MMS 8th Grade Fall Field Trip

As part of the 8th grade social studies curriculum, we will be traveling to Cantigny in Wheaton, Illinois. The students will have the opportunity to explore both the historic house museum and the First Division Military Museum. WHEN: WHERE: COST: DUE: Wednesday, September 14th CantignyHistorical House and Military Museum $7.00 SEPTEMBER 8, 2011 Please return the permission slip and payment by this date.

ATTENTION: Students must bring a brown bag lunch with a drink, wear closed-toed/tennis shoes and wear comfortable outdoor clothing. Dress for the weather because we will go rain or shine! Chaperones Needed!! We will need one chaperone for every ten students to accompany us on our field trip. Without your help, we will not be able to attend. Please contact your childs homeroom teacher if you can attend. Call (847) 526-6210 or email us at mms8thgradeteam@d118.org. Please fill out, sign (on back) and return this form with payment (cash or check to MMS) in a sealed envelope.

If I wish for my student to receive any medication on this trip, I must either submit a Field Trip Medication Information Form or Extended Trip Medication Form to my students school. I hereby waive any claims against the Board of Education and its individual members, employees, agents, and assigns arising out of my students attendance on this student trip and agree to hold harmless and indemnify the Board of Education and its individual members, employees, agents, and assigns, either jointly or severally, from and against any and all liability, claims demands, damages, or causes of actions or injuries, costs, and expenses, including attorneys fees, resulting from or arising out of my students attendance on this student trip.
