Change of Custody form for each child whose custody you want to change. What You Have to Do Fill out the Petition form(s) and make 4 copies of each. Make sure the information you put in the blanks is true and accurate. By signing the petition you are swearing that all of the information in it is true. You will file the Petition, along with the Affidavits and the Notice of Motion form, at the clerks office at 28 North Clark, 2nd floor. The clerk will stamp them and give you a court date. How to Complete the Sheet The number of each instruction matches the same number on the Petition for Change of Custody Form. Double click on the blank lines in the form to highlight the line, then press delete. The line should disappear. Then, type in the required information according to the numbers below. 1) Write the child's name here. 2) Write the case number here. Use the same case number that was on your original parentage order. A copy of that order should be attached to the Petition. If you do not have a copy of this order you can get one at the Clerk's Office at 28 North Clark, 2nd floor. 3) Write your name here. You are the Petitioner on these forms and the other parent is the Respondent.

4) Write the date on which your Parentage Order was entered 5) You must delete either able or unable. If the Respondent is working, he or
she will have to pay child support and you must delete "unable." If the Respondent has no income, delete "able."

6) Write the amount the other parent, Respondent, will pay for child support. 7) Write the name of the parent who had custody of the child. 8) Write your name in the second blank. 9) Sign your name in pen. 10) Sign your name on the Certification. The Certification is part of the Petition and
swears that you are telling the truth about the things you wrote. 11) Write the date here. 12) Write your name.
