Youth Provision Consent Forms for 2011 / 2012
Dear parent / guardian Please find enclosed a variety of consent forms that we require to be filled in and returned to us, to enable your child / ren to participate in CFC Youth activities throughout 2011/12. These forms are to cover all young people in secondary school years and up to 18 yrs of age.
All forms correctly filled in and handed in by Sunday 5th September will be stand the chance of wining a 15 iTunes voucher. I know, I cant believe it either!!!
These forms provide us with the opportunity to communicate effectively with you and / or your child as to what events are coming up understand any medical / dietary information provide a safer environment and experience for your child.
These forms will also be available on the Youth section of www.cfcbelfast.com as well as the main church Facebook page www.facebook.com/CFCBelfast Please either post them back to CFC, or drop them in at the Resources Desk at the back of church, marked for my attention. Note, for outside trips, including residentials, separate forms relating to those specific activities will be provided to enable your child/ren to participate. Communication with Young People

The nature of communication will be primarily to inform, or remind young people of upcoming events. At times there may be communication of a pastoral nature within the boundaries of our child protection guidelines. As the role of multimedia and social networking grows, CFC will be committed to embracing youth culture and new technologies in way that enables us to relate to young people, be accessible to for them and to do this is a safe and appropriate manner. Currently, CFC staff, interns or volunteer working in CFC Youth are discouraged from accepting friend requests from anyone under the age of 18. CFC will operate a formal Youth page (http://www.facebook.com/cfcbelfastyouth) to post comments, hear about events etc. CFC Youth leaders and volunteers will also be signing a contract which ensures their own social networking sites and pages of personal information are of an appropriate nature. Having relationships with young people and being in close communication with them is an essential part of CFCs approach to youth work. Telephone calls, texting, email and face to face conversations are still acceptable (and encouraged) ways for young people to communicate with leaders and build relationships.

Youth CONNECT Groups are mid-week small groups of young people within their own school year group, led by at least two youth volunteer leaders. The focus of these groups is primarily faith-based, and serve as a fantastic way for young people to build friendships, ask questions, and journey with others. This year we are running Youth CONNECT GROUPS for school years P7 Year 14 (4th 6th form up to and including 18 years of age)
If you wish to be or continue to be in a Youth CONNECT Group, please tick this box and we shall be in contact with you soon with details of day / times / leaders.

To comply with the Data Protection Act 1998, we need your permission before we take any photographs or recordings of your child/children. Please answer the questions below, then sign and date the form where shown. We will not use the images taken, or any other information you provide, for any other purpose. We may use group images with very general labels, such as youth enjoying sport or making Christmas decorations. 6.
