Certificate of Registration (Board Certificate) & valid PRC ID STCW 95 Certificates (Certificate of Competency and Endorsement Certificate) Medical Certificate from DOH accredited medical clinic/hospital with QSS in place, using the DOH approved seafarers pre-employment medical form, which must not be more than one (1) year from the date of issuance. If submitted during application for STCW 95 Certificates, should be not less than fifteen (15) days prior to expiry date. Approved seagoing service a. Company Certification of Sea Service shall include the type of vessel, the rank occupied, vessels gross tonnage, vessels trading area and inclusive dates of sea service. b. Seafarers Identification Record Book (SIRB) one photocopy each of pages one and two, and other pages reflecting sea service (In case of lost/damaged SIRB, any acceptable documentary proof of sea service and affidavit of loss shall be required) Two (2) piece passport size picture in proper uniform with white background. Long brown envelope National Assessment Center (NAC) Certificates for training courses under Regulations V & VI (Basic Safety Course, Advance Firefighting, Medical Emergency First Aid, Proficiency in Survival Craft and Rescue Boat) GOC (with at least a year validity) ECDIS and Fast Rescue Boat (If applicant will board a ship equipped with ECDIS and Fast Rescue Boat)

Category A Marine Deck Officers with expired STCW 95 Certificates who have served on board seagoing ship/s for at least one (1) year accumulated approved seagoing service during the preceding five (5) years from the date of filing of the application for revalidation. Category B If an applicant was unable to serve on board seagoing ship/s for at least one (1) year accumulated approved seagoing service during the preceding five (5) years from the date of filing of the application for revalidation, he must comply with the following: 1. Basic Safety Training 9. Cargo Handling and Care of Cargo 2. Ship Simulator and Bridge Teamwork (Operational Level) 10. MARPOL 73/78 Annexes 1 & 2 3. Advance Fire-Fighting 11. ECDIS (if applicant will board a ship equipped 4. Medical Emergency First-Aid with ECDIS) 5. Proficiency in Survival Craft and Rescue Boat 12. Proficiency in Fast Rescue Boat ( if applicant 6. GOC (with at least a year validity) will board a vessel equipped with fast rescue 7. Radar Simulator Course boat) 8. ARPA AND MUST HAVE PERFORMED ANY OF THE FOLLOWING RELATED FUNCTIONS FOR AT LEAST ONE (1) YEAR DURING THE LAST FIVE (5) YEARS FROM THE DATE OF FILING OF THE APPLICATION. 1. ON BOARD FILERS (Additional Requirements) thru companys authorized representative recognized PRC (For applicants under Category A only) 1. Certification under oath that the marine officer is on board at the time of the application and that the documents submitted are genuine and authentic, duly signed by the companys responsible official 2. Telex from the master/principal 3. POEA Contract 4. Authorization letter for the companys authorized representative (both from the company and from the on-board officer) 5. Crew List signed by the Master and with vessels stamp 6. All documents to be submitted must be stamped certified true copy by the company, duly signed by the responsible official.
