The admission application form is to be used only by those candidates who are eligible and wish to apply for admission after qualifying CET- 2011. 10(a) Condition of passing +2 from U. T. Chandigarh is waived off in respect of wards of serving defence personal/ex-servicemen vide letter no. 19/1/3-IH(3)2007/18322 dated 14.09.2007. 11. No entrance test is required for candidates seeking admission against Foreign Indian Students (NRI) seats. NRI candidates shall be admitted on the basis of merit of their qualifying examinations the equivalence certificates for which is to be issued by Panjab University, Chandigarh. All the admissions against NRI seats in the Panjab University must be strictly limited to genuine NRIs and/or their sons and daughters. Second Counselling To be notified. (if need be)

Second Counselling will be conducted for the vacant seats, if any. There will be no Third Counselling. A waiting list of candidates upto 25% of the total seats will be prepared for filling up the seats, which may fall vacant subsequent to the second counselling. The application form may be downloaded from GMCH website: gmch.gov.in & payment of US $ 100 or ` 5000/- in case of NRI applicants and ` 500/- each for (General Category) and ` 300/- (SC Category) of applicants should be remitted alongwith application form addressed to the Co-ordinator, Centralized Medical Admission 2011. 2. Eligibility Criteria: a) MBBS U.T. Pool The applicant must fulfill the following conditions:i) Attained the age of 17 years on or before 31st December 2011. ii) Passed +2 (12th Class) examination from Schools/Colleges recognized by the Chandigarh Administration and situated in the UT of Chandigarh, as regular student of the said Schools/Colleges. He/She should have passed in the subjects of Physics, Chemistry, Biology & English individually, and must also have obtained a minimum of 50% marks in the aggregate of Physics, Chemistry and biology at +2 level in the first attempt. However, in the case of members of the Scheduled Castes, relaxation in marks not exceeding 10% shall be allowed; and iii) Condition of passing +2 from U. T. Chandigarh is waived off in respect of wards of serving defence personal/ex-servicemen vide letter no. 19/1/3IH(3)- 2007/18322 dated 14.09.2007. iv) Qualified in CET 2011 conducted by Panjab University, Chandigarh. No entrance test is required for candidates seeking admission against Foreign Indian Students (NRI) Seats. 3

b) B.D.S. The applicant must fulfill the following conditions:i) Attained the age of 17 years on or before 31st December 2011. ii) Candidate having passed 10+2 examination of the Punjab School Education Board/CBSE, New Delhi or its equivalent examination conducted by a recognized Board/University/Council, as recognized by the syndicate with at least 50% marks in the aggregate of Physics, Chemistry and Biology (Botany and Zoology) provided he/she has qualified in English of 10+2 standard. However, for students belonging to reserved category (SC/ST), the minimum marks for admission shall be 40% as per rules and regulations of Dental Council of India. iii) Qualified CET-2011 conducted by Panjab University. iv) For eligibility criteria for NRI seats please see Sr. No. 7 of Page 6. v) In case no candidate from District Hoshiarpur (Preferably of Judge (Jaj) Community) is available, these seats will be filled through open merit. c) B.A.M.S. The application must fulfill the following conditions:i) Attained the age of 17 years on or before 31st December 2011. ii) Candidates who have passed 10+2 examination from recognized Boards/Bodies/Councils or any other examination recognized as equivalent to 10+2 examination by the Panjab University with Physics, Chemistry and Biology obtaining at least 50% marks in the aggregate, provided he/she is qualified in English of 10+2 Standard. Admission will be based on Merit in CET. However, in case of candidate belonging to the Scheduled Castes, relaxation in marks not exceeding 10% shall be allowed; iii) Qualified CET 2011 conducted by Panjab University.
NOTE:- Counselling/Admission will be subject to the approval from Govt. of India on the recommendation of Central Council of Indian Medicine, New Delhi.

d) B.H.M.S. The applicant must fulfill the following conditions:i) Attained the age of 17 years on or before 31st December 2011. ii) Passed 10+2 examination from a recognized Board/Body/Council or any other examinations recognized as equivalent to 10+2 examinations by the Panjab University with Physics, Chemistry, Biology and English at +2 level; iii) Qualified CET 2011 conducted by Panjab University.
3. Distribution of Seats: - The distribution of seats for MBBS/BDS/BAMS/BHMS for various categories is as under:-

Spot Payment at the time of admission. The fee for all the courses is required to be deposited on the spot, after the counselling on the same day. B.D.S. * Indian Nationals ` 85,867/- per annum Foreign Indian Students (NRI) US $ 7240 + US $ 650 as NRI Registration Fee + ` 19502/- as other usual charges in the first year * IMPORTANT NOTE (FOR BDS CANDIDATES):1. Admitted students will be required to pay full fee at the time of admission. 2. Selected Candidates will have to make spot payment and no extra time will be given. Payments for the above mentioned fees will be accepted through Bank Draft (in the names of Registrar, Panjab University, payable at State Bank of India, Sector 14, Chandigarh). 5

The Candidates will have to pay the exact amounts as per P.U. fee structure prevailing at the time of admission. The amounts mentioned above are approximate. Therefore, the candidates are advised to bring sufficient amount to meet with, if there is increase (any). The amount over and above the fee structure, if any will be accepted in cash only. 4. The fee structure if changed by the University from time to time shall be applicable accordingly. 5. Candidates, who want to avail the hostel facility, are advised to bring an extra amount of ` 6000/- (approx.) to cover the hostel charges. 6. No entrance test is required for candidates living abroad and seeking admission against Foreign Indian Students (NRI) seats. They shall be admitted on the basis of merit of their qualifying examination, the equivalence certificates for which is to be issued by Panjab University. 7. Candidates desirous of seeking admission under the category of Foreign Indian Students (NRI) candidates, who are present in India will be admitted on the basis of merit in CET- 2011 if otherwise eligible as per Panjab University Norms. NRI candidates living abroad shall be admitted on basis of their Scholastic Aptitude Test II (SAT-II) and comply with the requirements of Govt. of India as well as Panjab University as prescribed for them from time to time. BAMS ` 50000/- p.a. + ` 5000/- (refundable) security advance Category General /SC/ST BHMS Category ` 50000/- p.a. + ` 5000/- (refundable) security advance General /SC/ST Notes:A) No other charges like medical charges/building fund etc. are to be charged. B) The fee structure is subject to revision by the competent authority irrespective of details given in the brochure. C) Refund of Payment:- The Candidates who are admitted to the Course and want to surrender the seat prior to the last date of admission schedule approved by the Panjab University, may be refunded the entire amount of fees & funds collected at the time of counselling after retaining ` 1000/- (` One thousand only) as administrative/ processing charges provided the vacant seats gets filled up. For BDS students, refund of fees will be as per Panjab University Rules/Norms. D) No charges are refundable if the seat remains unfilled. E) In case a candidate of BAMS/BHMS vacates the seat after the last date fixed by the Panjab University as per its schedule he will have to submit an indemnity bond of ` 50000/- in favour of College Principal for surrendering the seat to avoid financial loss to the managements of said colleges. A specimen copy of indemnity bond is appended with this brochure as Appendix F (Page No-16).

GUIDELINES FOR ADMISSION TO MBBS/BDS/BAMS/BHMS GENERAL RULES i) Candidates will apply for admission for the Course of their choice i.e. MBBS/BDS/BAMS/BHMS on the prescribed form for that course only as per details given below after determining their eligibility (Appendix GApplication form) Blue form MBBS Yellow form BDS Pink form BHMS Green form BAMS Admitted students will fill up admission form of the allotted College and complete other formalities as per the regulations of the concerned college. ii) Fees are to be paid on the spot on the day of the admission/counselling. iii) Candidates who fail to appear for interview-cum-counselling on the specified date and time will not be considered. The candidates must ensure that they must report at the counselling venue on the stipulated date on or before 9:30am sharp. iv) The candidates should bring with them the following original documents at the time of interview-cum-counselling and attach Photostat copies self attested with the application form. a) CET Result Card. b) 10+1 Certificate of the School/College concerned. c) 10+2 Certificate of the Board concerned. d) Matriculation certificate showing the date of birth. e) Affidavit attested by the Magistrate 1st Class/Notary Public, explaining gap in studies, if any. f) Character certificate from the institution last attended: 1. For MBBS Certificates as per Appendix A, B & C (Page 12-13). 2. For BDS/BAMS/BHMS Certificates as per Appendix A & C(Page 12-13). g) Proof for claim of reserved category, if claimed as per reservation rules. h) Three photographs of passport size (5 cm x 4 cm) i) Income Certificate showing total yearly income from all the sources issued by the employer in case of salaried parents/guardians and an affidavit attested by the Oath Commissioner in case of other non-salaried parents/guardians. j) In case of candidates belonging to Scheduled Castes & Tribes, a certificate to this effect, issued by the Tehsildar/Magistrate 1st Class/Deputy Commissioner of the area to which an applicant actually belongs. A certificate not bearing the seal of the competent authority will not be accepted. A specimen copy of the certificate is appended with this brochure as Appendix D (Page 14). v) Any seat(s), which remain vacant from any of the reserved categories shall be dereserved and added to the general category seats at the end of the counselling for the reserved categories. vi) The student belonging to a reserved category who obtains admission on the basis of his/her merit in the general category shall not be counted towards the seats reserved for the categories concerned.

vii) ADMISSION CRITERIA:a) Admission will be made strictly on merit for all categories of candidates. A separate merit list for each category will be prepared based on the marks obtained in Common Entrance Test 2011 conducted by Panjab University. The result of CET, ipso facto, does not entitle a candidate to get admission. b) In the event of a tie in the marks obtained in CET, a candidate getting higher percentage of total marks in the qualifying (10+2) examination in the subjects of Physics, Chemistry & Biology shall rank higher and if the marks in (10+2) examination in the said subjects are also the same, then the candidate older in age shall rank higher. c) No alterations will be allowed in the application form after its submission. d) In case the information furnished or certificates/documents submitted are found to be fake or the candidate is found to have withheld or concealed information in the application form, notwithstanding legal action under the law of the land, his/her admission shall stand cancelled and the fee deposited shall be forfeited. e) Applications received late or incomplete will be rejected. f) All legal disputes relating to admission of students will be subject to courts having jurisdiction in Chandigarh. g) A candidate who reports late on the day of counselling or on any subsequent date of counselling will not be eligible for the seat according to his/her original merit position, but will be offered a seat where the seats are available at that time. h) Any candidate who could not attend the first counseling due to any reason, he/she continues to remain eligible to appear and be considered for selection in subsequent counseling as per his/her CET Merit. i) It is mandatory for the candidates to attend counseling in person for admission at their own expense. No proxy is allowed. j) The candidature of the eligible candidates, who do not deposit the prescribed fees in full and original certificates immediately after selection, shall stand automatically cancelled and vacancies so caused shall be offered to the candidates next in order of merit. No correspondence or appeal in this connection will be entertained by the Admission Committee. The attested copies of the certificates or similar other documents will not be entertained for granting admission to a candidate to the MBBS/BDS/BAMS/BHMS Course. k) Candidates passing their (10+2) or equivalent examination from Boards/ Councils other than Schools Education Boards of Punjab, Haryana, Himachal Pradesh and Delhi State will have to submit migration certificate from the respective Council / Board within a period of one month from the date of enrolment. l) The original certificates of the selected students will be retained by the College and shall returned only if the seat is surrendered by the candidate, as per instructions received from Director General Health Services, Ministry of Health & Family Welfare, New Delhi vide letter No. F.No. U 12021/33/2209MEC (Pt.II) dated 03.02.2010. m) The selected candidates should be declared medically fit by the constituted medical board at the time of their admission to the course.

n) All admissions to MBBS/BDS/BAMS/BHMS Courses shall be provisional and subject to the approval by Panjab University. o) Central Pool seat for MBBS Course will be filled up on the basis of recommendations of the Ministry of Health & Family Welfare, Govt. of India, New Delhi. These recommendations are made by the ministry from names sponsored by various institutions as given below. Therefore applicants for Central Pool seat should apply to following authorities:Sr. No. Category Authority to whom the applications are to be sent 01. For MBBS the All India quota seats numbering 07 seats shall be filled through Directorate General of Health Services, Ministry of Health & Family Welfare, Govt. of India, New Delhi on the basis of pre-medical entrance examination conducted by the CBSE. 6. RESERVATION OF SEATS: (a) Union Territory Pool (MBBS): i) Scheduled Caste 14% i.e. 07 Seats ii) Physically 03% i.e. 02 Seats (1st preference to the candidate suffering form Handicapped Locomotor Disabilities of Lower Limbs between 50%-70%, 2nd preference will be given to the candidates suffering from Locomotor Disabilities of Lower Limbs between 40%50%, in case no suitable candidate from 50% -70% disability is available) 9

iii) Scheduled Tribe Nil b) B.D.S.: Seats: Open & Reserved The number of seats as announced in the Admission Notice shall be strictly adhered to, except for such minor adjustment as may be necessary on account of rounding off, of fractions to accommodate reservations. i) Scheduled Caste 15% i.e. 13 Seats ii) Scheduled Tribe 7.5% i.e. 06 Seats iii) Jaj Community 2 Seats Inter-Changeability & Transfer of Seats In the Reserved categories given the seats shall be regarded as interchangeable. That is to say, if a sufficient number of eligible candidates are not available to fill up the seats reserved for Scheduled Tribes, seats may be filled up from among the eligible candidate belonging to the Scheduled Castes, and vice-versa. i) In case sufficient number of eligible candidates, belonging to both SC/ST are not available, the vacant seats shall be transferred to the open merit category. ii) In case sufficient numbers of eligible candidates, belonging to Foreign Indian Students (NRI) are not available, the vacant seats shall be transferred to the open merit category. c) General Pool (BAMS/BHMS): i) Scheduled Caste 15% ii) Physically 03% i.e. 01 Seat (1st preference to the candidate suffering Handicapped form Locomotor Disabilities of Lower Limbs between 50%-70%, 2nd preference will be given to the candidates suffering from Locomotor Disabilities of Lower Limbs between 40%-50%, in case no suitable candidate from 50% -70% disability is available) Any seat(s), which remain vacant from any of the reserved categories shall be dereserved and added to the general category seats at the end of the counselling for the reserved categories. The above reservation shall be subject to the following conditions:a) The number of students belonging to reserved category but admitted on general merit shall not be included in the number of seats reserved. b) Candidates belonging to Scheduled Castes/Tribes are allowed a concession of 10% marks for determining their eligibility for admission to the courses in which a certain percentage of minimum marks has been prescribed provided they have obtained minimum pass marks prescribed by regulations. c) Production of SC/Physically Handicapped certificate from the competent authority. The Physically Handicapped certificate shall not be older than three months from the last date of submission of application. Foreign Indian Students (NRI) Seats: i) MBBS: NRI Candidates may apply for admission on the application form available at Appendix - E of the brochure (Page 15). Under the NRI Category 03 seats shall be filled up as per preference order given as under:-

a) First preference will be given to those NRI candidates who have ancestral background of Chandigarh refer note-4. b) Second preference will be given to those NRI candidates who have ancestral background of States/UTs other than U.T. Chandigarh. c) The Candidate against NRI seats will be admitted on the basis of merit determined by the marks obtained in Physics, Chemistry & Biology (PCB) in the qualifying examination. Eligibility & equivalence of these candidates is to be determined by the Panjab University, Chandigarh on the basis of qualifying examination equivalent to 10+2 examinations of PSEB/CBSE/ICSE. They are not required to appear in CET 2011. In case a seat under the NRI category remains vacant, the same shall be filled from General category candidates as per merit of CET. Note-1 If sufficient number of candidates under 1st category are eligible and available then they will be admitted first even if students under subsequent category are higher in merit. Note-2 The NRI students will have to give a bank guarantee for the balance fee, if they opt to pay fee in installments as enumerated on page no. 5 instead of paying in lumpsum i.e. US$ 75000/-. Note-3 The relevant provisions of this brochure are liable to change as per the directions of the Fee Committee & Admission Committees constituted in light of directions of Apex Court in the case of TMA Pai Foundation V/s State of Karnataka, Islamic Academy of Education V/s State of Karnataka. Note-4 For ancestral background of Chandigarh, the grandparents/parents of the candidates should be resident of Chandigarh for a minimum period of 5 years at anytime since the origin of Chandigarh and should have immovable property in his/her name in Chandigarh for the last at least 5 years. A certificate to this effect is required from DC-cumEstate Officer or Municipal Corporation of Chandigarh. A similar certificate from competent authority is to be submitted in case of students with ancestral background of other states/UTs. ii) BDS: NRI Candidates may apply for admission on the application form available at Appendix - E of the brochure (Page-15). a) Foreign Nationals/PIO/NRI candidates will also have to comply with any other requirement prescribed by the Govt. of India & Panjab University, from time to time. b) NRI candidates living abroad shall be admitted on the basis of the merit of their qualifying examinations the equivalence certificates for which is to be issued by Panjab University.
