Medical Assistance
Every C&I staff is entitled to adequate medical care and is enrolled with Total Health Trust (THT), which is a Federal Ministry of Health approved HMO. Under this scheme you would be allowed to pick a hospital from a list of carefully selected hospitals in your locality so that you can get prompt medical attention whenever the need arises. The THT enrolment form is attached with your staff documentation forms and should be completed and retuned to our offices for processing after which a medical card would be issued to you

As a staff you would be registered under a THT plan which allows you access to the following medical services: 1. Full unlimited general out-patient consultations 2. Specialist consultation 3. Supply of drugs and medicaments prescribed 4. Laboratory investigation 5. Radiology 6. Medical Hospitalization in general ward of designated hospital 7. Emergency treatment for injuries 8. Minor surgery 9. Basic Gynecology 10. Physiotherapy 11. Family planning

GENERAL EXCLUSIONS: The following medical services are not covered under the THT Gold schemes but offered under a discounted fee paying basis: 1. CT Scan 2. Dialysis 3. Specific treatment for HIV/AIDS 4. Complex Major & Tertiary surgery 5. Cosmetic Surgery 6. Prosthesis and other fittings 7. Investigation for Infertility 8. Treatment for Infertility 9. Eye glasses 10. Denture and special dental procedures 11. Cytotoxic and radiotherapy treatment.

EMERGENCY: In the event of emergency please follow steps below: Go to your chosen provider or any other provider in 1. the THT network and provide your THT registration details for immediate assistance

In the event that you cant get to your chosen THT provider or any other hospital on the THT network then please proceed to the nearest medical center for treatment. THT would make a refund as soon as you can provide your medical registration details

2. How do I apply for Leave?
Every staff is eligible for annual leave, however, a staff can only apply for annual leave after that staff has spent at least 12 calendar months in our employ. When you are due for leave all you need to do is request for a leave form which must be completed and returned to commence the process. If you require further details, please call/write us

4. How do I get details of my tax and pension remittances?
Monthly tax and pension deductions made on your salary is remitted to the FIRS and your choice PFA respectively. On Pension: Register with a Pension Fund Administrator of your choice and send your registration details (PFA account number) to C&I so that remittances can be made monthly. Whenever you require details of your Tax or PFA, please call / write us

6. I didnt get my salary, what should I do?
A bank information / PFA detail form is always attached to the set of forms sent to all new staff. Please ensure you get this forms properly completed and returned before the 13th of the month. All the forms that come in after the 13th of the month might have to wait till the next month to get on Payroll, however if you have completed your forms before the deadline but have not gotten your salary then you shouldnt worry because a special salary code would have been prepared for you already. This code would be sent to you as an sms with instructions on how to convert to cash. If you still have difficulties, please contact us immediately.

7. How do I get my ID card / THT card?
Every C&I outsourcing staff is given an identification Card and a medical card. If you have completed your staff documentation forms but still havent gotten this cards then all you need to do is alert us. All employee documents are usually dispatched via courier, however in the event that your are yet to get yours, please call or write us immediately But remember that you need to complete and return all staff documentation forms first.
