Nursing Home
A Nursing Home/Community Hospital is a hospital that caters to the health care needs of the community. It is involved in preventive medicine and therapeutics. Setup A small nursing home typically has 4-5 beds, 1 intensive care unit (optional). 1-2 junior doctors, 1-2 senior specialists, couple of visiting specialist doctors, a pharmacist in charge of a dispensary, 1-2 nurses, 1-2 ward boys, 1 receptionist (usually is the cashier also). Value Creation 1. Reducing the disease burden on the local people. 2. Treating simple ailments 3. Administering vaccinations 4. Prevention and Social Education: This includes educating on the evils of alcoholism, drug abuse, etc. 5. Counselling: This includes fertility treatments, mental rehabilitation, psychiatric treatments etc.

Protagonists

Junior Doctor
Roles First point of contact to the patients. Handles all routine cases. Investigates some chronic ailments. Performs minor surgeries Challenges: Prioritize cases Educate the nurses, ward boys, pharmacist and receptionist on the protocols and procedures of the establishment. Time management Endures maximum work load, needs to balance work.

Specialist Doctor
Roles Co-ordinates between other doctors Maintains wards - decided which patients need to be admitted to the wards and when they may be discharged. He is usually the head of administration of the hospital. Also head of finances. Challenges Co ordination with other doctors - These might include general practitioners or other specialist doctors. He is involved in maintenance of wards and other in patient departments.

Nurse
Roles Follows the orders given by the doctors Responsible for administering medicines, food, IV fluids, cleaning and dressing Challenges Is accountable for timely care of the patients. Supervision of ward boys

Ward Boy
Roles Responsible for cleaning and maintenance Need to be meticulous while cleaning medical apparatus Challenges Transportation of critical patients. General maintenance of hospital like toilets, wards.

Pharmacist
Roles Dispenses the medicine Advices patients on dosage and intake procedures Informs them what needs to be done in case of skipped medicine or over dosages. Maintains stock of medical goods. Challenges Discarding expired drugs Prioritizing medicine based on cost and importance Proper storage of medicine

Receptionist
Roles Responsible for recording the patient chief complaint and prioritising cases. Needs to inform patient about time any possible delays Needs to record patient details Inform doctor in case of emergency Collect amount from patients Challenges Reasonably accurate assessment of the seriousness of a case. Managing anxious patients Dealing with patients who refuse to wait/pay

OB Model
Independent Variables
Individual level variables: Ability - The doctors and nurses must exhibit adequate ability in handling the cases. Values - Doctors and nurses need to adhere to strict ethical norms like not disclosing sensitive patient information. Attitudes - Every individual at the hospital must exhibit a passion to serve the community. Emotions - Doctors and nurses at times work under high stress. It is imperative that they maintain calm and keep a check on their anger and anxiety especially during times of emergency. Learning - Doctors should ensure that they are up to date with the latest developments especially the outbreak of epidemics and the measures adopted by government and other health agencies in handling it. They must also update their knowledge through short term refresher courses, seminars, conferences etc. Decision Making - A good decision can sometimes save a life. A doctor should be ready to make informed decisions and convey the same to the patients and their families. Biographical Characteristics - Age can be a factor that comes into play in a nursing home environment. An elderly doctor with good number of years in service may be reluctant to heed to the advice of a younger colleague. Group level variables: Team - The doctors and nurses must work as a team while treating patients. There must not be any form of ego issues that can cause hindrance to their work. Communication - Every member of the nursing home must communicate (both oral and written communications) clearly and without ambiguity. This can be crucial while dealing with sensitive cases. Leadership - A doctor should be a role model to the nurses, ward boys, receptionist and pharmacist. Power and politics - A senior doctor must refrain from exercising his/her authority over other staff unnecessarily. Conflict - All issues amongst medical and support staff must be resolved amicably and must not be allowed to interfere with the working of the nursing home.

Organization level variables: Structure - Every employee of the nursing home must know his/her duties and responsibility. They must also be aware of the roles played by other staff members. Policies - Every employee must adhere to the nursing home policies like dress code, response to emergencies, monetary benefits, leave etc. Practices - The nursing home as an entity must refrain from any form of malpractices like foeticide, inappropriate diagnosis and treatment, unnecessary surgeries. (these need to be classified into separate headings) Interpersonal relationship -The nursing home must be sensitive to the need of the staff and treat them with dignity. Objective - The nursing home should strive towards treating patients and satisfying its clientele.

Dependent Variables
Productivity - In a nursing home the measure of productivity could be how well the doctors are able to manage their cases. It could be measured by patient satisfaction. It could also be measured from the organisation point of view whether the management is happy with a doctors performance when compared to their salary. Absenteeism - This is a serious problem in nursing homes. In small setups there may usually not be any replacement for a particular employee. In such cases, absenteeism affects normal functioning of the nursing home. Typically people in nursing homes work 6 days a week on an 8 hour shift basis. Deviant Workplace Behaviour - Very rampant especially among ward boys and helper staff. There are various reasons for this. For example: drugs like morphine are freely available, and if not regulated will lead to addiction. Alcoholism is also a common issue. Organizational Citizenship - A nursing home setup would usually expect its employees to do beyond what their specific roles are. For instance, a doctor can assist a critical patient from one ward to another although that is the role of a ward boy. Also avoiding any form of conflict would go a long way in maintaining harmony in the nursing home environment. Job Satisfaction - This can vary from very high to very low. It is very subjective to every employee of the nursing home. Job satisfaction depends on the kind of workplace, the kind of patients, monotony of job, financial gains, etc.
