Te new FRCR part 1 Anatomy examination
comprises 20 cases/images, with fve ques-
tions about each. Te cases are labelled 01 to
20 and the fve questions are labelled (a) to
(e). Te authors have set out to emulate this
format by gathering 200 cases which, from
their experience, are representative of the
cases on which candidates will be tested. Te
book consists of 10 tests with 20 cases each,
and 5 stem questions each. Te answers, along
with an explanation and tips, accompany each
test at the end of the chapter. Tis will help
candidates to identify the level of anatomical
knowledge expected by the Royal College of
Radiologists. Te aim of this book is not to
replace the already available literature in ra-
diological anatomy, but to complement it as a
revision guide. Whereas radiological anatomy
atlases and textbooks provide images with
labels for every possible identifable structure
in an investigation, the cases in this book have
only 5 labels, simulating the exam.

Test 1 Answers
Case 1
(a) Arch oI the aorta
(b) Oesophagus
(c) Trachea
(d) Right subscapularis muscle
(e) Azygos lobe/ssure
This is the appearance oI a collapsed oesophagus which is always Iound behind a much
more easily recognised trachea.
An azygos lobe is a normal anatomical variant Iound in 1 oI people. It is separated
Irom the rest oI the upper lobe by two Iolds oI parietal and two Iolds oI visceral pleura.
Case 2
(a) Quadriceps tendon
(b) Patellar ligament
(c) HoIIas Iat pad (inIrapatellar Iat pad)
(d) Posterior horn oI lateral meniscus
(e) Neck oI bula
This sagittal MRI oI the knee is taken through the bular head; thereIore, the meniscus
must be the lateral meniscus
Case 3
(a) LeIt maxillary sinus
(b) Right Ironto-zygomatic suture
(c) Right coronoid process oI mandible
(d) Right mastoid air cells
(e) Odontoid process (dens) oI C2 vertebra (axis)
All answers have two marks awarded. Always label the side when possible. Even iI you
get the s ructure right you will only be awarded one point iI the side is not included in the
answer.

Case 4
(a) Superior mesenteric artery
(b) Splenic vein
(c) LeIt renal vein
(d) Body oI pancreas
(e) Abdominal aorta
Look Ior the tadpole shape oI the splenic vein (tail) and portal conuence (head). The
pancreas is located anteriorly to the tadpole.
To distinguish the aorta Irom IVC: The aorta lies to the leIt oI the IVC, is smaller in diam-
eter and is surrounded by a concentric echo-bright area which represents peri-arterial Iat.
Case 5
(a) Right rectus abdominis muscle
(b) LeIt external iliac artery
(c) Right gluteus maximus muscle
(d) Coccyx
(e) LeIt obturator internus muscle
When presented with an MRI case, rst, it is important to identiIy the sequence. A useIul
hint is to remember that uid is bright on T2 weighted, Iat is bright on T1 weighted
images.
Case 6
(a) Right lateral epicondyle oI humerus
(b) Right capitellum oI humerus
(c) Right neck oI radius
(d) Right biceps brachii muscle
(e) Right shaIt oI ulna
Case 7
(a) Right styloid process oI ulna
(b) Right trapezium
(c) Right capitate
(d) Right lunate
(e) Right head oI thumb metacarpal
Phalanges and metacarpals should be named (not numbered) according to the correspond-
ing digit, e.g. thumb not rst metacarpal.

Case 8
(a) Right hemidiaphragm
(b) LeIt head oI clavicle
(c) Anterior border oI heart
(d) Arch oI aorta
(e) LeIt main bronchus
This barium swallow image is taken in the right anterior oblique position. Three major
impressions in the oesophagus are seen anteriorly. These are made by the aortic arch, the
leIt main bronchus and the leIt heart chambers Irom above down.
Case 9
(a) LeIt subclavian artery
(b) Trachea
(c) LeIt common carotid artery
(d) Right supraspinatus muscle
(e) Spinal canal
The supraspinatus muscle is superior to the spine oI the scapula and thereIore medial to it
on axial section.
Case 10
(a) Genu oI corpus callosum
(b) Suprasellar cistern
(c) Straight sinus
(d) Pituitary gland
(e) Clivus
There are six subarachnoid cisterns which are identiable on a sagittal MRI oI the brain
(Suprasellar cistern, Interpeduncular cistern, Pontine cistern, Cisterna magna, Quadrigeminal
cistern, Cavum velum interpositum).
Case 11
(a) Lens oI right eye
(b) Basilar artery
(c) LeIt posterior cerebral artery
(d) LeIt optic nerve
(e) Quadrigeminal plate (leIt superior colliculus)
Vessels in MR are represented as signal void (low signal) because oI ow arteIact. The
posterior cerebral artery is Iound in the ambient cistern.
