INSTRUCTIONS AND APPLICATION FORM FOR A LETTER OF ASSESSMENT (SECONDARY, COLLEGE, UNIVERSITY EDUCATION) BEFORE SUBMITTING YOUR DOCUMENTS,

PLEASE READ ALL OF THE FOLLOWING INFORMATION CAREFULLY. Incomplete applications will be returned! The Comparative Education Service provides assessments for general and employment purposes, and not for university admission. The letter may be used for admission to various colleges and institutes at the discretion of the institution to which you are seeking admission. The assessment will not be used for admission to the University of Toronto. Submit the completed application form and all required documents to: Comparative Education Service, School of Continuing Studies, University of Toronto, 158 St. George Street, Toronto, Ontario, Canada, M5S 2V8. Note: We reserve the right to review original documents, to obtain verification from institutions attended, and to request additional documents when necessary. Please provide documents for each program of study you attended (for example, if you have a Ph.D. degree, copies of all the degrees/ diplomas and official transcripts earned prior to this program are required). The same service fee covers the assessment of all your qualifications. If you do not require your secondary education assessed, please do not submit this document unless requested. You may not select to exclude certain qualifications from the assessment procedure. All documents submitted will be reviewed. If you have previously obtained a Canadian degree or diploma prior to receiving a foreign degree or diploma, you are required to include copies of the Canadian degrees or diplomas and transcripts as part of your application package. These documents are necessary for the overall review of your education. INCLUDE two photocopies of each of the following documents: 1. All degree(s)/diploma(s) in the original language and official English translation(s). 2. All official transcript(s)/indeks or statement(s) of marks in the original language and official English translation(s). 3. One piece of official photo identification showing date of birth. Note: if your name has changed (i.e. the name on the application form is different from the name shown on the degree/diploma), provide your marriage certificate or proof of name change document showing both your original name and current name. Rush Service: $508.50 ($450.00 + 13% GST) (Within five working days upon receipt of all required documents.) Estimated processing time may increase during particularly busy periods.

NOTES: Submitting documents by mail: Do NOT send original documents by mail. Two sets of photocopies are required. Please ensure that the entire page is copied and that all the text is readable. Submitting documents in person: Bring original documents and two sets of photocopies. Originals will be returned immediately. Do NOT send application packages by fax or electronically. Additional copies of your assessment letters can be ordered. You may also request copies to be sent to a Third Party. The cost is $33.90 (30.00 + 13% HST) per set. Please use the appropriate form. Payments should be made in Canadian dollars, in the form of a certified cheque, bank draft, or money order made payable to the University of Toronto. Payment can also be made by credit card. We do not accept cash or debit payments. Cancellation: An application submitted for regular service may be cancelled if the request is received within five business days. An administrative fee of $50.00 will be applied. An application submitted for rush service cannot be cancelled. Verification of documents may be necessary. If your former institution charges a fee for this service you will be responsible for the extra costs. This can also increase the time required for processing.

Requests for assessment by third parties will not be processed without written approval of the person whose name appears on the documents. I hereby request an assessment of the enclosed documents, which I certify are accurate and genuine academic credentials earned by me. I understand that all fees are nonrefundable, and all documents submitted are nonreturnable. I further understand that the University of Toronto may contact the institution(s) that issued the document(s) for verification or information. I will cover any extra costs for this service if required by my former institution. Furthermore, I understand that measures will be taken in the event that my documents are determined to be false. These measures include the nullification of any assessment letters already issued and the notification of other credential agencies.
