DISCHARGE PLAN

MEDICATIONS TO TAKE AT HOME


Taught client to continue antibiotic therapy and giving importance to its route, dosage, frequency and its duration. If antibiotics : Azithromycin 50mg/tab Once a day If mucolytics : Ambroxol 75mg/cap Once a day If antipyretics : Paracetamol 500mg/tab every 4 hours

EXERCISES
Encouraged to take a regular exercise like walking and take a rest for at least one and a half hour each time exercising is performed. Advise to avoid strenuous or vigorous activities (e.g. running, lifting heavy objects, etc.) to prevent shortness of breath or difficulty of breathing

TREATMENT

Advised the patient to continue the prescribed medications, specially antibiotics, otherwise pneumonia may recur.

HEALTH TEACHINGS

Instructed the significant other to position the client in mid high back rest if difficulty of breathing occurs.

Instructed the guardians that if hyperthermia occurs, the patient must have a tepid sponge bath.

Encouraged the patient to drink 5-8 glasses of water per day to keep hydrated and to loosen and liquefy secretions.

Advised to place curtains in the patients room if photophobia is still present.

OUT PATIENT FOLLOW UP


Instructed to return 2 weeks after discharge, especially if signs and symptoms of pneumonia reoccur such as shortness of breath, difficulty of breathing and excessive secretions.

DIET

Encouraged the client to eat foods rich in protein ( e.g. meats, dairy, and legumes), iron (e.g. liver and clams) and green leafy vegetables, as tolerated by the patient. The significant other was also instructed about strict aspiration precaution.

SPIRITUAL

Ask to continue her worship service and religious beliefs and pray to God to provide her strength and for fast recovery.
