ANAMNESIS

A. INTERVIEWS The were no informants or significant persons to be interviewed to get some information about patient A B. FAMILY HISTORY a. Siblings The family is composed of four siblings; Patient A is the 2nd to the youngest. No further data was gathered because of the lack of informants to speak for the client i. Prenatal No information regarding the pregnancy of patient As mothers pregnancy. ii. Birth Patient A was born in Cebu City on March 30, 1975. b. Infancy and Childhood Characteristics Patient A was the 2nd to the youngest among the 4 siblings. Patient A has friends during his preschool age and was also involved in fights with his childhood friends c. Psychosexual history The patients sexual awareness started when he was 14 years old, on his 2nd year in high school. It was on this time that he started having a crush and actually had a girlfriend who after sometime broke up with him. This break-up with his girlfriend bagged down his self esteem. In his adolescence, he committed rape cases.

d. School history Patient A finished his elementary education in Panabo Elementary School. When he was in high school he studies at Alejandra Navarro National High School but on his 4th year level he wasnt able to finish his studies due to financial constrain and lack of interest wala na ko ganahi ug eskwela unya nag binugoy-bugoy ra pud ko, dayon sige ra kog uban as akong mga barkada as verbalized by patient A.

e. Religious and Social Adaptability Patient A and his family is a Seventh Day Adventist by faith. He also claimed that he was a drinker, a cigarette smoker and also a drug user during his adolescent period. f. Occupational History When the patient stopped studying during his 4th year in high school, he joined a gang group were they hold-up a passenger and snatch the bag or jewelries. g. Marital History The patient is single. However, he is looking forward to marrying someday. e. Onset of the present illness Patient A was admitted for several times already at Southern Philippines Medical Center with the last admission at April 2002, how ever patient claimed to obscured and have not received any measure since. Four months prior to admission, patient A claimed to murder 1 bystander for argument about a cigarette smoking and was arrested and jailed. However, in jail patient A claimed to murder 1 inmate after a drinking binge. Patient A stayed on jail for 1 month and 3 days and was referred and subsequently admitted at Southern Philippines Medical Center. 10 days prior to admission patient A claimed to obscure again and stayed with relatives at the province. Relatives were worried about the patients condition and behavior and told the patient to come back to Southern Philippines Medical Center, hence patient A come for this admission.
