Request for Proposals Spring 2012


The Service-Learning Student Project Fund (SLSPF) is designed for students enrolled in designated service-learning classes. These students may request funding to offset costs directly related to their service-learning projects. Service-learning instructors must approve the project and sign off on the proposal cover page. Students may request up to $500, and must submit a detailed final expense report after the project is completed. We encourage students to be creative and expansive in planning a service-learning project. Work closely with your instructor and the Thayne Center for Service & Learning to explore the wide range of possibilities. Think big! Examples of items and services that qualify for this program (not a comprehensive list): Printing of materials created by you (brochures, fliers, cards, handbooks, etc.)* * All printing is done in the Taylorsville Redwood Copy Center and coordinated through the Thayne Center for Service & Learning. Logistics are negotiated on a case-by-case basis. Arts and crafts items Website domain name Amateur film production and editing Materials and other costs related to hosting community events

Note: The SLSPF is not monetary compensation for your service hours and cannot be used as monetary donations to nonprofit organizations. The fund is intended to directly support the cost of projects, events, and products created and implemented by SLCC students. Note: If your service-learning instructor requires a project proposal for his/her class with requirements similar to this RFP, consult with Lisa Walz prior to submitting an application to best determine how you can dovetail the assignments and avoid duplication. Submission Procedures Proposal Outline Proposals for the Service-Learning Student Project Fund must include the following: 1. Cover Page, including instructors signature (see final page of this document). 2. Proposal Narrative, 1-2 typed pages must include the following information: a. Detailed description of the proposed service-learning project

Page 2 of 6 Who Describe your non-profit community partner organization. What is their mission? Briefly explain what they do and who they serve. What Outline the details of your project. Where If your project involves planning a specific event, where will the event take place? If not an event, where will you distribute your product? When Include the day/time of your event, the time frame in which you will complete your project, or by when you will distribute your product. Why What community-identified need does your project address? How did the community identify this need and how did you become aware of it? How What is your action plan strategy for designing and carrying out this project? Be specific. b. Anticipated end result of your work What do you hope to accomplish? How will you know that your project was successful? c. How does your project relate to the learning objectives of your service-learning class? Refer to your syllabus or ask your instructor for guidance. 3. Budget, provide a detailed, annotated budget describing all projected expenses. In your annotations, include the following: a. 1-2 overview paragraphs explaining how you plan to spend the money b. Individual items or services c. Retailer or store where those items or services will be purchased d. Cost of each item or service e. Quantity of item or service f. Total cost for all items and services Tips: All printing will be done in the Taylorsville Redwood Copy Center and coordinated through the Thayne Center for Service & Learning. Logistics are negotiated on a case-by-case basis. Expenses omitted from the budget proposal will not be reimbursed; however, if your project continues to evolve and requires additional funding, you may re-apply for funds multiple times. Try to be as comprehensive as possible the first time. Because SLCC is a state government organization, we are exempt from paying state sales tax. Please contact Lisa Walz (lisa.walz@slcc.edu or 801-957-4688) for SLCCs Tax Exempt Number. If you would like to order food for an on-campus event, please meet with Lisa Walz BEFORE submitting your proposal. You must get a catering quote from SLCC Food Services and include this quote in your budget outline. You may use Microsoft Word, Excel, or another similar spreadsheet program. See Sample Budget Design on page 3.

Proposal Submission Submit the proposal both electronically (.doc, .docx, or Adobe PDF format) and deliver a hard copy delivered to the Thayne Center for Service & Learning. o Clearly indicate the following in the electronic document name: The last name of the primary applicant Department prefix Service-learning course number Section number Instructors name Example: Garcia_ENGL 1010_001_Jones. o The subject line of the submission email must clearly state: Student Service-Learning Project Fund plus the last name of the applicant. Email submissions to lisa.walz@slcc.edu. Deliver a hard copy of the proposal and cover page, including original signatures to the Thayne Center on Taylorsville Redwood Campus, Student Center 020C. Please note: If your proposal is not accepted as is, you will receive detailed feedback and guidance. It is our goal to fund all viable projects with thoroughly outlined proposals. We highly encourage you to review our notes, meet with your instructor, and resubmit before the final deadline. Please direct any additional questions to Lisa M. Disbursement of Funds Receiving Your Fund Disbursement There are two options for disbursement of funds: 1. Reimbursement You are responsible for purchasing all items and services using your own money. Once you have completed purchases for the project, submit itemized receipts to the

Page 4 of 6 Thayne Center. At that point, the Thayne Center begins the process of requesting a reimbursement check be sent to you through the SLCC Business Office. You may have the check mailed to your home, or it can be picked up in person at the Thayne Center. Please inform the Thayne Center office manager which of these two choices you prefer. 2. Low Cash Dollar (LCD) Check You will receive an SLCC check from the Thayne Center. SLCC formats LCD checks in triplicate, which includes an original copy and two carbon copies. When you pick up the check, you will sign a form with your name, S#, drivers license number, address, and phone number. You will then have 24-48 hours to use the check at a retailer and return the carbon copies with original itemized receipts. If the carbon copies and additional itemized receipts are not returned within 48 hours, a hold will be placed on your SLCC records. This means you will not have access to transcripts, course registration, financial aid, and other important SLCC services. Final Report When purchases are complete and all required paperwork returned to the Thayne Center, you will then turn in a Final Report. If the Final Report is not turned in by the week after final exams, a hold will be placed on your SLCC records. This means that you will not have access to transcripts, course registration, financial aid, and other important SLCC services. Your final report provides details about the results of the project. Include the following: a. Describe the results of the project. b. What went well? What didnt go well? What could have been done better? c. Finalized budget with exact items listed on the receipts (use Sample Budget Design). d. Original itemized receipts, if they have not already been turned in to Thayne Center office manager with your LCD check carbon copies. Submit the Final Report electronically to Lisa Walz at lisa.walz@slcc.edu. Also, please deliver a hard copy to Lesa Bird, Thayne Center office manager. Final Reports are due no later than May 11, 2012. When you come into the office to submit a hard copy of your Final Report to the Thayne Center, you will be asked to take a short online survey. Plan to spend about 15 minutes dropping off your hard copy Final Report and completing the survey using one of the Thayne Center student computers. Service-Learning Showcase All students awarded funding are strongly encouraged to present their work at the Spring Semester Student Service-Learning Showcase on Friday, April 27, 2012, 1:30-4:00pm at the Taylorsville Redwood Campus in Parlors A & B. The Showcase is a prestigious opportunity for you to tell the story of your project and what you accomplished by the end of the semester. SLCC administrators, staff, faculty, other students, and your family and friends are invited to attend this event. Lite refreshments will be served.
