Micro-Abrasion: another alternative to Dental Whitening

Dental whitening is a treatment that many dentists are doing due to the large popularity this procedure has, because of the expectations of everyday more demanding patients regarding esthetics. Patients want whiter and brighter smiles in order to look younger and more attractive according to current standards of physical beauty. There are many materials and procedures in dental whitening but basically it is done with the use of different types of peroxides with different concentrations according to their needed use, not only using them for home treatments but also for dental office procedures with higher concentrations of this materials, where some of them are self-activated and where others require external activation factors like light and/or heat to become effective. However, sometimes the use of this materials is not enough, like in fluoride and other minerals or chemicals that have caused tooth stains. Sometimes it is possible to remove this dark or light stains using micro-abrasive techniques. Micro-abrasion may also work to remove stains that are resistant to professional prophylactic techniques, such as in the case of heavy tobacco stains and others that have already penetrated the enamel. It is very important to notice that the use of this technique, should be limited to healthy enamel because in some cases of fluorosis the quality of the enamel is poor and it is so fragile that it can be damaged with even a small amount of pressure. For this reason, the technique should be limited to superficial stains. It is also important to not create unrealistic expectations for the patient since the prognosis for a good outcome with deep stains is poor. A previous examination of trans-illumination of the tooth can give us an idea of how deep are these pigmentations. The combination of micro-abrasion with conventional whitening in general improves the appearance of the patient and in the majority of cases patients are satisfied with the results. The technique consists of the use of a heavy paste called Densell Lustre that contains abrasive agents made from Silicon Carbide mixed with Hydrochloric acid. As we know, inadequate use of this type of acid in the mouth presents a number of risks to the patient like cause burning lesions in the oral cavity as well as in the face and eyes. This is why office made mixtures are not recommended, these materials come in precisely measured quantities and exact consistencies that allow safe use by a professional. However, in all cases eye protection for the patient and professional should be used as well as adequate labial and lingual retraction. Once the necessary precautions have been taken, a very small amount of material is placed directly over the stains. Treat with a low speed contra-angle by rubbing the material over the surface of the tooth using a medium prophylactic brush. Use caution to avoid spilling particles outside the dental tissue. The use of high volume vaacum ejectors is recommended to remove excess material. The time of exposure to the material should be short so it is placed and rubbed on the surface of the tooth for a maximum of 2 or 3 minutes, at a rate of 3 or 4 times per session. Rinse thoroughly between each application to make sure that no material remains. At the end of each session of 3 or 4 applications the enamel is polished with aluminum oxide discs and the patient is asked to rinse well in case any residue remains in the oral cavity. For the best results this technique can be combined with at home carbamide peroxide bleaching to help lighten dark stains and to leave a more uniform color. This often happens in the presence of white stains which sometimes do not disappear completely but when the tooth is lighter in color, they are less visible. After the first or second session the patient can be given the device and whitening materials for use at home to accelerate the process and reduce the number of sessions of micro abrasion. The treatment be finished with a whitening in the office if it is considered necessary and if there isnt significant sensitivity or contraindications avoiding unnecessary procedures for the patient. El uso de blanqueadores en odontologa es un tratamiento que realizan muchos profesionales debido a la gran popularidad que este procedimiento tiene gracias a las expectativas cada vez mas exigentes que tienen los pacientes respecto a la esttica de su persona esperando tener una sonrisa blanca lo que la hace mas juvenil, limpia y agradable de acuerdo a los estndares actuales de la belleza fsica. Existen muchsimos materiales y procedimientos para realizar blanqueamientos dentales pero bsicamente se trata de la utilizacin de diferentes tipos y concentraciones de perxidos ya sea en guardas para tratamiento casero o por tratamientos realizados en el consultorio con materiales a mas alta concentracin algunos de los cuales son auto activados y otros requieren factores coadyuvantes como luz y/o calor para hacer efecto. En ciertos casos sin embargo el uso de estos materiales no es suficiente. En casos de fluorosis a veces puede ser posible el quitar manchas obscuras o blancas por medio del uso de tcnicas de microabrasin. Es tambin posible su uso en manchas que no ceden a la profilaxis profesional por ejemplo en el caso de tabaquismo y otros agentes externos que ya han manchado y penetrado el esmalte. El uso de esta tcnica sin embargo deber ser limitada a esmalte sano y resistente ya que en ciertos casos de fluorosis la calidad del esmalte es muy pobre y es muy frgil al grado de que se puede romper fcilmente con solo hacerle presin con un instrumento dental, por lo mismo deberemos limitar este procedimiento a casos de manchas superficiales. Antes de empezar debemos saber que hay pigmentaciones muy profundas y con un pronstico bajo de xito por lo que hay que tener cuidado de no crearle falsas expectativas al paciente. Un examen previo de transiluminacin del diente nos puede dar una idea de que tan profundas son estas pigmentaciones. Sin embargo la combinacin de micro-abrasin con el blanqueamiento convencional por lo general aumenta de una manera muy significativa la esttica del paciente y en la mayora de los casos tenemos pacientes muy satisfechos. La tcnica consiste en la utilizacin de una pasta de consistencia pesada llamada Densell Lustre que contiene agentes abrasivos a base de carburo de silicio con un cido clorhdrico. Como sabemos la utilizacin de este tipo y concentracin de cido en boca implica una gran cantidad de riesgos por lo que no se recomienda usar preparaciones caseras que pueden causar quemaduras no solo de la regin oral, sino de cara y ojos del paciente. Las presentaciones profesionales de estos materiales vienen en una consistencia y proporcin exacta que nos permite manejar este procedimiento con mayor seguridad. Sin embargo siempre se deber utilizar proteccin de ojos para paciente y operadores al igual que el mejor aislamiento posible como retractores de labios y lengua. Una vez que se han tomado las medidas de proteccin necesarias se coloca una muy pequea cantidad del material directamente sobre las manchas a tratar y con un contrngulo de baja velocidad se frota el diente con el material utilizando un cepillo de profilaxis mediano con cuidado de no volar partculas fuera del tejido dental. Por lo mismo se recomienda tener la cnula de succin de alto volumen cerca para atrapar el material expulsado. El tiempo de exposicin al material deber ser corto as que se colocar y frotar por dos o tres minutos mximos por tres o cuatro veces por cada sesin. Se enjuaga abundantemente entre cada aplicacin y revisamos que no exista material remanente. Al final de la sesin de tres o cuatro aplicaciones se pule el esmalte con discos de xido de aluminio y se pide al paciente que se enjuague bien por si hay residuos de materiales en la cavidad oral. Para mayor efectividad se puede combinar esta tcnica con blanqueamiento a base de perxidos para ayudar a aclarar las manchas oscuras y tambin para dejar un color mas uniforme como sucede en el caso de la existencia de manchas blancas que por lo general no desparecen en su totalidad pero al tener el diente un color mas claro estas son menos visibles. As que desde la primera o segunda sesin se le puede entregar al paciente su aparato y materiales de blanqueamiento en casa para acelerar el proceso y disminuir el nmero de sesiones de micro abrasin. Igualmente se puede concluir el tratamiento con un blanqueamiento en el consultorio si es que as lo requiere y si no hay sensibilidad extrema o contraindicaciones para este procedimiento evitando tratamientos innecesarios para el paciente.

With Clorhexidine. Combines the properties of tissue conditioning of Dentusoft with a long lasting antibacterial to improve the healing process. Ideal for treatments of implants. Assures the inhibition of the growth of microorganisms during every treatment. Microbiologically tested. Its indicated for treatment of painful mucous or damged tissues, by the use of new dentures or delicate tissues, relining for pre-protetics surgery, post implant, artificial gingival construction in dentures, or dentures on implants. It has a disinfectant effect, sustained-release and helps the process of healing. Dentures relined with Dentusoft Bacter with chlorhexidine are not irritating and more acceptable by patients. For lower dentures, especially in bilateral partials, the elasticity and strenght of Dentusoft Bacter with chlorhexidine is such that the functional load is equally distributed to the tissue, eliminating irritations. Recommended for soft peripheral border. Dentusoft Bacter with chlorhexidine is a temporary use material. Bonds perfectly to a clean denture base. Easy to remove at the end of the treatment and it does not affect the original structure of the denture. Con clorhexidina. Recomendado para bordes perifricos blandos. Dentusoft Bacter con clorhexidina es un material de uso temporario. Se adhiere perfectamente a la base de la prtesis limpia.
