Your Request for Ballot Secrecy Folder Sample

compliance with the Panel's Memorandum dated April 13, 201.0, the undersigned hereby submits the first prototype of the Ballot Secrecy Folder that was given to him by OTC Paper Supply. Also submitted is the covering letter of OTC dated February 5,2OI0, addressed to Chairman Jose A.R. Melo, through the Committee on Specifications. The prototype and covering letter were received at 1-0:45 a.m. of February 9,201,0.
Since suggestions were made by the Commission En Banc on how to make the folder more user-friendly, a second prototype was submitted, together with the specifications, during the February 17, 201.0 En Banc meeting. The second prototype and specifications, both of which incorporated said suggestions, were approved by the Commission En Banc on the same day. The approved second prototype was surrendered by the undersigned to the Commission En Banc.

As regards the Panel's April 9, 2OI0 directive for the undersigned to submit all memoranda and documents related to the procurement of the ballot secrecy folders, said memoranda and documents would be the same ones that would be obtained by the Panel from either the Bids and Awards Committee or the Office of the Commission Secretary. The undersigned will, therefore, no longer submit the same.
