Documentation Portfolio Assessment Plan

Objectives of the portfolio: To provide a more accurate picture of students' specific achievements and progress by putting together in a portfolio the documents pertaining to the skills, ideas and accomplishments of the learners.

Content of the portfolio: 1. Title page of the portfolio including the following information: Title: A Documentation Portfolio in Elementary Algebra Name of the student Year and section Date of submission 2. Table of contents Title of the Activity Page Number Reflection 3. Works of the students Activity 1 Topic: Measurements Objective: Students are expected to demonstrate behaviour which shows appreciation of the importance of measuring devices. Task: Writing a reflection paper on a video about measuring instruments. (Affective domain) Activity 2 Topic: Real Number System Objective: Students are expected to illustrate integers on a number line. Task: Constructing a model of the number line. (Product-oriented) Activity 3 Topic: Algebraic Expressions Objective: Students are expected to illustrate algebraic expressions using algebra tiles. Task: Creating algebra tiles. (Product-oriented) Activity 4 Topic: First Degree Equations in One Variable Objective: Students are expected to translate verbal statements involving general or unknown quantities to equations. Task: Translating verbal phrases to mathematical expressions. (Process-oriented)

Activity 5 Topic: Linear Equations in Two Variables Objective: Students are expected to draw the graph of Ax + By = C on a table of values for x and y. Task: Sketching the graph of ax + by = c based on a table of values for x and y. (Psychomotor domain) Activity 6 Topic: Special Products and Factoring Objective: Students are expected to factor polynomials. Task: Factoring out trinomials which are products of two binomials. (Process-oriented)

4. Holistic or analytic scoring rubrics for every lesson Measurements Real Number System Algebraic Expressions First Degree Equations in One Variable Linear Equations in Two Variables Special Products and Factoring 5. Reflection of the student about their activities

Procedure: 1. Design a portfolio assessment plan for the students with the corresponding rubrics. 2. Provide an activity to be accomplished by the students for each lesson. Measurement Real Number System Algebraic Expressions First Degree Equations in One Variable Linear Equations in Two Variables Special Products and Factoring 3. Design scoring rubrics for each activity. 4. Before the end of the school year, ask the students to compile their works in a documentation portfolio. 5. Ask the students to include their reflections in the portfolio.
