1.1

Introduction
Purpose

The purpose of the Contractor HSQE Conditions Manual is to ensure that all contractors who work on or about Serco Dubai Metro (SDM) premises are aware of their Health and Safety, Quality and Environmental (HSQE) responsibilities. For the purposes of this document, contractors is taken to mean all staff that are not directly employed by SDM.

1.2

Objective

The principal objective of the Contractor HSQE Conditions Manual is the control of HSQE risk to and from contractors on SDM Premises.

1.3

Scope

The Contractor HSQE Conditions Manual is a contractor facing document that applies to all contractor works (whether let by SDM or a third party) that result in non-SDM staff being present on SDM Premises.

1.4

Process

The Contractor HSQE Conditions Manual contains descriptions of all HSQE requirements of contractors undertaking work on Serco Dubai Metro Controlled Infrastructure. As part of the contractor management process, those conditions that are applicable to the contractor and/or the work activity will be identified on a contractor / work activity specific menu. In outline the process is as follows: SDM Sponsor identifies those conditions applicable and submits to the contractor; Contractor reviews the completed menu and either accepts or rejects the condition, where a condition is rejected, this should be supported by an explanation. This annotated menu is then returned to the Sponsor; The SDM Sponsor reaches agreement with the contractor regarding applicable conditions. If agreement cannot be reached between the Sponsor and the contractor, the work may not proceed.

2.1

Contractor Safety Conditions


Contractors Responsibility for Safety
fully and unreservedly accept that responsibility for the safety of the work activity rests with the contractor; work with any other employers or contractors sharing the workplace or providing employees (such as agency workers), so that everyones health and safety is protected, and; be responsible for compliance with all relevant regulations and applicable standards (including this manual).

Organisational Arrangements

The contractors shall appoint member(s) of their management team who have defined authority that includes: ensuring that a safety management system is implemented and maintained; reporting to senior management on the performance of the safety management system, including needs for improvement; ensuring awareness of SDM safety requirements throughout the contractor's organisation; liaison with the Serco Sponsor on matters relating to the contractor's management system that result from auditing or non-conformances; documenting all related corrective actions; ensuring that contractors and sub-contractors work to correct practices including accepted method statements defining: o safety of others (including the public); o health and environmental precautions; o plant and equipment to be used (including Personal Protection Equipment), and; o training and licensing requirements; ensuring that the works comply with any manufacturer's safety related recommendations, instructions and guidelines.

2.3.1

Access Conditions
Permit to Work

All works on Serco Dubai Metro Controlled Infrastructure are subject to the Permit to Work (PTW) Procedure. The purpose of the Permit to Work (PTW) procedure is to ensure that planned works (and where required, unplanned works) are assessed, and appropriate controls applied to mitigate risk to the safety of customers, staff, contractors and third parties. The PTW process is an indispensable part of the integrated management system which ensures that:

a safe system of work is developed comprised of appropriate measures to manage the identified risks/hazards that this safe system of work is established by a competent Person In Charge (PIC) to manage the safety of a workgroup working within a worksite; others are not unduly affected by the acts or omissions of this workparty.

The PTW process will achieve this in all PTW scenarios by ensuring: work activities are planned and co-ordinated to avoid unplanned and unwanted events that may affect safety and system performance; all risks are avoided or mitigated by using suitable precautions; arrangements are clearly communicated to all concerned parties to mitigate any detrimental impacts; common resources may be shared to maximise efficiency and effectiveness e.g. passenger trains/ Engineering Vehicles utilised to support multiple worksites.

The safe performance of works across the three stage PTW system (pre-application planning, approval and coordination and implementation on site) can only be achieved by all the responsible parties to the PTW process working together to consistently plan, approve, perform and complete the range of engineering, maintenance and third party works. SDM have identified individuals within Serco to act as Sponsors of Contractor Activity. The Sponsor is responsible for reviewing contractors submissions under the PTW process and ensuring that all SDM interested parties are consulted regarding the works. It is the Sponsors recommendation to the Planning Manager that a PTW Application may be approved as being safe to implement. The contractor will comply with the PTW process as required by the Serco Sponsor. The PTW Procedure is detailed at SDM procedure DM001-ENM-ALL-PC-00019 and is available from the Serco Sponsor.

2.3.2

Contractors use of a nominated applicant for PTW applications

The contractor is required to appoint an appropriate nominated applicant, who should be made known to the Serco Sponsor, for the completion of PTW applications. PTW applications will be accepted from applicants other than the nominated applicant but this may result in a delay in the issuing of permits resultant from the increased likelihood of errors within the application.

2.3.3

External approvals, permits and licences

There are external approvals that are pre-requisites to SDM approvals, and vice versa. Therefore the contractor shall present details of the schedule of approvals required for works to the Serco Sponsor prior to submission to ensure all approvals will be applied for in a timely manner. The contractor must apply for all relevant external licences, permits and permissions and ensure they are in place in the agreed timescales. Contractors shall ensure that all external approvals, permits and licenses are sought and obtained prior to commencement of work, these include, but are not limited to RTA and Dubai Municipality requirements.

2.3.4

Clash Management

SDM operates a system called the Works Priority Hierarchy as follows (highest priority first):

SDM uses this hierarchy in the event of clash (a work party being unable to conduct work on site safely due to another work party) to decide which work party should conduct their work first. It is the contractors responsibility to inform the SDM Landlord of the premises where the work is being conducted of the clash with another work party immediately that they become aware of it to allow the work prioritisation to be decided.

2.4.1

SDM Permits and Activity Based Risk Assessments


Isolation Permits

Mechanical and electrical isolations are subject to safety permits. The application for a these safety permits must be submitted 6 weeks in advance of the works commencing and are only valid once signed by an electrical / mechanical isolation safety permit authorised person. Each Permit shall: Have a stated validity period; be signed by a designated SDM Electrical / Mechanical Authorised Person (as appropriate); only be valid for the locations affected; detail the electrical/ mechanical precautions that must being employed; be available for inspection at site

The contractor shall identify where Isolations Permits are required within the scope of their works and comply with the stated requirements.

2.4.2

Excavations

Excavation on or about SDM premises is specifically subject to a safety permit. The application for an excavation safety permit must be submitted 6 weeks in advance of the works commencing and is only valid once signed by an excavation safety permit authorised person. The details of the process to be employed are detailed in Safety Permit Excavation DM001ENM-ALL-WI-00835. Each Permit shall: have a stated validity period; be signed by a designated SDM Excavation Authorised Person;

only be valid for the excavation locations affected; detail the method of work and precautions that must being employed for the excavation; be available for inspection at the site of the excavation

The contractor shall identify where Excavation Permits are required within the scope of their works and comply with the stated requirements.

2.4.3

Hot work permits

Hot Works are specifically subject to a safety permit. The application for the safety permit must be submitted 6 weeks in advance and is only valid once signed by a Hot Works safety permit authorised person. The following works are strictly prohibited on SDM Premises unless a Hot Work Permit has been obtained: flame cutting burning welding track welding grinding any operation producing sparks the use of a thermic lance plasma cutting

Each Permit shall: have a stated validity period; be signed by a designated SDM Hot Works Authorised Person; only be valid for a single location and work site; detail the method of work and precautions that must be employed for the work including whether or not fire protection and detection systems require isolation (If there is likely to be a substantial interval between working shifts, arrangements shall be made for the fire protection or detection system to be reinstated during this period or for an hourly inspection of the isolated area to be undertaken); be available for inspection at the site.

The contractor shall identify where Hot Works Permits are required within the scope of their works and comply with the stated requirements.

2.4.4

Consultation on Safety Permits

The contractor shall discuss the proposed works (i.e. scope of work and method of working) with the Safety Permit Authorised Person before applying for a permit. Contractors shall submit applications for permits at least 6 weeks before the intended start date. This period of notice shall extend to 8 weeks where the isolation of fire detection or prevention equipment or exemptions are required. In an emergency a lesser period of notice may be given provided that a contingency method statement has been approved by the Safety Permit Authorised Person to cover such eventualities.

2.4.5

Suspension and Withdrawal of Safety Permits


in an emergency; if the conditions attached to the permit are not followed; if it is necessary for any safety requirements.

2.4.6

Working at Height

Working at height is defined as work in any place, including a place at or below ground level or obtaining access to or egress from such place while at work, except by a staircase in a permanent workplace where, if measures were not taken, a person could fall a distance liable to cause personal injury.

Where working at height is undertaken, the contractor shall: conduct and submit a Working at Height Risk Assessment. An accepted template for a Working at Height Risk Assessment is detailed in the SDM Form DM001ENM-ALL-XX-00064 - Working at Height Risk Assessment Template. devise safe systems of work for such activities; ensure that all work at height is carried out safely ensure that any person responsible for erecting, or dismantling access equipment has successfully completed a recognised training course for that activity.

2.4.7

Work in Confined Spaces

A confined space is defined as any space of an enclosed nature where there is a risk of death or serious injury from hazardous substances or dangerous conditions (e.g. lack of oxygen). Where work in confined spaces is undertaken, the contractor shall: Conduct and submit a Confined Space Risk Assessment. An accepted template for a Confined Space Risk Assessment is detailed in the SDM Form DM001-ENMALL-XX-00065 Confined Space Risk Assessment Template devise safe systems of work for such activities; ensure that all work and entry into confined spaces is carried out safely assess each work location independently taking fully into account any limited access and potentially poor ventilation; ensure that any person identified through the risk assessment process as being involved in the confined space activity has successfully completed a recognised training course for working in confined spaces.

2.4.8

Work on or adjacent to utilities and buried services

The contractor shall obtain all information on the location of buried or other services in the location prior to commencing work on site. No construction activity may be carried out in proximity of any services unless the utility provider and the Serco Sponsor have given prior permission. The contractor shall comply with all the requirements of public utilities companies when carrying out work on or adjacent to utility services. The contractor shall make all necessary arrangements with the utility engineer to safeguard the cables or apparatus.

2.4.9

Access to electrical sub-stations, working equipment, relay and other secure rooms

When access is required to designated rooms, all personnel must be demonstrably fit and competent to access and conduct work in these rooms. Keys to these rooms are only issued in response to a specific need submitted in writing to the Serco Sponsor. The Serco Sponsor must approve details of the nature and duration of the work and the exact number and location of the workforce. No contractor shall work in such rooms without authority to do so. All equipment within such rooms must be protected by a dust-proof screening. The contractor shall not use such rooms as stores or workrooms.

2.4.10 Demolition
The contractor shall: be a demolition firm approved by the Serco Sponsor, and; produce a demolition plan and submit to Serco Sponsor for approval.

2.5.1

Competence Requirements
English language

The default language for all contractors to SDM shall be English and this requirement shall extend to: verbal communications; all written communications and instructions including any training or technical material provided by any contractor following the modification, renewal or replacement of any asset.

The Serco Sponsor may permit the presence of contractor's team members who are not competent in the English language provided that the contractor can demonstrate to the satisfaction of the Serco Sponsor that: such team members receive the required safety training / briefing (including any emergency procedures) before commencing work in a manner that they can fully understand; such team members will be under the charge of a Person In Charge (PIC) who is fully competent in the English Language; the Person In Charge (PIC) ensures that instructions (including any associated temporary signage) are effectively communicated to, and understood by, all such team members whilst on SDM premises.

2.5.2

General competence requirement


ensure that both its own staff and those of any sub-contractors under its control possess sufficient qualifications, skills and experience to fulfil their duties; maintain records of such competence for examination by the Serco Sponsor; ensure that its staff (and those of its appointed sub-contractors) are competent and fully aware of all applicable SDM requirements and the controls and process to

manage the health, safety and environmental risks pertinent to both the works and individual staff roles. Where required by the Contract and before starting work on site, attend and achieve the required standard in the courses agreed with the Serco Sponsor. These courses will cover the procedures and work instructions to ensure that staff are safe on or about the operating railway and shall be delivered by an SDM accredited training provider.

The cccredited training provider will issue a certificate to the contractor's personnel who have successfully completed a course. This certificate must be: carried at all times whilst on duty, and; produced for inspection when requested by SDM staff.

The certificate will not be renewed on expiry unless the contractor's personnel have attended and passed the appropriate refresher training course. All training certificates issued by the accredited training provider remain the property of SDM and are valid for the duration stated on the certificate. No certificate shall be valid if it has been defaced. SDM reserves the right to withdraw the training certificate at any time. Contractors should allow sufficient time to ensure that its staff are properly trained and registered before starting work. If the contractors staff fail to produce the stipulated licences when requested to do so by any member of SDM staff, they shall be required to leave the SDM premises immediately. No cost or delay incurred by the contractor as a result of such a failure will be accepted by SDM.

2.5.3

Competence of safety critical staff

The list of activities that SDM deems safety critical are available from the Serco Sponsor in the Safety Critical and Safety Significant Roles Matrix DM001-HES-ALL-MI-00250. The contractor shall: identify all the works that they will be conducting that are safety critical under SDMs definition for the Serco Sponsors approval; demonstrate the competence, fitness to work, qualifications and training of the staff identified to conduct the safety critical activities.

The contractor shall provide a means of identification for all staff employed on safety critical works on site. This method of identification will be agreed with the Serco Sponsor prior to commencing work.

2.5.4

Asset specific competence

Suppliers must demonstrate to the Serco Sponsor the competency to perform works on specific assets as required by the governing standards. Relevant standards can be obtained from the Serco Sponsor.

2.6.1

Fitness for Duty


Drugs and Alcohol

The contractor shall comply with the SDM Drugs and Alcohol Procedure DM001-HES-ALL-PC00066, which is available from the Serco Sponsor. This procedure includes the requirement for random testing for drugs and alcohol and testing post incident and for cause.

2.6.2

Control of hours worked

The contractor shall have in place a management system to ensure that their employees do not carry out safety critical work in circumstances where he is so fatigued or where he would be liable to become so fatigued that his health or safety or the health or safety of other persons on a transport system could be significantly affected. Compliance with the SDM Fatigue Management Procedure DM001-HES-ALL-PC-00065, which is available from the Serco Sponsor, would satisfy this requirement.

2.6.3

Medical requirements

All contractors shall comply with SDMs medical requirements. These are defined for staff conducting Safety Critical tasks and available from the Serco Sponsor.

2.7
2.7.1 2.7.2

Behaviour
Smoking Removal of suppliers personnel from SDM Premises

Smoking is not allowed on SDM Premises unless in designated smoking areas.

Contractors shall take appropriate actions to prevent and eliminate unacceptable behaviour or conduct. SDM and the Serco Sponsor (or their representatives) reserve the right to object to and (where appropriate) immediately expel any of the contractor's personnel or sub-contractors employed on the works.

2.8

Personal Protective Equipment

Personal Protective Equipment (PPE) is defined as equipment 'which is intended to be worn or held by a person who is at work and which protects him or her against one or more risks to his or her health and safety'. Contractors shall identify the PPE to be used by their staff and their sub-contractors whilst on SDM premises within their method statements, which must be agreed by the Serco Sponsor prior to work commencing. The contractor will be responsible for supplying all PPE agreed for use on SDM premises and that it is: properly assessed before use to ensure it is suitable; maintained and stored properly; provided with instructions on how to use it safely; and; used correctly by its staff (including sub-contractors) whilst on SDM premises.

2.8.1

Use of Hi-Visibility clothing

The contractors personnel shall wear high visibility clothing, which identifies their employer at all times when on or about the operational railway. Where, following a risk assessment, the wearing of a separate high visibility garment could provide a risk of personal injury, then the contractor shall agree other arrangements with the Serco Sponsor to ensure that the contractors staff are easily identified. Contractors staff shall not wear SDM branded high visibility clothing.

2.9

Welfare Arrangements

The contractor shall ensure that welfare arrangements are in place during working hours at all work sites in accordance with Labour Law No. 8 (1982).

The contractor shall ensure that personnel have access to adequate welfare facilities. Where the intent is to use SDMs facilities an application should be made in advance to the Serco Sponsor for approval. The contractor shall ensure that personnel are aware of the welfare arrangements provided.

2.9.1

First aid

The contractor shall ensure that personnel have access to adequate first aid facilities, and ensure that personnel are aware of the first aid facilities provided. The Serco Sponsor shall co-ordinate such arrangements where contractors agree to share such facilities. The contractor shall ensure that suitable numbers of trained first aiders are available during working hours at all work sites

2.10

Communication

2.10.1 Safety Communications


If the contractors staff are required to convey safety critical information to SDM staff this should be conducted in accordance with the SDM requirements which are set out in Safety critical communications DM001-HES-ALL-WI-00255. The Serco Sponsor may require the contractors staff to demonstrate competence

2.10.2 Radio transmitters and transceivers


Contractors wishing to use radio sets, transmitters and receivers on or about SDM Premises shall only do so following agreement from the Serco Sponsor. The Serco Sponsor shall consider: an assessment of any risk arising from: o the use of such equipment, and; o the failure to use such equipment; output power; allocated frequency.

The use of radio transmitters and transceivers in Signal Equipment Rooms is prohibited.

2.10.3 Mobile phones


Mobile phones of any description shall only be used when the user is in a place of safety or if there is an emergency.

2.11

Housekeeping
keep the work location in an orderly state appropriate to the avoidance of: danger to persons; and adverse impact on the environment. take all necessary precautions to avoid unauthorised access. ensure that the work site is free from all the contractors equipment, materials and waste: o on completion of the works, or; o when the site is returned to normal operations, unless covered by a storage licence. make suitable provision for disposing of unused or redundant assets and materials, and are responsible for determining the correct disposal methods. These shall be approved by the Serco Sponsor before implementation.

2.12

Plant and Equipment

All plant and equipment to be used on SDM premises must be justified within the method statement and approved by the Serco Sponsor and if required by the Serco Sponsor (e.g. for cranes, lifting gear, pressure vessels), must have been inspected by an SDM approved third party prior to work commencing on site.

2.12.1 Access equipment


Contractors shall: strictly observe any restrictions imposed by the Serco Sponsor on the use of access equipment; ensure that equipment is appropriate for use and fit for purpose and conforms with the requirements of all applicable regulations; obtain approval from the Serco Sponsor prior to the erection of any scaffold; ensure that scaffold adjacent to track and remaining in place during Revenue Service Hours is subject to Clearance requirements in 1.12.2; arrange for details concerning the use and loading of scaffolds to be made available to the site staff; ensure that any person(s) that alter, erect, dismantle or otherwise interfere with any scaffold on the site are supervised by competent operatives; ensure that training certificates are available on site for inspection; use appropriate signs to indicate that any scaffold being erected, altered or dismantled is not suitable for use and prevent unauthorised access; prepare method statements to include a drawing showing the relationship between the scaffolding and track; provide clear and comprehensive information for operatives. provide specialist certification and calculations where the scaffold is in whole or in part non-standard; ensure that the design of any scaffold: o minimises intrusion and obstruction to railway operations;

o maintains security of SDM property; o considers risks of vehicular impact; o maintains safety and environmental control of works; ensure that a risk assessment is carried out to examine the scaffolding and its location; not use track trolleys to support trestles unless both are secure; insulate equipment from the low voltage that may be generated for signals and maintenance unless the Serco Sponsor directs otherwise; produce documentary evidence of material quality control and inspections to the Serco Sponsor on request; inspect all ladders, scaffolds, trestles and staging at agreed intervals as agreed with the Serco Sponsor; hold records of inspection on site; ensure that all inspected equipment is suitably labelled; label all failed or defective equipment Do not use and remove it from site or place it in a secure area for the purpose of holding defective equipment; label all scaffolding in use as loading platforms to indicate safe working load; ensure that no load extends above protective screens; ensure that only experienced persons carry out work on staging erected in roof areas (lighting battens) and that all necessary safety harnesses and anchorage points are provided and used.

2.12.2 Clearance approvals


No structure (temporary or permanent), plant, equipment or materials may be erected or stored within 3 metres of the running rail without first obtaining formal clearance approval via the Serco Sponsor. The sole exception to this requirement is where the items concerned are only in place or used during Engineering Hours or a possession with adequate protection, which ensures that no trains, or engineering vehicles will run on the track adjacent to those items.

2.12.3 Temporary lighting and power supplies


General requirements The contractor shall: o provide and maintain adequate but not excessive lighting and power supplies for all parts of the works as required, and provide lighting for means of escape if required to manage risks to As Low As Reasonably Practicable (ALARP), and; get approval from the Serco Sponsor of design of lighting or other electrical installations if they want to use SDMs power supply. This is so the Serco Sponsor can check that the lighting will not cause any problems. The use of generators shall be stated in the method statement and approved by the Serco Sponsor;

Voltages and insulation. Where any contractor installs temporary lighting or power supplies: o the electricity supply shall be 110V, centre tapped to earth and not exceeding 55V to earth, except with the written approval of the Serco Sponsor;

the voltage of electric lighting in tunnels shall not exceed 50V except with the written approval of the Serco Sponsor; in confined or damp locations the voltage of hand-held apparatus shall not exceed 25V; whenever practicable, all power tools shall be fed from a 110V centre tapped transformer supply which does not exceed 55V to earth; other equipment requiring higher voltages or other special requirements shall be notified to and agreed by the Serco Sponsor before starting work.

2.13

Materials

All materials to be used on SDM premises must be justified within the method statement and approved by the Serco Sponsor prior to work commencing on site. Contractors shall not deliver materials through public areas of the operational railway during Revenue Service Hours except with the approval of the Serco Sponsor. Contractors shall: take precautions to prevent any spillage from occurring; submit details of proposed precautions to the Serco Sponsor for approval; in case spillage occurs take precautions to contain the spill immediately and prevent it from coming into contact with the railway, public highway, adjacent property, land and waterways, footpaths, persons, trains, and railway equipment; clean up the spillage and remove all wastes from site immediately according to appropriate disposal methods and inform the Serco Sponsor Should any spillage occur the contractor shall immediately report it to the Operations Control Centre (OCC) in accordance with the Accident and Incident Reporting and Investigation Procedure DM001-IRI-ALL-PC-00088

2.13.1 Conveyance of loads


Conveyance of loads on lifts and escalators Contractors shall only convey loads (other than hand-held loads) on escalators and in lifts during Engineering Hours and after the Serco Sponsor has granted prior written authority for their movement The preferred means of transporting loads other than handheld loads is in lifts. All loads transported on escalators must be protected to ensure debris does not damage the escalator.

Conveyance of hazardous materials and substances The contractor shall ensure that all hazardous materials for use on SDM premises are approved by the Serco Sponsor prior to use. Contractors may transport materials and hazardous substances, where approved for use, in the required containment and in a manner addressed and agreed under a method statement for the works accepted by the Serco Sponsor.

2.14

Temporary Works
provide, maintain and remove on completion of the Contract all temporary works needed for the execution of the Contract; submit detailed design drawings, calculations and specifications for all temporary works to the Serco Sponsor for approval; ensure that designs are submitted at such times agreed with the Serco Sponsor; design all temporary structures to carry the loads they are required to support; not erect any part of the temporary works until the Serco Sponsor approves that part; modify the detailed design drawings, calculations and specifications for temporary works if required by the Serco Sponsor; ensure that their designer of temporary works co-operates with the Serco Sponsor, confirming: o any risk assessments have been undertaken; o any residual risk associated with the temporary works that is not reasonably practical to eliminate are controlled; provide and maintain, in a safe and clean condition, surfaces of all temporary foot ways including ramps, landings, stairs and platforms; not leave any temporary works (of any type) with wide gaps, rough edges or slippery surfaces; keep equipment clear of access ways; route electric cables to avoid tripping hazards and the possibility of damage by vehicles, persons, plant or equipment; cover and adequately protect cables or hoses that cross access ways to eliminate tripping hazards; note that approval by the Serco Sponsor of the detailed drawings of temporary structures in no way relieves the contractor of any responsibility under the Contract; ensure the duration of the work is agreed with the Serco Sponsor.

2.14.1 Temporary fences and hoardings


The contractor shall: not remove existing fences or hoardings, except as instructed or approved by the Serco Sponsor, and shall keep them free from damage. provide, alter and adapt all necessary fences and hoardings to enclose the works and storage areas and fix them securely and maintain them in good condition clear away and make good all fences and hoardings on completion ensure that gates are kept closed at all times and locked when not in use or when the site is unoccupied provide a set of clearly labelled duplicate keys to the Serco Sponsor, and a set to the Station Master.

The contractor shall ensure that temporary fences and hoardings: provide adequate levels of fire protection and use materials approved by the Serco Sponsor; exclude and protect the public, SDM staff and all other unauthorised persons from

their activities and from dirt, dust and debris generated by the works; protect the works from the public and SDM staff; protect the works, materials and plant from theft; are erected and maintained to the approval of the Serco Sponsor.

The contractor shall obtain any necessary Dubai Civil Defence planning approvals/ exemptions. The contractor shall agree the hoardings with the Serco Sponsor, station management and the Dubai Civil Defence (as appropriate), so that the fire plans can be amended. Where hoardings restrict exits from the stations an analysis of its effect on evacuation times should be undertaken by the contractor. The contractor shall erect fences and hoardings before any other work starts, unless otherwise approved or instructed by the Serco Sponsor.

If the Supplier removes panels from hoardings on platforms or adjacent to the track during Engineering Hours or under a possession, they must be securely replaced to the Serco Sponsor's satisfaction at least 30 minutes, or other such period as the Serco Sponsor may instruct, before the traction current is due to be switched on. The contractor shall immediately carry out all adjustments that the Serco Sponsor considers necessary, to his or her satisfaction. The contractor shall erect temporary structures to the approval of the Serco Sponsor, and shall take adequate precautions to prevent unauthorised entry to SDM Premises. The contractor shall provide, to the approval of the Serco Sponsor, fences, hoardings and warning lights to all excavations and obstructions. The contractor shall maintain fences and hoardings adjacent to areas accessible to the public, clear of screw heads, nails or other protrusions. When work has ceased at the end of each shift, the contractor shall protect all exposed, uncompleted work with hoardings or other protection panels, as described in this condition. At each site, the contractor shall provide notices on the outside of the hoarding at the main gate stating: the name of the Serco Sponsor, Principal Contractor or main Supplier who may be contacted for permission to enter the site; the name and telephone number of the custodian with keys to the access door.

2.15

Storage

2.15.1 General requirements for storage


No contractor or person shall store materials without the approval of the Serco Sponsor. Contractors shall:

Ensure they do not store any material in a position where it could fall, slip, roll or be blown onto the track, railway equipment, public highway, platforms or walkways; Take all reasonable steps to ensure no damage to the asset is sustained by stored material (e.g. tile flooring); When storing cable drums on platforms contractors shall secure them with a chain and padlock within a hoarded area; Carry out a thorough safety inspection at the end of each shift and after use of the storage area to ensure that stored items are secure;

Comply with all legislation and relevant guidance in respect to chemical substances and materials storage; Store liquids in enclosures or trays to contain any spills or drips; Protect and store materials in such a way as to minimise unnecessary damage, wastage or spoiling of goods; Ensure all containers are properly labelled and Material Safety Data Sheets (MSDSs) are provided, available on site and adhered to as required.

2.15.2 Station storage


The process for station storage approval is detailed at Guidance for granting a station storage license DM001-OPE-ALL-WI-00265 and applied for using the Stations Storage Licence Form DM001-OPE-STA-FO-00130. Both of these documents are available from the Serco Sponsor.

2.15.3 Lineside storage


Contractors requiring to store any materials in or about the lineside should request permission from the Serco Sponsor via the SDM Work Instruction Storing tools, equipment and materials on the lineside DM001-ENM-ALL-WI-00534. Note: there is minimal space in the SDM lineside environment for storage.

2.15.4 Hazardous materials and substances


Contractors shall not store any hazardous materials in stations except where approved by the Serco Sponsor, in the quantities and containers specified as part of that approval.

2.16

Fire Safety
dispose of waste or other materials by burning on any site; take or store anywhere on the railway any cylinders of industrial or flammable gases and containers of flammable or volatile substances without the prior written permission of the Serco Sponsor; store extremely flammable materials (i.e. flashpoint below 0C and Diesel / Petrol) under ground; and shall not store any flammable or highly flammable materials except where approved by the Serco Sponsor. use gas cookers, gas or petrol heaters and space heaters at below ground locations; overload power sockets at any time or use any adapters in connection with electrical equipment and power outlet sockets; relocate existing fire points; obstruct existing fire points, manual call points, hydrants and extinguishers ensuring that they are available for operation at all times; obstruct or obscure exits, signs and means of access, emergency stairs and doors; use, or permit to be used, fire fighting equipment and appliances for any purpose other than fire fighting; use or provide dry powder or halon extinguishers; install temporary huts made from combustible materials in below ground locations; use timber for any works in below ground locations without the prior approval of the Serco Sponsor; use polythene or other combustible materials to wrap, encapsulate, or protect any packages, equipment or materials brought in and stored or installed in below

ground locations except where a concession against the governing standard is in place; store, or allow to accumulate, any flammable or combustible materials on site, except as authorised by the Serco Sponsor; obstruct, interfere with or remove existing fire plans isolate any part of any fire protection system without authorisation from the Sponsor

Contractors shall: ensure that all personnel and sub-contractors are fully aware of SDMs requirements in respect of fire prevention and protection; take strict precautions to protect the site, any adjacent property and all persons from fire; minimise the quantities of approved flammable materials temporarily stored under storage licence, which shall at all times be the minimum consistent with safety and construction requirements and never greater than the maximum allowed for that material under the licence; remove immediately all empty drums, empty containers, surplus or waste material and used packing materials from operational premises and below ground locations at the end of each working shift; minimise all fire risks, including mopping up spillages or absorbing them in sand or other suitable material which shall then be disposed of by the Supplier; carry out a risk assessment of the site and arrange adequate fire safety training; heat water using electric immersion-heaters; use electric convection heaters for space-heating; ensure that existing facilities at the site remain accessible; request, via the Serco Sponsor, the isolation of fire detection equipment when any works may have an adverse impact on the equipment, e.g. smoke or dust. ensure that de-isolations of fire protection systems are performed at the end of each period of Engineering Hours unless current exemptions are in place and implement a system of hourly checks of the entire site in the event of such isolation being approved; provide suitable and sufficient fire fighting equipment throughout the site when the output of a risk assessment indicates an increase in the risk of fire at any work site or when required to do so by the Serco Sponsor; ensure that all extinguishers comply with British or NFPA Standards; ensure that periodic inspections of the Supplier's fire fighting equipment are undertaken by competent personnel; where appropriate nominate personnel to act as Fire Watchman; provide the Serco Sponsor with full information on moves and consequent changes, however minor, which may affect fire plans; prepare emergency plans that reflect SDM local arrangements; satisfy the requirements of the Serco Sponsor and the Dubai Civil Defence and provide their representatives with the facilities to inspect the fire prevention arrangements on the site; at the contractors cost, make such alterations and additions to the fire protection arrangements as the Serco Sponsor may require; make information available on request to the Serco Sponsor and any other authority that requires it;

make adequate provision for raising the alarm in case of fire.

2.16.1 Temporary fire points


If a temporary fire point is required to meet the conditions above, as a minimum it shall consist of: two 9 litre Aqueous Film Forming Foam (AFFF) fire extinguishers, clearly labelled 'Not For Use on Electrical Fires'; one 2 kg carbon dioxide (CO2) type fire extinguisher; fire blanket.

The fire point shall be constructed in a manner and from materials, which satisfy the Serco Sponsor and be easily recognisable.

2.16.2 Use of gas cylinders in below ground locations


Where cylinders are required to be used for cutting or welding purposes the contractor shall ensure that a Hot Work Permit has been issued for the work site and that all requisite controls are in place. Hoses connected to gas cylinders shall be of the braided or armoured type to prevent damage and risk of gas leakage All industrial gases may only be taken into below ground locations in the quantities permitted and shall be returned to the surface at the end of the work shift and stored as described below Storage of gas cylinders (above ground). Where shift working is continuous, the gases may be left below ground monitored by a Fire Watchman at all times, provided that: the new shift starts no more than one hour after the end of the previous shift which used the gases; hoses and cylinder keys are removed from the gas cylinders (and kept close to hand); valves are fully closed; all cylinders are properly supported and secured in the vertical position; hoses are coiled and placed to ensure that they are protected from damage; the Serco Sponsor is advised and approval obtained.

A sign at the temporary assembly point shall announce that gas is being used, the type of gas, and that the same notice is given to the Station Master. Contractors shall remove all gas cylinders when the public has access to the station. Additionally, contractors shall not take gas cylinders into the station in Revenue Service Hours.

2.16.3 Storage of gas cylinders (above ground)


The contractor shall: only store gas cylinders at ground level in locations approved by the Serco Sponsor and in accordance with clause 2.15 Storage; not store them inside stations during Revenue Service Hours not store them in a position that will cause an obstruction to passageways, passenger areas, ticket offices or staff accommodation, or be near any source of ignition;

ensure that cylinders are stored in locked cages, kept vertical and properly fixed and supported; ensure that all hoses and cylinder keys are removed from cylinders and kept close to hand; use cylinder trolleys for all movements of gas cylinders and all cylinders shall be secured to the cylinder trolley by either a suitable chain or strap; provide signage at the entrance of premises when gas cylinders are being stored.

2.16.4 Use of flammable and highly flammable materials below ground


Where flammable or highly flammable materials are required to be used below ground the contractor shall ensure that approval is sought from the Serco Sponsor. Contractors shall provide adequate ventilation to the satisfaction of the Serco Sponsor if the contractors use of flammable substances and volatile materials creates any risk of heavy vapours in confined spaces and take other environmental issues into consideration.

2.16.5 Storage of flammable and highly flammable materials below ground


Flammable and highly flammable materials may be stored on SDM Premises with the Serco Sponsors permission, in limited quantities, in suitable sealed containers (as appropriate) or purpose built stores, or in portable storage bins in areas covered by automatic sprinklers. Where the contractor has to use such materials, and has the Serco Sponsors approval to use them, it shall only use the minimum quantities needed to carry out the works satisfactorily.

2.16.6 Fire emergency plans


Contractors shall produce an emergency plan that: clearly states the procedures to be adopted for each emergency; details temporary assembly points and escape routes; lists the duties and responsibilities of personnel on site; identifies a senior site official with responsibility for liaison with the emergency services; includes the names and telephone numbers of the contractor's staff (including mobile telephones if applicable) who can organise or assist with emergency action in the event of an incident occurring on the site outside the contractor's normal working hours.

The contractor shall distribute and display the copies at each place of work together with any other documents, posters or notices required by law or directed by the Serco Sponsor.

2.16.7 Hot work


No work shall be carried out unless under a Hot Work Permit in accordance with SDM Work Instruction DM001-ENM-ALL-WI-00823 - Hot Work Safety Permit . See Section 2.4.4

2.16.8 Fire Watchman


When the contractor carries out hot works the contractor shall provide a competent Fire Watchman for the duration of the work.

All Fire Watchpersons shall be equipped with a minimum of: two 9 litre Aqueous Film Forming Foam (AFFF) fire extinguisher; one 2 kg carbon dioxide (CO2) fire extinguisher; one fire blanket; an effective means of communication.

All such items shall be: tested and serviced at specified intervals as agreed with the Serco Sponsor. provided at each site or location where hot work is being carried out.

Fire Watchman shall: not be engaged in other duties; be clearly visible and identifiable as such by wearing suitable garments to aid recognition (such as armbands or jackets); ensure that their attendance is recorded by the Person in Charge (PIC) on the Hot Works Safety Permit; ensure that all adjacent installations that may be damaged or increase the risk of fire spread, are protected or screened from the effect of the hot works by fire resistant covers or screens; immediately raise the alarm, should the need arise; remain on site for at least 30 minutes after the hot work has been completed; ensure that no fire hazard remains at the end of the work; make sure that all waste or litter is removed; make sure that hot steel ends of welding rods and waste materials are disposed of safely; damp down the surrounding area if necessary before leaving the work site; check the adjacent areas to ensure that no conducted heat has been transmitted.

When at stations the Fire Watchman shall: have their attendance (arrival and departure) recorded in the station log book; ensure that this information is relayed to adjacent stations in the case of hot working undertaken in sub-surface tunnels; carry a hand-held radio; ensure that automatic sprinklers and detection systems remain operative unless it is determined that the proposed work will affect the sensitive elements of the sprinkler or detection system. In which case, ensure isolations are in place where required for the work and that de-isolations are performed at the end of each shift; inspect the other side of any wall or partition on which work is being carried out to make sure that materials there are not in danger of being ignited by direct or conducted heat; ensure that any flammable materials are moved at least 15 metres away from the worksite, or are protected (depending on the practicability and the nature of the fire risk); ensure that, where hot work is to be carried out on enclosed equipment such as containers or ducts, the equipment is first cleaned to reduce the risk of any dust, combustible material or flammable vapors igniting during the work;

ensure that those persons in the vicinity of the hot work are: o o familiar with the use of the fire fighting equipment provided; familiar with the means of escape; aware of other persons working in the vicinity.

4.1

Contractor Environmental Conditions


General environmental requirements
exercise a duty of care and be liable for all claims, costs, expenses and disbursements (including remediation of the land to the state it was in prior to any contamination) incurred directly or indirectly by the any party as a result of any breaches of the applicable environmental laws by the contractor; ensure that parties may rely upon any reports, recommendations or assessments in relation to environmental matters, where the same are conducted by or on behalf of the contractor; use best practical means and good industry practice to avoid contamination or pollution to the environment; notify the OCC if contamination or pollution occurs in accordance with Section 6.

4.2

General organisational requirements

Contractors shall appoint member(s) of their management team who have defined authority that includes: ensuring awareness of environmental requirements throughout the contractor's organisation; liaison with the Serco Sponsor on the performance of the contractor's environmental management; documenting all environmental corrective actions; ensuring that contractors and sub-contractors staff work to correct and environmentally sound practices in line with mandatory and Serco Sponsor requirements.

4.3

Environmental nuisance
undertake the necessary investigations to establish whether or not the works may result in or have the potential to cause environmental nuisance(s); undertake investigations and evaluations at such a stage in the project that the implications of any findings can be incorporated into the design, programme of works and method statements as necessary; comply with all relevant legislation, regulations and guidance, and ensure that best practical means and good industry practice are followed to control nuisances to the SDM, its customers and neighbours, derived from noise, smells, fumes, dust, smoke, light, vibration or air pollution associated with the works; submit and adhere to method statements which outline and detail the measures to be employed to ensure compliance with any requirements of the local authority, other regulatory authorities, SDM requirements, and which demonstrate adequate protection for habitats, neighbouring properties and residents in respect to generating, controlling and monitoring nuisances; inform neighbours of potential nuisances prior to commencing specific projects or major project works in liaison with the Serco Sponsor, and provide evidence that neighbours have been properly informed;

establish a suitable system for dealing with customer/ neighbours queries and complaints that complies with the Serco Sponsors requirements.

4.4

Water
undertake the necessary investigations and assessments to establish whether water will be discharged as a result of the works. This shall include the quantity and quality of any water discharge which will be used to: o inform the requirement to monitor water discharges; o apply for relevant permits and licences; o meet municipality requirements, and; o exercise good industry practice in the management of such discharge in line with current legislation and regulations; assess whether the site has the potential to suffer from water ingress. This shall include as a result of weather conditions etc and ensure that mitigation measures are in place based upon the risk of water ingress. This shall be included in the emergency planning process; identify where waterways, drains, springs or water supplies may be intercepted, interfered with or cut through by the works, and outline in method statements suitable mitigation measures and actions to ensure functionality of these watercourses is not impeded. This shall include any requirements to notify, or receive permission from regulatory authorities or other bodies; submit and adhere to method statements which outline and demonstrate how water discharge, water ingress and retention of the functionality of watercourses as identified above shall be managed and mitigation measures employed so as to protect these water courses, the works and adjacent properties; comply with all relevant legislation, regulations, and guidance, and follow good industry practice; not make temporary or permanent connections to any water supply mains or wastewater networks without prior permission from the relevant authority; be responsible for any necessary actions for the training, diverting, or conduiting of open streams or drains intercepted by the works; provide silt tanks for water contaminated with suspended solids; take all necessary precautions to prevent water entering or being discharged from the site from entering upon the works of adjacent contracts or adjacent properties; provide, where necessary, temporary water courses, floodwalls, ditches, drains, pump sumps, pumping or other means that may be necessary to maintain the works free of water and record proposals in method statements; ensure leaks are fixed promptly to minimise water wastage. notify the Serco Sponsor immediately if any accidental discharge occurs in accordance with Section 6.

4.5

Waste management
Submit and adhere to method statements which outline proposals for waste management. This shall include identification of waste types, proposals to maximise reuse and recycling of materials, and methods for management of any special and hazardous wastes;

Comply with all relevant legislation, regulations and guidance for the storage, transport and final disposal of waste in accordance with best practice; Make available upon request information regarding the tonnages of materials sent for disposal, recycling or reuse; Be responsible for all waste materials generated by the works, as agreed by the Serco Sponsor; Remove all waste from the site (except where otherwise specified in the Contract) at the contractors cost.

4.6

Noise and vibration


undertake the necessary investigation to establish whether or not the construction works or the project deliverables have the potential to produce significant noise impacts in relation to the sensitivity of surrounding and working areas. This investigation must include: o liaison with the Serco Sponsor; o an identification of the activities likely to produce levels of noise and vibration that exceed those set in current legislation; o an outline of any requirements to monitor noise and vibration levels during construction works required in respect to minimising damage to buildings and minimising complaints; use a competent person to assess the work that is to be carried out to determine whether or not an application for a No Objection Certificate (NOC) or local noise permits is required; undertake investigations and evaluations at such a stage in the project that the implications of any findings can be incorporated into the design, program of works and method statements as necessary; comply with all relevant legislation, regulations and good industry practice and use best practical means in respect to the control, management and reduction of noise and vibration arising from any site works and any assets which form the deliverable parts of the Contract; submit and adhere to method statements which outline and demonstrate how noise and vibration shall be managed and how any mitigation measures shall be employed. Method statements shall outline actions proposed to reduce noise and vibration levels and to minimise disturbance; details of any proposed noise and vibration monitoring programmes; details of plant type, location and noise control methods; and duration of operating hours for individual activities; not commence work until any applications for local noise approvals have been granted; notify the Serco Sponsor of any changes, dispensations or variations being sought in respect to local noise approvals; notify the Serco Sponsor immediately of any changes to arrangements set out in method statements relating to plant or hours of work; make available upon request results of any noise and vibration monitoring undertaken.

4.7

Archaeology and buildings of historical interest


undertake the necessary investigations to establish whether or not the works have the potential to damage or affect areas of archaeological or historical interest. This shall include interior features and any effects in respect to the character of such premises and must include liaison with the Serco Sponsor; undertake investigations and evaluations at such a stage in the project that the implications of any findings can be incorporated into the design, program of works and method statements as necessary; comply with all relevant legislation and regulations and follow good industry practice, for the protection, management, mitigation, control and limiting of any potential impacts or disturbance to those areas of archaeological/ historical interest, heritage site, and/ or species or habitat that are protected by specific legislation; submit and adhere to method statements which outline the measures to be employed for the protection of areas of archaeological and historical interest; suspend all activities on the discovery of archaeological remains until the Serco Sponsor has been contacted and suitable measures have been taken to prevent damage to the remains; unless the works are of a safety critical nature.

4.8

Ecology and geology


undertake the necessary investigations to establish whether or not the works have the potential to affect areas of ecological or geological interest. This must include liaison with the Serco Sponsor; undertake investigations and evaluations at such a stage in the project that the implications of any findings can be incorporated into the design, program of works and method statements as necessary; comply with all relevant legislation, regulations, and follow good industry practices to manage or mitigate any potential impacts on species or habitats; submit and adhere to method statements. These shall outline: o the measures to be employed for the protection of areas of ecological or geological interest, and; o the proposed methods of working to control and limit, as far as reasonably practical, any disturbance to areas of nature conservation interest and areas of geological interest; o organise and programme construction activities to ensure that the impacts on any protected species or habitat are minimised in line with good industry practice.

Pesticide control
submit and adhere to method statements which detail the pesticide type(s) to be used, outline the proposed quantities and detail the application methods. The method statements shall include the outcome of suitable Material Safety assessments; only use pesticides approved by Dubai Municipality;

5.1

Contractor Quality Conditions


Records

The contractor shall maintain records that are specified by the Serco Sponsor, and this shall include as a minimum: details of any non-compliance against any SDM requirements; records of audits and site inspections; records of the qualifications, competence and training of staff; quality assurance inspections conducted; equipment test calibration and verification checks conducted; process and manufacturing data relating to the Contract, including an audit trail for material and/ or component identity; any process, inspection or test activity so directed by special instructions or any Contract Quality Plan invoked by the Contract; non-conforming service or product records; records of all related incoming and outgoing certificates of conformity and associated release documentation; records of tender and contract reviews; the contractors policy with regard to quality; any other quality management systems requirements information.

5.2

Retention period

Records shall be retained by the contractor for a minimum of seven years unless otherwise specified by the Serco Sponsors at contract award.

5.3

Availability of records for inspection

The contractor shall make all such records available to SDM and the Serco Sponsors within three working days of any such request.

5.4

Statistical process control, audit and inspection procedures

Where for quality management purposes statistical process control procedures, audit or inspection procedures are adopted, full details of the proposed procedures used on the Contract are to be submitted to the Serco Sponsors for approval prior to implementation.

5.5

General quality requirements

intended destination that the identification, packaging, storage, preservation and handling do not affect conformity with product or service requirements; not proceed past hold points until all the specified activities have been satisfactorily completed and the related documentation is available and authorised; following receipt of a rejection, take immediate action to inspect all stocks and work in order to assess risk and loss and advise the Serco Sponsor of the findings; take preventative action to avoid a recurrence of the non-conformances; immediately inform the Serco Sponsor when the contractor has reason to suspect non-conformances with previously supplied products or services; be responsible for ascertaining the cause of and responsibility for nonconformances, and for taking suitable corrective action to prevent their recurrence; document all quality related corrective actions; ensure that contractors and sub-contractors work to correct practices including accepted method statements defining the techniques to be used, workmanship criteria and licensing requirements, etc; ensure that the works comply with any manufacturer's recommendations, instructions and guidelines, unless otherwise directed by the Serco Sponsor; operate and maintain all plant, equipment and processes in accordance with the relevant manufacturer's or contractor's specification or procedure, unless otherwise directed by the Serco Sponsors; take all necessary steps to identify, recall, re-inspect and replace any equipment or parts which have been inspected using inspection, measuring or test equipment whose calibration is found to be defective or unknown; store and protect inspection, measuring and test equipment and materials to prevent misuse, damage and deterioration; ensure that all equipment bears an identification of its calibration or maintenance status, in a manner that clearly indicates it is within the calibration or maintenance period.

5.6

Quality Plan
complete and submit a Quality Plan with the tender if required by the Serco Sponsor for review and acceptance; ensure that the Quality Plan demonstrates the contractor's process control system as required for the Contract; maintain and periodically revise the Quality Plan and forward it in its amended form to the Serco Sponsor.

The contractor shall:

5.7

Testing and inspection


demonstrate to the Serco Sponsor's satisfaction that acceptance sampling techniques are utilised; identify which sampling plan it intends to apply to the Contract and forward it to the Serco Sponsor for approval; reference the sampling once it has been approved in all relevant Contract Quality Plans submitted to the Serco Sponsor;

When required by the Serco Sponsor contractors shall:

be prepared to revert to 100% inspection in such cases where the failure rate exceeds the level of acceptance identified within the sampling plan; prepare for the approval of the Serco Sponsor an inspection and test plan which shall include such hold points agreed with the Serco Sponsor; maintain sampling inspection records in accordance with the Serco Sponsors requirements. Where statistical process control procedures, audit or inspection procedures are adopted, provide full details of the proposed procedures to the Serco Sponsors for approval prior to implementation.

5.8

Certification of conformity

When required by the Serco Sponsor, the contractor shall:

provide (for the Serco Sponsor) certificates of conformity for services and products that include: o Contractor's name and location address; o Contractor's full company name and manufacturing address, if different to above; o employers supplier rating system number; o unique certificate reference number and date of certificate; o details of valid third party approvals, applicable to the services or products provided and the establishment providing the services or products to the Serco Sponsor; o the Serco Sponsor's contract reference and, if applicable, quality plan number; o full description and quantity of supplies, including specification, drawing number and issue numbers and relevant standard applicable; o identification marks and serial numbers as appropriate; o details of authorised non conformities, Serco Sponsor concession or production permit references and a full statement of authorised deviations, operations or processes not conducted; o for all materials (raw or finished): cast and/or batch number(s); test report reference and, if called for, copies of test results and additionally for metallic materials; the condition of the material despatched; recommended heat treatment if the material is being delivered not in a final use condition; chemical analysis and / or mechanical testing certificates; inspection stamp and/or authorising signature; details of packaging and transportation where appropriate; ensure that all certificates include one of the following statements of conformity signed by an authorised signatory, on behalf of the contractor: o for supplies from a manufacturer, the format shall be Certified that the whole of the supplies detailed hereon have been inspected and tested, and (unless otherwise stated) conform in all respects with the requirements of the Contract; o for supplies from an agent, stockist or distributor (i.e. where the contractor is not the manufacturer) the format shall be Certified that the whole of the supplies detailed hereon have been inspected and tested and (unless otherwise stated) conform in all respects with the requirements of the

Contract; for services the format shall be Certified that the services detailed hereon have been inspected and tested, and (unless otherwise stated) conform in all respects with the requirements of the Contract; provide a copy of the contractor's authorised signatory list, showing as a minimum, name, job title, signature, designated authority level at the commencement of the Contract; ensure that any certification from sub-contractors for parts or services related to the Contract accompanies the Supplier's certificate of conformity; agree with the Serco Sponsor a certificate of conformance for use where projects or part projects are handed over; include in the certificate of conformance a 'Statement of design performance where design work is provided; provide statutory test certificates where applicable. o

5.9

Quarantine

Contractors shall provide secure quarantine storage for the storage of materials and products that are the subject of investigation regarding their conformance, or non- conformance.

5.10

Traceability

The contractor shall ensure that all the materials to be incorporated into works delivered to site are controlled at installation and are able to be traced to manufacturer, unless indicated otherwise by the Serco Sponsor.

5.11

Maintenance and servicing

Contractors that maintain or service SDM assets shall provide the Serco Sponsor with written details of how their arrangements for maintenance and servicing will ensure the reliability, maintainability, durability and serviceability of the asset.

5.12

Design
operate a change control system so that the appropriate issue of drawings, technical specifications, training material and customer supplied standards can be readily determined at all times; ensure that all design changes and modifications are be identified, documented, reviewed and approved by authorised personnel before their implementation; record the results of all design assessments; appoint or nominate a design authority for each design; ensure that designers hold BS-EN-ISO-9001 certification relevant to the scope of work or, if the design contractor is not certificated to BS-EN-ISO 9001, a documented management system which includes design shall be required, which would satisfy the requirements of the standard.

Computer aided design

Contractors shall provide Computer Aided Design (CAD) drawings in a style, format and software specified by the Serco Sponsor.

Asset Configuration Control Management

Contractors shall comply with the requirement of SDM Procedure DM001-ENM-ALL-PC-00027 Asset Configuration Control Management for all configuration change proposals that may affect the safe operation of the Dubai Metro. Details of this procedure are available from the Serco Sponsor

5.15

Asset commissioning and handover

Incident Management

SDM defines an accident as an unplanned, uncontrolled event which has led to asset/property damage, ill-health or injury or damage to the environment. SDM defines an incident as an unplanned, uncontrolled event which could have led to asset / property / environment damage, ill-health, injury or other loss The contractor must ensure that the requirement to notify, record, report and investigate incidents in accordance with Accident and Incident Reporting and Investigation Procedure DM001-IRI-ALLPC-00088 is communicated to all contractor staff and sub-contractors.

6.1

Accidental damage, obstruction or interference with assets

Should any damage occur to an asset the contractor shall immediately report the damage to the Operations Control Centre (OCC) in accordance with the Accident and Incident Reporting and Investigation Procedure DM001-IRI-ALL-PC-00088 The contractor is not permitted to rectify the damage for works out of scope as this may increase the risk of a consequential incident.

7.1.1

Monitoring Audit and Review


Work location inspection and monitoring

Contractors shall provide full and free access to the Serco Sponsor (and/ or their appointed agent) for the purpose of carrying out site inspections and monitoring to monitor compliance with health, safety, quality and environmental conditions. The Serco Sponsor (and/ or their appointed agent) may at any time undertake inspection of the equipment and monitor or check any aspect of the contractor's performance of the Contract. The contractor will promptly provide all reasonable co-operations in relation to any inspection or monitoring including:

granting access to any premises, equipment, plant, machinery or systems used in the contractor's performance of the Contract, or where such premises, equipment, plant, machinery or systems are not the contractor's own using, reasonable endeavours to procure such access; ensuring that appropriate security systems are in place to prevent unauthorised access to, extraction of and alteration to data during the audit; making any contracts, other documents and records referred to in the Contract available for inspection; providing a reasonable number of copies of any contracts, other documents and records referred to in the Contract, required by the auditor, or granting copying facilities to the auditor for the purposes of making such copies. making available, at the contractor's cost, any of their specialist staff as may be agreed necessary for the performance of inspections and monitoring. Working closely and co-operating fully with the Serco Sponsor and SDM staff.

The contractor shall fully and unreservedly accept that responsibility for the safety of the works rests with the contractor. The participation of the Serco Sponsor (and/ or their appointed agent) in inspections and monitoring does not in any way absolve the contractor from that responsibility.

7.1.2

Health, safety and environmental audits, inspection and tours by the contractor's safety officers

The contractor shall submit to the Serco Sponsor details of their proposed level of site health, safety and environmental surveillance together with the nomination of health, safety and environmental supervisors and officers who will have an overview of all site health, safety and environmental matters. Such details shall include outline programmes for: safety tours and detailed safety inspections; the monitoring of site activities to ensure that the agreed health, safety and environmental management procedures are followed.

7.1.3

Auditing

The Serco Sponsor (and/ or their appointed agent) shall have the right to carry out audits, as they consider necessary at SDM premises or at the contractors premises. The Serco Sponsor (and/ or their appointed agent) shall inform the contractor of the objective of any such audit before it is undertaken.

The contractor shall make available, at the contractor's cost, any of their specialist staff as may be agreed necessary for the performance of such an audit carried out by the Serco Sponsor (and/ or their appointed agent). The contractor shall work closely and co-operate fully with the Serco Sponsor and SDM staff. The contractor shall fully and unreservedly accept that responsibility for the safety of the works rests with the contractor. The participation of the Serco Sponsor (and/ or their appointed agent) in auditing does not in any way absolve the contractor from that responsibility.

7.1.4

Monitoring sub-contractors

The contractor is responsible for all aspects of its sub-contractors performance when on SDM premises. The contractor shall monitor the performance of its sub-contractors and ensure that the results are used to:

control their activities; assess the sub-contractors suitability for future works.

These results (records) shall be made available to the Serco Sponsor upon request.

7.1.5

Timescales for rectifying non-compliances

The Serco Sponsor (and/ or their appointed agent) and contractor shall agree the timescale for rectifying any non-conformances or sub-standard conditions that are identified during any audit or inspection.
