INSTRUCTIONS FOR CANDIDATES (Please read carefully before filling the form)
1. Each candidate has to submit the application on the prescribed form. Separate application form is required for each category. 2. The application should be filled in by the candidate in his/her own handwriting in block letters with BLACK INK or BALLPOINT. Fill all the columns and write Not Applicable if column is irrelevant. 3. O/A level candidate should mention equivalent Marks Obtained and Maximum Marks in the Admission Form awarded by the Inter Boards Committee, Islamabad. 4. If a candidate is admitted on the basis of statements made in the above application and subsequently it is found that any of the statements was false, the candidate shall not be admitted and if admitted, he/she will be expelled from the University and all fee and other dues paid by him/her to the University upto that time shall be forfeited. The student and his/her father/guardian would also be liable to any further departmental or legal action that the University may deem fit to take. 5. The University Administration has the right to fine, suspend, rusticate, relegate or terminate the studentship of the student who found guilty of: i. Say or do anything which might adversely affect the honour and prestige of Pakistan, or University and teachers and his/her educational institution; ii. Willfully and deliberately damaging or misusing the University property; iii. Non possession of Student Identity Card in the vicinity of the University; iv. Providing fake documentary proof for Medical Certificate, Fee Concession, ID card etc.; v. Fail to submit the Fee/Dues within the stipulated time frame; vi. Smoke in the Classroom, Laboratory, Workshop, Library and Examination Hall; vii. Form, or associate with, an Organization/Society/Club, or any other body, promoting caste distinctions and inciting parochial/linguistic/regional feeling; viii. Organize, or hold any function within the precincts of the University except in accordance with the prescribed Rules and Regulations; ix. Collect money or receive donations or pecuniary assistance for or on behalf of the University or any University Organization except with written permission of the Competent Authority; x. Stage, incite, or participate in, a walkout, strike or any other form of agitation which might create or is likely to create law and order problem for the University and affect or is likely to affect its smooth functioning; xi. Indulge in immoral activities, use indecent language, make indecent remarks, jokes or gestures or behave in an improper manner; xii. Keep or carry weapons, narcotics, immoral or subversive literature; and xiii. Use insulting or abusive language or resort to violence against a fellow student or employee of the University. xiv. I solemnly declare that I am taking admission on Regular / Self Support / Full Payment / Foreign / Expatriate basis with my own will. I solemnly declare that I will pay all dues including Regular Fee and Full payment/Foreign charges, regularly for the remaining period of my studies and in future will not claim any type of Concession or Waiver of any type of fee/dues/Charges payable towards me regarding my studies and will not submit any type of claim in any court of law in any case All the particulars mentioned in my application for admission and documents submitted are true and correct and I fully understand that if any of the statements made in the application is found to be incorrect, my admission in University of Sargodha, may not be made or if already admitted, would be liable to be expelled at any time during the course of my studies. In that case all fee and other dues paid by me shall be forfeited and any further departmental or legal action be taken which the university may deem fit to take. I also solemnly declare that, if admitted, I will abide by the discipline, rules, and regulations of the university as enforced at present and made from time to time by the university authorities in future. I will concern myself only with the academic activities and such extra curricular activities, which are allowed by the university for the healthy growth of body and mind. I undertake that I will not take part in any political activity or agitation and I will not involve in matters of discipline. The decision of the competent authority will be final and binding on me and it will be challenged/questioned only before the Supreme Court of Pakistan vide judgement/order of the Supreme Court of Pakistan dated July 01, 1992. I will be regular in paying university dues and will be punctual in attending my classes. I will not remain absent from teaching programs without prior permission of the university authorities. I undertake that so long as I am a student of the university, I will do nothing either inside or outside the university and its premises that may interfere in its orderly administration and discipline or may bring the university or its administration into disrepute.
