosis therapeutic device contains two separated water tanks, which are for treatment of the two hands or two feet. After filling up the device with tap water and putting it into operation, it operates for 10 minutes automatically. After switching it on, you only have to set current intensity to the required value and correct it according to the individual current sensation.

In the first phase of treatment, 3-5 treatments of 10-20 minutes per occasion are recommended weekly. In order to achieve long-lasting results, regular treatments are needed, which temporarily can be interrupted.
