INTRODUCTION Mental health or wellness affects every aspect of ones life, and the impact of this can be seen when mental illness occurs. The effects are not only mental but also physical. According to Travelbee (2002), nursing is an interpersonal process whereby the professional nurse practitioner assists the individual, family and community to promote mental health to prevent or cope with the experience of mental illness and suffering and if necessary to find meaning in these experiences. A total of 15 significant others were covered; they were randomly selected regardless of age, gender, race and relationship to patient. The respondents selected were those significant others who were able to take care of their patients for at least one week. The researchers conducted this study in order to determine the extent of nursing care implemented to psychiatric patients under seclusion in a mental rehabilitation center. This paper is guided by the use of Virginia Hendersons theory. METHODOLOGY This study used quantitative descriptive design. The study was conducted in a mental rehabilitation center located in Dumaguete City. The target population of this study were the significant others of patients under seclusion in the said institution for the year 2010-2011. A total of 15 significant others were covered.

They were randomly selected regardless of age, gender, race and relationship to patient. The respondents selected were those significant others who were able to take care of their patients for at least one week.

RESULTS Results from data gathering showed that 11 out of 17 nursing responsibilities enumerated by the group were always implemented by the nurses to patients in. It has a weighted mean that ranges from 4.3-4.9. Five out of 17 nursing responsibilities were often implemented by the nurses. It has a weighed mean that ranges from 3.6-4.1. However, one nursing responsibility is seldom done by the nurses. This responsibility is to assess the patients blood pressure, pulse rate, respiratory rate and temperature every shift. It has a weighted mean of 2.2.

CONCLUSION Results from the data gathering shows that eleven out of seventeen nursing responsibilities which include the social, spiritual and some of the physiological and psychological aspect were always implemented by the nurses to the psychiatric patients under seclusion. However, there were also irregularities in the implementation of such responsibilities. Thus, the assumption of the researchers has been supported by the results of the data gathered and analyzed that ideal nursing care is not always implemented. REFERENCES

RECOMMENDATION 1. That the staff nurses in the area will develop a strategy that will enable them to monitor and record patients vital signs every shift. 2. Those future researchers will utilize the results of this study to explore the factors that hinder the staff nurses to assess the vital signs of the patients. 3. That student nurses to be assigned in the area will strive to provide the ideal care that each psychiatric patient in seclusion should receive.
